Objective: To investigate the relationship between the plasma levels of endothelin(ET),nitric oxide(NO), calcitonin gene-related peptide (CGRP),glucagon(GG) and portal haemodynamics with esophageal variceal bleeding (EVB) in cirrhotic patients with portal hypertension(PHT).Methods: The patients with PHT were divided two groups [EVB(+) and EVB(-) groups].Plasma levels of ET, NO, CGRP and GG were measured.Colorful Doppler ultrasonography was used to measure the portal haemodynamics, including portal vein diameter (PVD), splenic vein diameter (SVD), portal vein velocity (PVV), splenic vein velocity (SVV),portal vein flow volume (PVFV),splenic vein flow volume (SVFV),SVFV/PVFV, portal vein congestion index (PVCI) and splenic vein congestion index(SVCI).Results:(1) Plasma ET and CGRP levels were significantly higher in PHT patients than that in normal group (P<0.01),but plasma ET and CGRP levels were no significantly difference between EVB(+) group and EVB(-) group(P>0.05).(2) Plasma NO and GG contents were markedly higer in PHT patients than that in normal group (P<0.01). Plasma NO and GG contents were also markedly higher in EVB(+) group than that in EVB(-) group (P<0.05).(3) PVD, SVD, PVFV, SVFV, SVFV/PVFV, PVCI and SVCI were all signigficantly higher in PHT group than that in normal group (P<0.01), but PVV and SVV were significantly lower in PHT group than that in normal group (P<0.01).(4) PVD,SVD, PVV, SVV and PVFV were no markedly difference between EVB(+) group and EVB(-) group (P>0.05), SVFV, SVFV/PVFV , PVCI (all P<0.05) and SVCI (P<0.01) were all markedly greater in EVB (+) group than that in EVB(-) group.Conclusion: Vasoactive substances ET, NO, CGRP and GG were connected with PHT. The hyperdynamic circulation of splenic vein (SV)was main source of increased PVFV. Plasma NO and GG levels, SVFV, SVFV/PVFV , PVCI and SVCI may be valuable parameters in predicting occurrence of EVB.
