Utilizing the techniques, such as RT-PCR, immunohistochemistry, optical microscope, electron microscope and radio-immunity, the paper investigates the effect of the regulating factors of myocardial remodeling TNF-cu ICAM-K MMP-9> TIMP-1 and neuroendocrine on congestive heart failure (CHF), meanwhile, observes the intervening effect of the treatment of tonifying Qi, warming Yang, promoting blood circulation and resolving phlegm. This paper studies the mechanism of CHF and'the treatment on cellular and molecular level. The results show that:1.The hemodynamics is abnormal in experimental rats with CHF, the treatment improves the cardiac function through enhancing the level of dp/dtmax, dp/dtmin and LVSP, and increasing cardiac output, and lowering the heart rates, and improving the cardiac systole and diastole effectively.2. The neuroendocrine is abnormal expressed in CHF rats, the treatment antagonizes the abnormal activation of neuroendocrine, decreases the plasma concentration of Angll, ALD, ET and ANP significantly.3. ISO induces the upregulation of cardiac TNF-a, ICAM-lmRNA and protein expression. The treatment can decrease the mortality of cardiac cell, which is due to inhibiting the expression of TNF-a and induced ICAM-lmRNA and protein.4.The heart weight index increases in the CHF rats, the treatment suppresses the myocardial hypertrophy, which is related with lowering the plasma level of Angll , ALD and TNF-a.5.ISO induces the decreased expression of TIMP-1 mRNA andprotein, while, the over expression of TNF-cu MMP-9 mRNA and protein in CHF rats. The treatment up-regulates the expression of TIMP-1 mRNA and protein, and inhibits the expression of MMP-9 mRNA and protein induced by TNF-a, which maybe the therapeutic mechanism of inhibiting the ECM remodeling.6. ISO induces collagen proliferation and myocardial fibrosis. The treatment inhibits myocardial fibrosis through decreasing the plasma level of Angll, ALD and ET.7.The pathological morphology shows the congestion of liver, lung, spleen and kidney in the failing heart. The weight index of liver and spleen are increased significantly, while the treatment decreases the weight index and relieves the visceral congestion.8.The treatment can protect the ultrastructure and decrease the motality.9.Heart Yang deficiency is the pathological basis, and the blood stasis and phlegm (fluid overflowing) are the key pathological points of CHF. The treatment can inhibit the myocardial remodeling effectively, with the comprehensive regulation from the multiple genes, systems, targets and locations. Therefore, it is an effective therapy.
