Diabetic nephropathy(DN)is one ofthemainchroniccomplicantsofdiabetic mellitus(DM)aswellas the common reason resulting in chronic renal failure, about 30% or 40% of which are caused byDN .Systematic studies on the prevention and treatment for DN by JieDuTongLuoBaoShen capsule(JDTLBS capsule) had been performed from the aspects of reference reviews and experimentalstudy in this essay. This essay put forward the the theory that the main of Xiao ke nephrorosis is thedamnificationofKinneybypoison,thoughmanytheoreticalstudyonthepathologicalmechanismofit. A perfect system of differentiation of symptom-complex is set up. The clinical significance ofprevention changing and dissolution the toxin is proved great important. by the argumentation thatthe valid path of tread Xiao ke nephrorosis is the way of remove toxicity remove the obstructionsupplement the kidney. This essay summarized the experimental study results and advances on the principle ofpreventing and treating DN, according to the pathologenesis of DN, such as glucoregulationtubulence, fat matabolism tubulence, vascular active substance, growth factor, chemotactic factor,reactive oxygen stecies, experimental animal model and so on. It also included evaluateiosandprospects on these above. The results and progresses of study on the losing balance ofextracellular matrix (ECM), the matrix metalloproteinase(MMPs) system and peroxisomeproliferator-activated receptors (PPAR) were summarized involving the pathogenesis of DN recantyears.2 theory StudyBy discussing the headstream,name gist, Chinese diagnosis, differentiation of symptom-complextheimportantpointofdiagnosisandtreat,theimportantpointofknowingtoxin,principleandmethodof treat etc. of Xiao ke nephrorosis with Chinese medicine theories quoting from classics works andcombining modern research result,we put forward the the theory that the main mechanism of Xiaoke nephrorosis is the damnification of Kinney by poison and the valid path of tread Xiao kenephrorosisisthewayofremovetoxicityremovetheobstructionsupplementthekidney.3 Experimental study3.1 The influence about JDTLBS capsule to the renal function in experimental diabetic rats 34 解毒通络保肾胶囊对糖尿病大鼠肾脏 ECM 积聚及其相关受体分子调控研究STZ-induced diabetic rats were divided randomly into normal control group, diabetic model group,postive control group (DM+Rosiglitazone), experimental group (DM+JDTLBS capsule) in Chineseherbal.Thevariousmarketswereobserved.TheresultsshowedJDTLBScapsuleobviouslyimprovethe rats' general condition and had a certain effect on reducing blood sugar ,serum cholesteroal andtriglyceride,whichhadasignificantstatisticsenseincomparingwithdiabeticmodelgroup(P<0.05).Besides it could inhibit renal hypertrophy in early stage and high filtration of renal glomerulus aswell as decrease the experimental rats' higher secrum BUN, Sre, which had a significant differenceagainst diabetic model group (P<0.05). It also reduced the experimental rats' urinary proteinamounts in 24h, having a markedly difference against diabetic control group (P<0.05).
