The Study On Sorting Out Syndrome Elements And Their Association Law Of Diabetes Mellitus

Diabetes Mellitus(DM) has a strong hereditary component and is affected by various chronic, systemic, lifelong endocrine diseases. DM mortality rates are close to those of cardiovascular disease and neoplasia, its complications affect all the organs of the entire body and it is a serious threat to the health and life of humanity. Modern medicine's treatment efficacy of DM is extremely limited. The lack of consensus standardization of syndrome diagnosis and curative effect appraisement which greatly hinder the development of the TCM theory and clinical popularizing. Thinking of these facts and academic background, we studied the standardization of TCM syndrome for DM from syndrome elements and combination angles, made qualitative and quantitative researches to syndrome elements and their causal relationship by building data base, using data mining methods such as Classification Analysis, BayesNet, explore the syndrome elements and their combination law, in order to lay the foundations of standardization of syndrome differentiation, provide method and experience.Thesis contains four main parts. The first part is "the review of modern studies of TCM syndrome for DM". We made respective reviews of the study general situation to standardization of syndrome diagnosis and the relationship between syndrome and laboratory targets, found the syndrome differentiation of DM refer to different standard, which cause confusion of syndrome diagnosis and syndrome differentiation, hinder the development of clinical and scientific research to treat DM by Chinese herb;the medical terminology of symptoms and syndrome are not standardization which lay restraints on the clinical exchange;syndrome types are complicated and changeable that the present reference standard can't completely include them;a regular model of syndrome diagnosis standard can hardly fit in with the needs of the complexity of DM symptoms, which lead to errors and omissions of clinical syndrome differentiation. The problems of the relationship between syndrome and laboratory targets is there lack one to one correspondence between complex syndrome and laboratory targets. Advance that achieve the standardization of syndrome diagnosis by sorting out syndrome elements of DM. It maybe the new exploration to achieve micro- diagnosis of syndrome by study the relationship between syndrome and laboratory targets from syndrome elements angles.The second part is "the study of standardization of syndrome diagnosis",which mainly explore the relationship between dispersion-thirst disease and DM by syndrome elements their combination law, hope to provide references on developing theory and raising clinical efficiency of DM TCM treatment. The study on syndrome elements of dispersion-thirst disease is divided into three respects: first, made analyses on the appearing rate of the basic syndromes of dispersion-thirst disease according to those famous ancient doctors literatures, then sorted out syndrome elements;secondly, set up database from dynasties disease cases , launched research to the relationship between syndrome element and its subordinate symptom by using data mining method;thirdly ,sorted out syndrome elements from prescriptions of dispersion-thirst disease;Carried on comparative analysis of three results of study and finally established the key element: internal heat ,Yin asthenia, Qi asthenia, internal dryness, Yang asthenia, phlegm. The association law is that 6 kinds of syndrome elements can get 21 kinds of combination forms, having contained 91.33% of the disease cases to count, the clinical coincidence rate is good.The research of syndrome elements of DM is divided into the philology learning and the clinical reviewing case study: collect the articles about diabetes syndrome type research which published in the past 25 years, analyse the appearing rate of the basic syndrome types of diabetes from the expert's view and clinical cases information, and then drew its main key elements on one hand;Set about setting up the database from the clinic reviewing cases on the other hand, use data mining methods such as cluster analysis and Bayesnet to explore key element and the relationship with its subordinate symptom on the other hand.Carry on comparative analysis of two results of study finally, establish the key element: Yin asthenia, Qi asthenia, blood silt, internal hea internal dryness, Yang asthenia, phlegm. The association law is that 7 kinds of elements can get 53 kinds of combination forms which having contained 97.2% of the disease cases, the clinical coincidence rate is good.The third part " structuring the new mode of dialectical system of diabetes with the core of the syndrome element " has been proposed " phenomenon make up of element, elements make up of syndrome, disease treatment according to syndrome, the disease and syndrome are combined, the prescription and syndrome is corresponding ", it is a new thinking of perfecting the dialectical system of diabetes, its core is the key syndrome element. The research of the key syndrome element and it combined law will bring the study come back to base.
