Splenogastric hygropyrexia syndrome (SHS) is one kind of common spleno -gastric sthenia syndrome and plays an important role in the disease of digestive system. Studies on it is helpful to explore in the spleen essence. But the animal model duplication is very important to SHS' pathogenesis, the new drug research and development and enhancing clinical curative effect. In this study, we try to find out the biology background and occurrence pathogenesis of SHS by the way of the pathology inflammation, COX-2, Bcl-2 protein expression and cell apoptosis under the direction of traditional Chinese medicine (TCM) theory of "the dysfunction of spleen in transport and transformation, and the consequent stagnation of wetness and produce heat, and finally resulting in the stagnation of damp-heat pathogens in the spleen and stomach." At the same time, we observed the influence of QingZhuoAn -ZhongDecotion on index of correlation to SHS' rat model.1. Literature ResearchThrough the ancient literature review research, we think that SHS approximately experienced four stages which has laid a foundation, germinates, formation, consummates and has formed a set of more perfect theory system possessing theories, therapies, prescriptions and herbs. TCM theory think the etiological factor and pathogenesis of SHS lies in internal cause or the external factor leading to the dysfunction of spleen in transport and transformation, and the consequent stagnationof wetness and produce heat, and finally resulting in the stagnation of damp-heat pathogens in spleen and stomach. Removing heat and dispelling dampness, combining together with coordinating spleen and stomach is the main principle.With regard to the modern research of SHS, the clinical diagnosis standard are dazzling, but mostly same except for minor differences;The SHS animal model study are not many, but also has not appeared the standard model which is publicly accepted and duplicated;The SHS essence research involves to the epidemiology, the stomach and intestines shape and function, the inflammation, the immunity, the hormone, the microbiology condition and the HP infection, the cell energy metabolism, aquaporin protein and the gene regulation and so on .2. Clinic ResearchRelationship between SHS of Chronic Gastritis and Cell Apoptosis, Protein Expression of COX-2Objects: Choose 55 cases of patients with chronic superficial gastritis (CSG), 30 cases belong to the group of SHS, and 25 cases belong to the splenic asthenia group. Another 10 cases belong to the group of Normal health examination people.Method: In the gastric antrum part, the mucous membrane of all observed object was taken under the help of gastroscope. The inflammation situation of gastric mucosa were observed by using gastroscope and common pathological staining method.The protein expression of gastric mucosa COX-2 and Bcl-2 was detected by immunohisochemistry. The cell apoptosis of gastric mucosa was detected by the home position terminal sign notation.Results: The different degree hyperemia dropsy of gastric mucosa of patients with CSG and adhering mucus were observed under gastroscope .Compared with splenic asthenia, patients with CSG manifest more obvious hyperemia dropsy, many may be accompanied by erosion or hemorrhage, the stomach mucilage quantity to be many;The pathology examination all appear infiltrating inflammation cells and the lymphocytes. The inflammation cell infiltrating degree of SHS group and splenic asthenia group was stronger than normal control group (P<0.01), and has the obvious difference between SHS group and splenic asthenia group (P<0.05).The cell apoptosis index of SHS group and splenic asthenia group was obviously higher than the normal group (PO.05). The cell apoptosis index of splenic astheniagroup obviously was higher than SHS group (PO.05). Bcl-2 protein expression in SHS group and splenic asthenia group is stronger than that of the normal group (PO.05);Bcl-2 protein expression of splenic asthenia group is lower than SHS group (P<0.05). The COX-2 protein expression of SHS group and splenic asthenia group is stronger than the normal group (P<0.05);SHS group' COX-2 protein expresses strongly is splenic asthenia group (P<0.05).HP masculine infection rate in the SHS group and splenic asthenia group is higher than normal control group (P<0.05);SHS group' HP masculine infection rate remarkably is higher than splenic asthenia group (PO.05).The inflammation degree and HP infection, COX-2 protein expression respectively appears positive correlation (PO.05).COX-2 protein expression and red body of the tongue, yellow greasy moss, tired hypodynamia, the few eats, anorexia have positive correlations relationship(P<0.05). COX-2 protein expression and gastric distention pains, turg, weary heavy limbs, loose stool obviously have no dependability (P>0.05). Apoptosis, Bcl-2 protein expression and tongue, pulse, symptom of patient obviously have no dependability (P>0.05).3. Animal ExperimentThe Influence of QingZhuoAnZhongDecotion on COX-2, Cell Apoptosis of Rat Model with SHS of Chronic GastritisObjects: 50 SD SPF rats were adopted. The number of female and male rats is equal. It was randomly divided into 5 groups. Such as normal control group, gastritis splenasthenic syndrome group, gastritis damp-heat syndrome group, dose-2 prevention and cure group, dose-1 prevention and cure group. The sample number of each group was 10.Method: For studying reproduction of animal model, we adopt the method of integrate of disease and syndrome. The gastritis splenasthenic syndrome model, gastritis damp-heat syndrome model, and gastritis damp-heat syndrome prevention and cure model were manufactured by mean of sodium salicylate, honey, fats, senna leaf, the artificial climate box and QingZhuoAnZhongDecotion.Results: The gastritis damp-heat syndrome rats model appeared the following symptom , for example anorexia, few drink, weight lose, hair loose rough, dysphoria,scrotum slack ptosis, not-forming stool or loose stool, yellow urine and little urine and so on.. QingZhuoAnZhongDecotion can improve above-mentioned symptoms after applying it.The rat gastritis splenic asthenia group and gastritis moist heat group are quite serious in gastric mucosa inflammation, comparing with the normal group, the difference is significant (P<0.05). The gastric mucosa inflammation integral obviously reduces after applying QingZhuoAnZhongDecotion. Comparing with gastritis moist heat group, inflammation integral remarkably improvedThe cell apoptosis index, Bcl-2, COX-2 protein expression : apoptosis cell in the epithelial cell, the wall cell, the main cell and Bcl-2 protein expression in the cytoblastema of wall cell, main cell and epithelial cell were observed under the microscope.COX-2 protein expression appeared in the cytoblastema.of surface epidermis, gland epidermis and inherent layer intersititial cells, but the masculine dyeing cell concentrates in the surface layer epidermis, the gastric pit gland cervical part and superficial layer inherent gland.The cell apoptosis index of splenogastric hygropyrexia syndrome group and splenasthenic syndrome group are larger than the normal group (P<0.05), but splenogastric hygropyrexia syndrome group is lower than spleen asthenic syndrome group (P<0.05), the prevention and cure group is smaller than splenogastric hygropyrexia syndrome group but larger than the normal group (P<0.05). Splenogastric hygropyrexia syndrome group and spleen asthenic syndrome group' Bcl-2 protein expression is stronger than the normal group (P<0.05), but the Splenogastric hygropyrexia syndrome group is lower than spleen asthenic syndrome group (PO.05). The prevention and cure group is smaller than splenogastric hygropyrexia syndrome group but larger than the normal group (P<0.05). The COX-2 protein expression in splenogastric hygropyrexia syndrome group and spleen asthenic syndrome group is stronger than in the normal group (PO.05), but the splenogastric hygropyrexia syndrome group is higher than spleen asthenic syndrome group (P<0.05). The prevention and cure group is smaller than splenogastric hygropyrexia syndrome group but larger than the normal group (P<0.05).4. Conclusions4.1 The research of splenogastric hygropyrexia syndrome was approximatelyexperienced about four stages that have laid a foundation, sprouting, formation and consummatement, and a set of more perfect theory system. The hierarchy of theories, therapies, prescriptions and herbs had been formed.4.2 Under gastroscope, gastric mucosa hyperemia dropsy of patient with chronic gastritis splenogastric hygropyrexia syndrome is obvious, accompanied by erosion or hemorrhagic spots and the more than normal, stomach mucus. The pathological inflammation in splenogastric hygropyrexia syndrome group is obviously stronger than the normal control group and spleen asthenic syndrome group, which can showed that the splenogastric hygropyrexia syndrome has the inflammation pathology foundation, is in the inflammation active stage, the early stage and the inflammation response is obvious.4.3 The HP infection is one of main disease causes of chronic gastritis splenogastric hygropyrexia syndrome. HP is similar to "moist heat" evil factor. This hints that "syndrome", "bacterium", "inflammation" exist certain correlation.4.4 The gastric mucosa inflammation degree and apoptosis accrescence in splenogastric hygropyrexia syndrome don't have the parallel relations. This hints that the cell apoptosis possibly is one of pathology physiology foundations of spleen gastric syndrome accompanied with asthenia and sthenia.4.5 The splenogastric hygropyrexi syndrome gastric mucosa Bcl-2 protein expression up-regulation is different from splenasthenic syndrome. This explains that gastric mucosal lesion process, Bcl-2 expression regulation is different. Bcl-2 possibly is one of molecule regulation genes forming comorbidity with different syndrome.4.6 The splenogastric hygropyrexi syndrome gastric mucosa COX-2 protein highly expression, also is obvious in the inflammation spot. This indicated that COX-2 forms one of inflammation pathology mechanisms of splenogastric hygropyrexia syndrome.4.7 The present symptoms of splenogastric hygropyrexia and COX-2 variance show positive correlation. This hints that the COX-2 variance may be one of the molecular biology foundation on traditional Chinese medicine spleen gastric syndrome accompanied with asthenia and sthenia symptom in which has provided the theory basis with comorbidity with different syndrome and different disease with same syndrome.4.8 Making rat model by the method of integrate of disease and syndrome appear near characteristic with the clinical splenogastric hygropyrexia syndrome. "Measuredsyndrome by medicine" May restore or improve many unusual experimental index. This confirmed that the duplicating splenogastric hygropyrexia rat model accorded with the traditional Chinese medicine splenogastric hygropyrexia syndrome attribute.4.9 The symptom, pathology, apoptosis, Bcl-2 and COX-2 change in the duplicated rat splenogastric hygropyrexia syndrome model are consistent with the clinical splenogastric hygropyrexia syndrome change. QingZhuoAnZhong Decotion may improve the above indexes, which indicated the curative effect of QingZhuoAnZhong -Decotion is exact.
