The expression of HLA and its relation to the biological behaviour of the laryngeal carcinoma were investigated, and the regulation of HLA expression in laryngeal carcinoma cell line (Hep-2) with IFN- γ was studied. The purpose of this study is to look for an effective biotherapeutic approach to treat the advanced laryngeal carcinoma. Surgery combined with immunotherapy and / or radiotherapy , chemotherapy may increase the survival rate of the laryngeal carcinoma patients and make the laryngeal carcinoma curable.The expression of HLA-DR in 95 specimens of squamous cell carcinoma of the larynx was studied using immunohistochemistry technique. All the patients were treated with surgery alone from 1982 to 1990 in Cancer Hospital, CAMS. There were 73 male and 22 female patients. Their ages ranged from 32 to 74 years ( mean 55.8 years ). There were 7 cases with stage I, 16 cases with stage Ⅱ, 31 cases with stage Ⅲ and 41 cases with stage Ⅳ disease. Ninety percent of the patients were followed up more than 3 years. There were 29(30. 5%) specimens showing a positive staining of HLA-DR and 66(69.5%) showing a negative staining of HLA-DR. Normal laryngeal mucosa and mucosa adjacent to the tumor did not express HLA-DR antigen. There were 24 well, 4 moderately, 1 poorly differentiated squamous cell carcinoma in the 29 HLA-DR positive specimens and 37 well, 28 moderately , 1 poorly differentiated squamous cell carcinoma in the 66 HLA-DR negative specimens.
