Mortality of severe acute pancreatitis is high , one of the reasons is that severe acute pancreatitis can cause multiple organ disfuntion syndrome in its very early stages, unfortunately, the mechanisms of multiple organ disfuntion syndrome is not clear. So its helpful to reduce the mortality of severe acute pancreatitis if we know the mechanisms of multiple organ disfuntion syndrome. Now the injury of local and systemic Multiple organ disfuntion syndrome in severe acute pancreatitis has not effective connection, none of splanchnic organ' inflammatory response like severe acute pancreatitis can cause systemic inflammatory response syndrome (SIRS) quikly and extensively. Recent research has demonstrated that the mechanisms of SIRS is the cytokines and inflammatory facters over-released.A central feature in the regulation of inflammatory gene product expression is the transcription factor nuclear factor [kappa]B (NF-[kappa]B). NF-[kappa]B is composed of a family of proteins that regulate the transcription of a variety of cytokine, chemokine, adhesion molecule, and enzyme genes involved in SIRS. Increased activation of NF-[kappa]B during severe acute pancreatitis has been associated with SIRS (poor outcome) in some studies.Despite our increased understanding of the role of inflammatory mediators in the pathogenesis of sepsis and SIRS, most antiinflammatory drug regimens have had little success in the treatment of SIRS .
