Background and ObjectivesPrimary hyperparathyroidism (PHPT) is characterized by inapproximately excessive secretion of parathyroid hormone (PTH) from one or more parathyroid glands. The actions of PTH to mobilize calcium from bone, excessively absorb calcium and excrete phosphorus in the kidneys, and promote production of 1,25(OH)2D3 lead to the clinical manifestations of hypercalcemia, hypophosphatemia, hypercalciuria and hyperphosphaturia. The pathologic changes of PHPT include adenoma, hyperplasia and carcinoma, among which adenoma is most common.PHPT is common in Europe and USA, which is third only to diabetes and hyperthyroidism in endocrinological diseases since 1970s. An epidemic survey conducted in USA during 1983 1992 showed the yearly prevalence of PHPT was 20.8 / 100,000 and there was 1 PHPT patient in every 1000 outpatients in North American. Adami et al. reported the prevalence of PHPT in Italian women aged 5575 and the whole population was 21 / 1000 and 3 / 1000, respectively PHPT is not so common in China as in North America and no data of incidence is available.With the introduction of the multichannel autoanalyzer, about 50% of adult patients were asymptomatic in western countries in the past 20 years.
