Initial Valuation Of Differentiation Mode Of Yang And Yin Syndrome Groups In The Acute Phase Of Stroke

Background: Stroke has one character of four aspects ascending and one aspect increasing: high attack rate, high death rate, high disabling rate, high recurrence rate and increasing complication. How to overcome are the focus, emphasis and difficult matter in medicine. The hemorrhage stroke of modern traditional medicine of Chinese (TCM) corresponds with intracerebral hemorrhage of modern western medicine, and the isehemic stroke of modern TCM with cerebral infarction of modern western medicine. However, the modern medicine considers that the versatile therapy early in the acute phase is most effective to treat this disease. The TCM has a thousands history in treatment disease of zhongfeng based on differentiation. Then the research on stoke of TCM is widening and broadening.Objective: To summary the academic though by my tutor, Professor Liu Maocai, on treatment stroke in the acute phase based on the differentiation of Yin and Yang syndrome groups. Then to modify, valuate and build the mode and diagnostic procedure in the acute phase of stroke based on the differentiation mode of Yin and Yang syndrome groups, which hope to bring the most curative effect of TCM on treating stroke as one entity of disease.Methods: 1) Based on studying the ancient and present literatures, the development of both TCM differentiation and differentiation in zhongfeng of TCM was summarized. The characters of syndromes of stroke of TCM were analyzed. And the clinical application of the differentiation mode and the research outcome of syndromes were reviewed. Then the theory frame of differentiation of Yin and Yang syndrome groups was explained. 2) By the prospective cross-sectional observational study, the TCM signs and symptoms of 1150 ischemic stroke patients were collected by the TCM stroke scale which was divided into the two parts of disease and syndromes, and to establish thedatabase of syndromes of the acute phase of ischemic stroke, Based on medical thought, through the data analysis software of SPSS13.0 for window, the key syndrome factors of differentiation of Yin and Yang syndrome groups were assessed for ischemic stroke in the acute phase. Whether the mode of differentiation of Yin and Yang syndrome groups was applied well in the acute phase of stroke was to be checked-up. And the items of the differentiation were valuated to the acute of ischemic stroke. 3) To building database of the base syndrome of the acute phase of stroke, the diagnoses decision tree of differentiation of Yin and Yang syndrome groups in the acute phase of stroke was built based on the method of data digging by the SSPS13.0 and Microsoft excel data package. And the diagnoses routine graph was formulated based on the traditional medical appraisal.Result: 1) The review of ancient and present literature shown that treatment on TCM stroke based on differentiation should be early, fast and versatile to apply in the acute phase of stroke. Traditional Chinese medicine differentiation mode has abundance contents. The differentiation of Yin and Yang syndrome is most essential and complicated. And there was a long history of TCM on treating zhongfeng by differentiation. Of the many differentiation modes, the differentiation mode of Yin and Yang syndrome of TCM zhongfeng had become more systematic. The stroke of TCM had both common and individual characters. The syndrome of phlegm and stagnant blood was binding each other and the excess syndrome of Fu-organs was frequent, which were the common character. The similar change or following change of heat and chill, or yin and yang, according to the different body constitution, which was individual character. So the construction of differentiation mode of Yin and Yang syndrome groups had better rationale. And the mode was applied well in clinical trails.2) The syndromes research of ischemic stroke in acute phase:â‘ It is shown that the signs and symptoms of red tongue, dry and bitter oral pharynx, greasy coat of tongue, red face and heat body, and yellow coat of tongue were the key-points to clinical differentiation based on 117 syndrome items analyses, which, except greasy coat of tongue, shown the heat aspects. Above four items were the most part of ones of Yang syndrome group. The symptom of dry and bitter oral pharynx was the key-point in the 12-item differentiation mode in cluster analysis, which was accordance to the tree analysis outcome in the data of five hospitals in Linnan zone.â‘¡In the analysis on key syndrome factors in initial database, the items of differentiation of Yin and Yang syndrome groups were more frequent. The factor analysis shown that the 12-items would be divided into four facets, which effected the most contents of TCM. But the valuation of the differentiation of the pulse was descending.â‘¢The analysis outcome of diagnosis zone, the discrimination function and regression equation based these 12-item of differentiation of Yin and Yang syndrome groups could build the statistic quantification for clinic.3) The syndrome exploring:â‘ The syndrome of hot and heat was the key one to discriminate the yin syndrome group and yang syndrome group in the stroke. However, the syndrome of yang hyperactivity caused by yin deficiency was key to discriminate in the group without syndrome of heat and hot. The syndrome of Qi deficiency as a tree node appeared earlier in group without syndrome of heat and hot than in group with it.â‘¡At last the diagnoses routine graph of differentiation of Yin and Yang syndrome groups was constructed through the medical evaluation based on outcome of the literature and syndrome research.Conclusion: The statistics conclusion of ischemic stroke can refer to the clinic diagnosis; meanwhile the conclusion supported well the clinical differentiation. The differentiation of Yin and Yang syndrome groups had the profound rationale, the evidence of syndromes research and clinic applying ability. The mode also needed to research more.
