ObjectiveTo investigate the role of serum prostate specific antigen(PSA) inprostate cancer screening among aged men.MethodsBetween July 2004 and December 2005, The level of serum PSA of8562 males over 50-years old for health examination was evaluated,respectively. Out of them 58 cases diagnosed as prostate cancer(screening group) were compared in terms of age distribution, level ofPSA, prostate volume, Gleason score and clinical stages with 82 patientswith prostate cancer in our hospital (control group).ResultsIn the screening group, the positive percentage accounts for 8.4%(PSA≥4 ng/ml) and the PCa positive rate was 6.8%0 (58/8562). Theaverage age and age distribution of the patients was insignificant different between these two groups. However, we found that the average prostatevolume of the clincal group(45.6±11.0)ml was significantly more thanscreening group (30.6±11.2) ml. The percentage of the cases which PSAvalue beyond 20ng/ml in the screening group was significantly lower thanthat in control group (43.1% and 75.6%, respectively), and the percentageof prostate cancer in the screening group related with the PSA value (P<0.01). The percentage of poorly differentiated prostate cancer in thescreening group was also significantly lower than that in controlgroup(39.7% and 65.9%, respectively). Furthermore, the cases withclinical stage T1+T2 accounted for 87.9% in the screening group, while itwas 25.6% in the control group (P<0.01). Among all cases, the serumPSA value in poorly differentiated prostate cancer was significantlyhigher than that in well-differentiated prostate cancer or moderatelydifferentiated prostate cancer, and the serum PSA value had asignificantly difference between early stage prostate cancer and advancedstage prostate cancer.ConclusionsSerum PSA is valuable to screening for prostate cancer amongnonsymptomatic aged people, and part of cases were diagnosed withpoorly differentiated prostate cancer, hence gives us an opportunity forthe early treatment.
