| Apoplexy is a common clinical disease with main symptoms including suddenly loss of consciousness, facial hemiparalysis, hemiplegia or dysphasia, which is seriously threatening humans' health for its high morbidity and disability rate while low curative rate. The earliest records of apoplexy could be found in Huangdi's Canon of Medicine dating back to over two thousand years ago. Generations of doctors have developed precious theories, accumulated diagnosis and treatment experience and left us abundant literatures about apoplexy through their long-term clinical practice. The recognition of apoplexy by Traditional Chinese Medicine (TCM) is a process of gradual development and perfection. Therefore, the study of these traditional Chinese medicinal literatures, dig of the ideological essence and summarizing of the prevention and treatment ideas are significant references for the research on clinical prevention and treatment of apoplexy.This thesis classifies analyses and concludes the traditional Chinese medicinal literatures from pre-Qin period to modern times via traditional research method. This thesis is divided into nine parts:From the first to the seventh part, it classifies and analyses the name, etiology and pathogenesis, diagnosis and treatment, location, prevention, recognition and development of contraindication of apoplexy. First, it basically makes clear the naming of apoplexy developing from symptom-based naming to independent disease-based naming. The earliest records of apoplexy found in Huangdi's Canon of Medicine named sudden syncope (bojue), yin-deficiency syncope (jianjue), coma (dajue), hemiplegia (pianku), hemicrania (pianfeng), hemiplegia after apoplexy (fengfei), apoplexia (fengyi), apoplectic stroke (feifeng), endogenous wind (neifeng), encephalemia (nanchongxue), anencephalohemia (naopinxue) and so on. Now the names are unified to be "apoplexy", also "stroke". Secondly, the etiology and pathology develops from external wind before Jin Yuan Dynasties to internal causes after that. Until modern time that the etiology and pathology of apoplexy is finally established. It is gradually realized that pathogenic factors such as wind, fire, phlegm, qi, deficiency and stasis lead to disease, among which deficiency of liver and kidney is fundamental leading cause. As to location of disease, it is considered that the location of hemiplegia and hemicranias is in "tendon", while the location of sudden syncope and yin-deficiency syncope is in the head or brain. The therapeutic rules of apoplexy develops from treating mainly on dispersing wind before Jin Yuan Dynasties to mainly on the methods of transforming phlegm, clearing heat, invigorating blood, supplementing deficiency and extinguishing wind. It tends to adopt the method of promoting blood circulation and removing blood stasis in recent years, Neijing has the earliest records of the prodrome, which pioneers the prevention of aura. In modern times, an improved theory of aura prevention has been formed.The 8th part is the exploration for prevention and treatment of apoplexy. On basis of documents analysis, it summarizes that the key principal for apoplexy treatment, shortly "liver and kidney nourishment as fundamental, and qi tonifying and blood promotion as key". In prevention of apoplexy, it also points out that it is suitable to take measures according to normal physical and stroke susceptible population respectively in view of constitution to prevent apoplexy.The 9th part is the discussion on rclative questions such as thinking about the meaning of "wind", reflecting upon treatment of apoplexy with dispersing wind, as well as exploring on "poison damage brain collaterals" and method of neutralizing poison to unblock collaterals to propose own opinion. |
