The researches from the end of 1980s' to present validated the resistance of insulin is an important factor of diabetes. In another words, quite a number of diabetes patients (70%-80%) exist insulin resistance. The research of traditional Chinese Medicine on the insulin resistance has got prevalent attention in recent years. Although people have widely deeply developed lots of clinic and basic researches and got great achievement, they have not gained distinct progress on the establishment of animal model and basic appliance, particularly on methods of estimating insulin resistance. Now in the field of traditional Chinese medicine, the acknowledgement of reasons and pathogenesis has not been unified.Cooperating with Experimental Animal Off ice of China Academy of Medical Sciences, we succeeded in developing the Fructose-Fed Rats (FFR) model in China. Besides, we applied the euglycemic hyper insulinemic glucose clamp (GO technique to evaluate the model and got the similar data as the overseas' .Based on many years clinic experience of professor Jiang,Liangduo and according to recent research and explore, we tried to bring forward the pathogenesis of insulin resistance which may be deficiency of Qi and stagnation of blood, accumulation of damp-heat. Using the GC technique, we primarily validated pathogenesis of insulin resistance of traditional Chinese medicine.Our research first used international accepted method of estimating insulin resistance and FFR model to probe ameliorative effect of compound herbs Tang Shen capsule to insulin resistance and some action mechanism. It methodology received approval of experts from all over the world.The experimental research includes five parts as following:1 Insulin Resistance and Hypertension Rat Model Induced by Fructose-rich and Its FeaturesObjective: The aim of the study was to create a rat model (Fructose-Fed Rats, FFR) of insulin resistance (IR) & hypertension , and to Investigate its features. Methods: Six-week-old male Sprague-Dawley rats were fed with either normal rat chow group (60% vegetable strach, 5% fat and 24% protein, n=35) or fructose-rich diet group (60% fructose, 5% fat and 20% protein, n=35) .All rats was performed the euglycemic hyper insulinemic glucose c 1 amp (GC) technique to estimate insulin sensitivity, Blood pressure was elevated with tail-cuff method. Results: It was showed blood pressure of the FFR groups was significantly increased comparing to control groups, The average rate of glucose clamp, as a measure of insulin sensitivity (M value) and weight was significantly lower in FFR compared to control (P=0. 012-0. 002) after 4 weeks or 8 weeks. TG was significantly decreased in the control group comparing to FFR group (P=0.036). However, FFA&FINS was the same as TG. But no significantly diffrence(P=0. 066, 0. 074). Conclusion: This rats model is characterized by typicalinsulin resistance & hypertension, It may provide a advantageous tool for investigations of insulin resistance and hypertension.2, Research Report of the Establishment about Euglycemic Hyper insulinemic Glucose Clamp Technique Under Conscious StatusAt the end of the treatment period, rats were anesthetized with sodium pentobarbital (50mg/kg, intraperitoneal) .The left common carotid artery and the left jugular vein were exposed and then cannulated with a polyethylene tube (PE50, Becton Dickinson and Co., Sparks, MD)for collection of blood samples and administration of insulin and glucose. The technique for glucose clamping was as described previously. Briefly, on the day following an overnight fast (approximately 12h), each conscious rat was placed in a foam plastic jacket, which allowed movement of all four limbs and forward vision. At the start of the glucose clamp, fasting blood glucose measurements were obtained and the initial load of insul in (25mU/kg of humalinR, U-40, Shionogi Pharmaceutical Co., Osaka, Japan)was infused in a bolus, followed by an infusion of insulin at a rate of 4mU/kg/min for 150 min.
