Part One The Criteria and Clinical Utility of Motor Nerve Conduction BlockBackground: Motor nerve conduction block is of crucial importance in differentiating ALS from MMN and other demyelinating motor neuropathy. The criteria of CB remain controversial. More strict criteria may lead to under-diagnosis of potentially treatable neuropathy, on the other hand, loose criteria may result in misdiagnosis of ALS. The degree of CMAP amplitude and area reduction across a standard segment in a large sample of ALS has not been reported, although it is important in the establishment of CB. When the reduction of amplitude and area across a standard segment exceed the limits of normal but not reach the criteria of CB, it may potentially have clinical significance except for phase cancellation and need further study. AIDP is an acute demyelinating neuropathy while CIDP and MMN are chronic demyelinating neuropathy, further study of the characteristics of motor nerve conduction may help to understand the mechanisms and nature of CB involved in these disease and facilitate the diagnosis of these diseases.
