.1 Gonadotropin-releasing Hormone Stimulation Test In Early Differential Diagnosis Of Young People's Physical Puberty Delay And Reduction Of Men With Low Gonadotropin Function Value Of Some Neuropeptides And Neurotransmitters In Mice Gt <sub> 1

Object:There is controversy of clinical significance of LHRH exciting test in differential diagnosis(DD) of constitutional delayed puberty (CDP) and hypogonadotropic hypogonadism (HH).We want to investigate the clinical significance of GnRH exciting test in DD of CDP and HH. Method:We prospectively investigated and followed up 123 cases from 1982 to 2002. This article analysis the result of their GnRH exciting test. According to their puberty development status, these cases are divided into 3 groups, normal group (n=34), CDP group (n=41) and HH group (n=48). After they received GnRH-exciting test, they are followed up until they are older than 18 years. They are all at genital stage I. After injected by 100ug GnRH, the blood samples (3 ml) are taken at 0,15,30,45,60,90 and 120 minutes. The serum LH and FSH level are determined by radioimmunoassay. Then they are followed up every 6-24 months. Result:1. According to the serum LH peak level and LH increment' s receiver operating characteristic curve(ROC), we can find out the cut-off point of LH peak level and LH increment are 8IU/L and 5IU/L respectively, combined with these two indices, the sensitivity and specificity of this diagnosis criteria is 92. 7% and 91. 7%. In HH, CDP and normal group. According to the peak LH/FSH ratio ROC , the sensitivity and specificity is 87. 5%, and 52. 4% respectively, when the cut-off point of peak LH/FSH ratio is 1.0.2.After simplified the GnRH Exciting test(only the LH level of 0,60 and 120 minutes to be analysed) , we can find out that the final diagnosis of HH and normal group are completely same as those of the cases before it was simplified, but there are 2 cases of false negative in CDP group, the diagnosis of CDP can be made after followed up or take GnRH Exciting test one year later.
