| Diabetes mellitus is a kind of metabolic disease with endocrine secretion. It is concerned not only with general and chronicity, but also with special enxironmental factors. It is commonly to heighten plasma glucose. The essential patho-physiological funtions is metabolic disorder with hypoinsulinism absolutely or relatively. The metabolic disorder include blood glucosed, protein, fat, aquifer, electrolytes and so on.It sometimes induce the disorder of acid-base balabce. The distinction is hyperglycemia, glycosuria, impaired glucose-tolerance as well as the anormal experimental result of insulin releasing. Traditional Chinese medicine deem that the Diabetes mellitus is pertain the category of XIAOKEBING. When his clinical feature was the hydrodipsia and polydipsia, the polyphagia and hunger, the increasing of urinary production, the tabefaction of physique and body, or the sweet taste of urine, he should be diagnosed XIAOKEBING. This research is to discuss the point of patho -geny, pathogenesis, differentiation and prescriptions of herbs according to data analysis of outpatients with diabetes mellitus from professor Zhou Zhongying.33 patients with diabetes mellitus were treated with differentiat -ion. Better curative effects could be seen, that meant most patients' main symptoms were significantly relieved, especially hypodynamia, poly -dipsia, polyphagia and hyperdiuresis. But the disfunction of tabification was improved relatively slowly. General living quality was improved and ccores increased 13.8 on average. The effective rate was 90.90%,and clinical relieving rate reached to 87.88%.The location of diabetes mellitus is the lung, the stomachus and the kidney. Deficiency, dryness-heat, humid-heat and stagnated-heat are the main pathological factors. Extra pathogenies is intemperance of taking food. Innate deficiency, the debilitation five solid organs, emotional disfunction and the excessus erotism are inner pathogenies. Deficiency of health energy and yin is the principle, and the combination of dryness-heat, humid-heat and stagnated-heat is the subordination. Excess and deficiency complicate and come into being the agglutinative and hardly curative sufferings. The pathological factors are farther concluded. That means deficiency(both vital energy and yin), dryness-heat, humid-heat and stagnated-heat and the occurrence rates are 84.85%,27.27%,48.48% and 63.63%.The common fact is that several pathological factors occur at the same times, and 3 or 2 pathological factors are more ordinary. For example, These factors that deficiency and stagnated-heat or deficiency, humid -heat and stagnated-heat are easier to be seen. The effective therapeutic method is large prescription with compound principles according to multiple pathogenies, differentiations and symptoms. That means using multiple prescriptions and herbs to reinforce deficiency and reduce excess ,balance yin and yang, regulate qi and blood. In a word, a doctor should always draw a clear distinction between, dryness-heat, humid-heat and stagnated -heat, chronic and emergent situations, reinforce deficiency and reduce excess.Several typical cases mentioned in the research come from professor Zhou Zhongying' s clinic recently. From these cases ,the thought of dia - gnosis and treatment, the characters of differentiations can be seen.Professor Zhou Zhongying emphasize constantly that one principle should be to solve different in the treatment, and that means with the analysis of different symptoms, to discover pathogenies from differentiations, and then to make prescriptions based on differentiations. Individual treat-ment, combination of disease and differentiations that leading to select specific herbs to improve curative effects are pointed. The dialectica and concentrating should be based on the theory that dryness-heat, humid-heat and stagnated-heat are the pathogenies of diabetes mellitus. The herbs are explained in the article who are to cool the blood and to subdue the blood glucose when used in th diabetes mellitus treatment, and they should be used with the theory of triple-heat. |
