Objectives1. Investigating on the profile of sex hormone level ,the relationship between the sex hormone and every component of metabolic syndrome (MS) in postmenopausal women with MS;2. Assessment of the level of androgen receptor(AR)and estrogen receptor(ER), the relationship between AR,ER and each component of MS in postmenopausal. women;3. Evaluation the expression of (CAG)n polymorphism of AR gene,and the relationship between the (CAG)n polymorphism of AR gene and every component of MS in postmenopausal women with MS;ObjectsIn total, there were 168 postmenopausal women who were older than 55 enrolled into the study. Among these postmenopausal women, 78 (age 55-81, 64. 48±8. 22)were healthy; 90 (age 55-79, 66. 41±7. 41) suffered from MS. The standard of healthy old women was established by presbyatrics branch of CMS in 1995; the standard of MS was supposed by diabetes branch of CMS in 2004.Methods1.Measurement of fasting blood glucose (FBG) and lipid by chromatometry (Automatic Analyzer, Hitachi 7600).2.Measurement of plasma sex hormone by the enzyme-linked immunosorbent assay technique. Measurement of fasting serum insulin(FINS) by Solid-phase, two-site chemiluminescent immunometric assay.3 .Measurement of AR and ER:AR on the peripheral leukocyte was measured by indirect immunofluorescent labeling with flow cytometry (expressed by mean fluorescence intensity and positive rate of androgen receptor). ER was measured by direct immunofluorescent labeling with flow cytometry.4.Measurement of (CAG)n polymorphism of AR by PCR ,then sequencing the DNA.Results1. The profile of sex hormone level ,the relationship between the sex hormone and every component of MS in postmenopausal women with MS;i. The E2,TT,E2/TT,SHBG,LH and FSH were significantly difference between in the MS group and in healthy group.ii. The menopause time, WHR, blood pressure,HDL-C,FBG, FINS, HOMA-IR,E2/TT,SHBG, FSH were significantly difference in different waist circumference groups. Sowere that in different BMI groups.iii. The level of E2 was negatively correlated with age, menopause time, WHR,SBP, FSH ,LH and positively correlated with SHBG.iv. SHBG was negatively correlated with BMI, waist circumference ,WHR,SBP,DBP,TG,FBG,FINS and HOMA-IR; positively correlated with HDL-C.v. TT was positively correlated with menopause time, waist circumference , SBP,TG,FINS,DHEA and HOMA-IR;. negatively correlated with E2/TT and FSH.2. The level of AR and ER, the relationship between AR,ER and each component of MS in postmenopausal women:i. The fluorescence intensity and positive rate of ER were significantly higher inMS group than in healthy group.There was a tendency of fluorescence intensityand positive rate of AR in MS group ,but there was not significant.ii. The positive rate of AR was negatively correlated with SHBG.iii. ER was positively correlated with age,BMI, waist circumference, WHR, TG, FBG, FINS and HOMA-IR; The fluorescence intensity of AR was negatively correlated with TC and LDL-C.3. The expression of (CAG)n polymorphism of AR gene,and the relationship between the (CAG)n polymorphism and every component of MS in postmenopausal women with MS:i. The length of (CAG)n of AR gene was from 18 to 34,mean 23.34±2.44 inhealthy postmenopausal women.It was from 8 to 29,mean 22.29±2.82 inpostmenopausal women with MS.ii. There was a significant difference in (CAG)n of AR between healthy postmenopausalwomen and those with MS.iii. There was not relationship between (CAG)n polymorphism and the level ofAR in postmenopausal women.iv. The (CAG)n polymorphism was negatively correlated with the level of serumTT.v. The (CAG)n polymorphism of AR was not correlated with components of MSin postmenopausal women.Conclusions1. The E2,E2/TT and SHBG decrease, TT increases in postmenopausal women with MS.The diversities of E2,SHBG and TT are concerned with components of MS in postmenopausal women.2. The decrease of E2 is significant within 10 years after menopause. It is presumption that the time of supplemental estrogen should be within 10 years after menopause.3.The expression of ER potentizes with the decrease of E2 ,and there is a potentiation tendency in expression of AR with the increase of TT. Thus we suppose that is a reflex accommodation of human to sex hormone.4. The expression of ER is correlated with components of MS in postmenopausal women with MS.5. The (CAG)n polymorphism of AR is present in postmenopausal women. (CAG)n is shorter in women with MS than that in healthy women.6. The (CAG)n polymorphism of AR is negatively correlated with the level ofserum TT.
