| Objective:To investigate the therapeutic effect of Xianyu decoction combined with chemotherapy and approach its' conceivable mechanism of action from the direction of vascularization in treatingⅢb andⅣstage None-small Cell Lung Cancer(NSCLC),which will provide the experimental foundation in further study and exploitation of Xianyu decoction.Methods:1.Clinical research:60 patients with stageⅢb andⅣNSCLC were randomly divided into Xianyu decoction group,chemotherapy group and combined group,all patients in each group were treated for two course(6w).The clinical symptom,the size of tumor,body weight,the toxicity of hematology,the function of liver and kidney,the Karnofsky scores and et al were detected and compaired between each group.2.Experimental research:50 C57BL/6 mice planted by Lewis lung cancer cells 24h later were randomly divided into 5 groups after weighing and numbering,each group had 10 mice.The groups were Xianyu decoction group, combined group(treated with cisplatin and Xianyu decoction),chemotherapy group(treated with cisplatin),model group of Lewis lung cancer and normal control group.High dosage of Xianyu decoction were used in Xianyu decoction group and combined group,in which the dosage of intragastric infusion was 2.28 Xianyu decoction per 20g weight of mouse.Lose dose of cisplatin of 0.02mg/20g/d was used in combined group and chemotherapy group.0.2ml Sodium Chloride was lavaged in model group of Lewis lung cancer and normal control group.The lavaged dose in each group was 0.2ml.21d after intragastric administration,immunohistochemistry was used to detect the expression of VEGF and the formation of MVD of lung cancer cells,FQ-PCR was used to detect the expression of KDR mRNA and NF-κB mRNA in tumor-bearing mice with Lewis lung cance.Results:Clinical research:1.Although the total effective rate of combined group in the change of tumor size was higher than that of Xianyu decoction group and chemotherapy group,the difference between them was without significant(P>0.05).2.The effect of relieving the clinical symptoms such as cough,debility, anorexia and et al in Xianyu decoction group and combined group were better than that in chemotherapy group(P<0.05 or P<0.01),proving that Xianyu decoction could relieve the clinical symptoms of the patients in stageⅢb andⅣNSCLC,elevate the therapeutic efficacy of chemotherapy.3.The survival quality in Xianyu decoction group and combined group was much better than that in Chemotherapy group(P<0.05 or P<0.01).4.The incidence rate of adverse effect in hematological system in chemotherapy group was lower than that in chemotherapy group(P<0.05 or P<0.01).5.The incidence rate of adverse effect in liver function in chemotherapy group and vomit,the degree of diarrhea was lower than that in chemotherapy group,the difference between them was statistical significant.(P<0.05 or P<0.01).Experimental research:1.The inhibitory rate on mouse tumor of combined group was much higher than that of Xianyu decoction group,chemotherapy group and model group(P<0.01).2.The expression of VEGF and the formation of MVD of lung cancer cells in combined group was much lower than that in Xianyu decoction group,chemotherapy group and model group(P<0.01).3.The expression of NF-κB mRNA in tumor-bearing mice with Lewis lung cance in combined group was much lower than that in Xianyu decoction group,chemotherapy group and model group(P<0.01).Conclusions:Xianyu decoction combined chemotherapy can elevate the therapeutic effect,relieve the poison and side effect of chemotherapy, improve symptoms,raise the Karnofsky scores,stave the depravation of illness, help to accomplish chemotherapy period.Xianyu decoction matching low dose chemotherapy can inhibit Lewis lung cancer the expression of VEGF,KDR and NF-κB,depress the vasoformation in the tissue of lung cancer,which will be the conceivable mechanism of Xianyu decoction in treatingⅢb andⅣstage NSCLC. |
