[Objective] To clarify the hand function of moderate and severe carpal tunnel syndrome after surgery and relative factors of hand strength recovery.[Materials and Methods] 44 patients who had clinically and electro-physiologically been confirmed carpal tunnel syndrome and had the carpal tunnel release in 2013 to 2014 in huashan hospital are involved in the research. Then the data of two-point discrimination of the index finger, grip and lateral pinch strength in patients suffering from carpal tunnel syndrome before operation, one month after operation and six months after operation were recorded, so were the DML from the wrist to the abductor pollicis brevis of the median nerve(preoperative, intraoperative).[Result] The two-point discrimination showed no difference one month after operation compared with preoperation, but the grip and lateral pinch strength had significant decrease. The two-point discrimination showed no difference six months after operation compared with preoperation, but the grip and lateral pinch strength had significant increase. The moderate CTS patients showed correlation between the grip, lateral pinch strength recovery and preoperative DML and difference between pre and intraoperative DML. The severe CTS patients showed no correlation with any of the relative factors.[Conclusions] 1.most of the CTS patients had an excellent outcome of the hand functions;2. The grip and lateral pinch strength had significant decrease one month after operation, but had significant increase six months after operation;3. The moderate CTS patients showed correlation between the grip, lateral pinch strength recovery and preoperative DML and difference between preoperative and intraoperative DML.
