Studies On Clinical Effect And The Experimental Of Expelling Stagnation And Invigorating The Spleen Method In Treating Peptic Ulcer

Objective:This study was aimed at further confirming the therapeutic effect of expelling stagnation and invigorating the spleen method on peptic ulcer and at exploring its mechanisms to accelerate ulcer healing.Method:Clinical part:Sixty patients with confirmed peptic ulcer under gastroscope and differentiated as Gan-stagnat Pi-deicient syndrome were randomly assigned to two groups, the treated group(30 cases)treated with kaiyu jianpi formula and the control group(30 cases)treated with ranitidine,4 weeks as one therapeutic course. Observe the changes before and after treatment in Symptom integral, ulcer-healing rate, clinical effective rate. Experiment part:Model of experimental gastric ulcer was prepared with Okeba’s method with some modification. The animals were randomly divided into 6 groups:normal group, model group, ranitidine group, kaiyu jianpi formula low dosage group, kaiyu jianpi formula middle dosage group, kaiyu jianpi formula high dosage group. Fifteen days later, ulcer index, nitride oxide(NO),prostaglandinE2(PGE2),epidermal growth factor receptor(EGFR) and basic fibroblast growth factor(bFGF) were determined respectively.Results:Clinical part:After medical treatment for 4 weeks, clinical research has confirmed that, in the treated group, there are 12 cures,10 prominences,5 effections, the total effectiveness is 90.0%. But, in the control group, there are 6 cures,8 prominences and 12 effections, the total effectiveness is 86.7%. By reexamining the gastroscope we can find that, in the treated group there are 6 cures,14 prominences and 8 effections. The total effectiveness is 93.3%. In the control group, there are 2 cures,11 prominences,13 effections. The total effectiveness is 86.7%. There are remarkable differences between them(P<0.05). In the other hand, by comparing the symptom improvement, we can find remarkable differences in many clinic symptoms sucn an dul stomachache, abdomen puffiness and anorexia so on. Experiment part:Kaiyu jianpi formula can alleviate the lesion induced by acetic acid; can reduce ulcer index of rats; can increase the content of serum NO and the level of plasma PGE2. Besides, kaiyu jianpi formula can increase(regulate) the protein expression of EGFR and bFGF in the mucosal epithelial cells of the ulcer margin.Conclusion:Kaiyu jianpi formula could effectively accelerate the ulcer healing. The relative mechanisms may be as follows:improve the gastric mucosal defensive function by increasing the synthesis, release or secretion of NO、PGE2; promote regeneration of epithelial structures and re-estabishment of the underlying connective tissue by growth factors(EGFR、bFGF).
