The Clinical Research Of Song Mingsuo's Academic Thoughts And Clinical Experience And The Regulation Of Spleen Chengqi Decoction On The Treatment Of Pediatric Acute Moths

Objective:To explore the academic thoughts and clinical experience of Song Mingsuo,advisor of the national inheritance work of academic experience from aged experts in traditional Chinese medicine(TCM),famous TCM expert of Shanxi Province,and chief physician of Traditional Chinese Medicine of Shanxi Province.This paper studies the academic development of Teacher Wang to summarize his academic thoughts,clinical experience and scholarly research methods,so as to deepen the academic understanding,improve clinical skill as well as to collect more clinical research material to inherit Teacher Song’s academic experience.Children areeasily attacked by pathogenic factors because of their immature constitution and deficient lung and spleen,especially the delicate lung whose defenseQi can’t secure the exterior.Pediatric acute tonsillitis develops quickly,often with high fever and severe condition.Frequent relapses and lingering would lead to respiratory tract infection,chronic tonsillitis and so on,greatly influencing the growth and development of children.This paper summarizes Teacher Song’s clinical experience about this disease,further explores the regularity in his drug usage,and evaluates the clinical curative effect of Chinese medicineclysis treatment in treating tonsillitis of lung-stomach heat pattern.The research improves the external treatment method of Chinese herbal treatment for pediatric acute tonsillitis,which is convenient,effective and easy to practice;and the design idea and operation process can also provide clinical evidence for the traditional theory of "lung and large intestine are related".Method:1.Summarize the academic thoughts and experience of Teacher Song.Many methods are employed,such as literature review,in-depth interview,lecture learning,clinical observation,record explanation,case discussion and clinical research.Teacher Wang’s academic thoughts,clinical experience are explored,and the practice thoughts and innovation ideas are discussed mainly from such aspects as disease and mechanism diagnosis,syndrome difference,treatment and medicine,prescription modification as well as case discussion.2.Study the clysis method for acute tonsillitis and evaluate the curative effect.Based on the review of Teacher Song’s medicine regularity in treatment,perspective randomized controlled research method is applied to study the clinical curative effect of Chinese medicine clysis treatment in treating pediatric tonsillitis of lung-stomach heat pattern with SPSS statistical analysis,thus to provide objective evidence for clinical clysis treatment.Results:The characters of Teacher Song’s academic thoughts include three aspects as follows.①Internal medicine and then pediatrics study provide a solid base for clinic practice;②besides pediatrics,he also studies many other diseases in different branches to achieve a more comprehensive mastery of TCM;③ persistent clinical practice promotes the academic development.1.Summary of Song Mingsuo’s academic thoughts:①Correlation of four examinations,and especially inspection of tongue and vitality.Teacher Song tends to judge the disease condition and prognosis by inspection of vitality,and identifies the constitution and disease nature by tongue inspection.Shangen(the bridge of nose)and Qichi(area under the pupil)are emphasized in facial inspection.② Qi aspect is emphasized in fever treatment,while spleen and stomach also need to be protected.Treatment of this stage has a crucial influence on transformation and prognosis of disease.Thus more attention should be paid on protecting spleen and stomach,which can be reflected in aspects like releasing of the exterior and interior,mastery of purgation,controlled procedure in treatment as well as regulation of recovery.③Miscellaneous treatment focuses on spleen and stomach.Regulation of spleen and stomach can not only strengthen health qi,but also can help to treat many other miscellaneous diseases.And Tiaopi Ba Fang(Eight formulas regulating spleen)show the crucial function of spleen and stomach.④ Song tends to use prescriptions and methods following criterion on the base of precise syndrome differentiation when treating chronic diseases。⑤various administration methods including external method are employed.Based on patient’s wellbeing and syndromedifferentiation,Song advocates various administration methods and performs external treatment actively to relieve children’s pain and improve clinical curative effect.2.Summary of Song Mingsuo’s clinical experience.①ZhiziWendandecotion can clear heat and resolve phlegm,regulate qi and downbear turbidity.Teacher Song uses it to treat diseases caused by phlegm-heat.② Chinese medicine clysis method is employed by Teacher.Song to treat pediatricexcess syndrome or heat syndrome.③ TiaopiChengqi decoction is modified by Teacher Song based on Xiehuang powder,Da Chengqi decoction,QingreXiepi powder,which can clear spleen,purge heat,unblock bowels and eliminate accumulation.It can be used for diseases caused by accumulated heat of spleen and stomach.④ Teacher Song’s clinical experience of treating pediaticexternmal fever contraction of wind-cold,wind-heat,summerheat-dampness,dampness-heat,spring warmth,and dampness-warm.⑤ clinical experience of treating pediatric acute or chronic tonsillitis are summarized.⑥experience summary of gypsum,rhubarb and antelope’s horn:Gypsum can clear high fever of qi aspect;Rhubarb can purge the bowel excess of Yang brightness,and conduct pathogen qi downward to relieve the depressed heat through Yang brightness’s way.Antelope’s horn can clear heat and tranquilize mind.3.The clinical research of pediatric acute tonsillitis.①The properties that teacher’s drug treatment of pediatric acute tonsillitis are cold and cool,with sheen,bitter,sweet;belong to the Chinese medicine,give priority to with lung and stomach,as well as heart,liver,spleen.There are stronger correlation between gypsim,rhubarb,cortex moutan,periostracum cicadae and stiff silkworm.② Curative effect evaluation of Chinese medicine clysis in treating pediatric acute tonsillitis:Both groups can abate fever effectively,and it takes average 20 hours(20.07±12.85)to reach the normal temperature in experimental group,while 36 hours(36.27± 19.09)in control group.There is statistically significantdifference(P<0.01)in fever abatement time and degree,in which treatment group has better effect than control group.There is statistically significantdifference(P<0.05)in curative effect on single symptoms like pharyngalgia,odynophagia,antiadoncus,while no statistically significantdifference(P>0.05)on Pharyngeal mucosa congestion,tonsil congestion.After 3 days’ administration:①Curative effect evaluation on TCM symptoms:theeffect rate of the control group is 57.14%with the total effect rate of 97.40%;while the effect rate of the experiment group is 81.58%with the total effect rate of 98.68%.There is statistically significantdifference(P<0.05)in curative effect on TCM symptoms,in which treatment group has better curative effect than control group.②Curative effect evaluation on disease:theeffect rate of the control group is 50.65%with the total effect rate of 97.40%;while the effect rate of the experiment group is 77.63%with the total effect rate of 98.68%.There is statistically significantdifference(P<0.05)in curative effect on disease,in which treatment group has better curative effect than control group.
