| Research purposes:This study intends to start from the activation effect of chiropractic on skin mast cells,to explore whether the mechanical stimulation of local skin by chiropractic therapy can activate the local skin mast cells of the Du Meridian acupoint and whether the activation of the stimulus and endocannabinoid system is activated.Relatedly,the relationship between chiropractic therapy and mast cells and endocannabinoid system was discussed,which laid a foundation for the in-depth study of the mechanism of traditional chiropractic therapy and provided ideas for its clinical effects research.research method: 1)Meta-analysis of domestic clinical research literature on body surface stimulation in children with mental retardation。 Through the systematic evaluation of related literatures on the treatment of children with mental retardation based on body surface stimulation,the Chinese Knowledge Network Database(CNKI),the Wanfang Database Knowledge Service Platform,and the Chinese Science and Technology Periodical Database(Vip)are used as data sources to retrieve the body surface.Stimulation of Chinese literature on the treatment of mental retardation.According to the inclusion criteria and exclusion criteria of the study,a total of 16 articles were included in the study.The treatment regimens were mainly acupuncture,rehabilitation,drugs,massage,auricular acupoints,and Chinese medicine fumigation.2)Immediate effect of chiropractic on the activation of skin mast cells in the acupoints of rats。 A total of 48 male Sprague-Dawley(SD)rats,aged 6-7 weeks,were randomly divided into normal group and shielding group.The two groups were randomly divided into control group(6 rats),light stimulation group(6 rats),middle stimulation group(6 rats),and re-stimulation group(6 rats).After one week of adaptive feeding,the control rats only simulated the grasping action.For the treatment,the light,medium and heavy stimulation groups were given chiropractic ridges 7,14,21 times.The skin tissues of Dazhui(DU14),Zhiyang(DU9)and Mingmen(DU4)acupoints of normal group and shielding group were selected respectively.The aggregation and degranulation of mast cells in mast cells were observed by toluidine blue staining.Three immunohistochemical methods were used to detect the aggregation.Expression of CB1 and CB2 receptors at Traditional Chinese Medicine were selected from 6 to 7 weeks old.They were randomly divided into 4 groups,6 in the blank group,and 6 in the chiropractic light stimulation group.6 in the chiropractic re-stimulation group.After one week of adaptive feeding,the rats in the blank group only simulated the grasping action,and the light,medium and heavy stimulation groups were given chiropractic ridges for 7,14,21 times,and the chiropractic treatment was performed at a fixed time every day,and treated for 5 days per week.2 days,for 3 consecutive weeks,a course of treatment was taken after the end of the experiment.4)Clinical study of chiropractic therapy for children with mental retardation。 All cases were from mental retardation patients in the rehabilitation department of the People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine.Clinically,30 children with mental retardation were diagnosed according to their medical history,age,clinical symptoms and related examinations.According to the inclusion criteria,according to the random number table.The method was randomly divided into 2 groups,all of whom received routine rehabilitation training.The blank group(15 cases)only received routine rehabilitation training,and the chiropractic group was chiropractic plus conventional rehabilitation training(15 cases),each treatment of chiropractic 7 times a day.1 time,5 times a week,2 days off,for 3 months.Before the treatment,1 month,2 months,and 3 months after treatment,the infant development rating scale and the Gesell development scale were used for evaluation,and the spss 22.0 software was used for statistical analysis.Research result:1)Through the screening and analysis of clinical literature,a total of 15 clinical research literalness for the treatment of mental retardation were included,in which body surface stimulation combined with conventional rehabilitation treatment combined with conventional rehabilitation therapy,combined with RR value in total clinical effective rate and DQ value(MH method)and 95% CI were 1.21(1.1~1.34),4.42(1.85~7.0),P<0.05,suggesting that surface stimulation therapy combined with conventional rehabilitation has certain advantages;while acupuncture combined with conventional rehabilitation therapy is more conventional than conventional rehabilitation.In the rehabilitation group,the RR value(MH method)and 95% CI were 1.2(1.08 to 1.34),respectively,while the DQ total value RR was 4.80,95% CI(1.56 to 8.04),and the RR of the speech DQ.= 4.51,95% CI(-7.2 ~ 16.23),P value is less than 0.05,suggesting that acupuncture combined with conventional rehabilitation also has certain advantages.2)The immediate effect of chiropractic on the activation of mast cells in the Du Meridian acupoints of rats showed that the degranulation rate of mast cells in Dazhui(DU14)and Zhiyang(DU9)points increased in the light stimulation group and the re-stimulation group compared with the blank group,and was significant.The difference was(P < 0.05),and the mast cell degranulation rate of Mingmen(DU4)acupoint was significantly increased in the re-stimulation group compared with the blank group(P<0.05),and the above phenomenon disappeared after the injection of sodium cromoglycate solution(shielding group).There was no significant difference in the number of mast cells,degranulation number and degranulation rate between the groups(P>0.05).3)After 3 weeks of continuous chiropractic intervention,the degranulation rate of mast cells in the Zhiyang(DU9)and Mingmen(DU4)acupoint groups increased in the peripheral group,and there was statistical significance(P<0.05).The re-stimulation group of ZhiYang(DU9)points also showed a significant increase(P<0.05),and the results of immunohistochemistry showed that the protein expression of CB1 in the local area of Zhiyang(DU9)was significantly higher in the stimulation group than in the blank group and the light stimulation group.There was a significant difference(P < 0.05),and there was a significant difference(P < 0.05)between the re-stimulation group and the light stimulation group.The results of CB1 in the Mingmen(DU4)point showed that there was a significant increase in the middle stimulation group and the blank group and the light stimulation group,and there was a significant difference(P<0.05),while the CB2 protein expression in the middle stimulation group was significantly higher than that in the blank group.(P<0.01).At the central stage,there was no significant difference in the expression of CB1 in the hippocampus of each group(P>0.05),while the CB2 protein expression in the mild,moderate and severe stimulation groups was significantly different from the blank group(P<0.05).In the brain,there was a significant difference between the expression of endogenous cannabinoid degrading enzyme(FAAH)in the re-stimulation group and the light-stimulated group and the blank group(P<0.001),and there was also a significant difference between the middle-stimulated group and the blank group.(P<0.05).4)Compared with the conventional rehabilitation group,the chiropractic group scored significantly and significantly(P<0.05)in the social and life-study ability scores of the infants’ junior high school students’ social life scale after 1 month of intervention.The increase was significantly different from the next 2 months(P<0.05).Conclusion: 1)The immediate effect of chiropractic treatment can degranulate the surface superficial mast cells of Dazhui(DU14),Zhiyang(DU9)and Mingmen(DU4)acupoints in rats,and the effect is related to the stimulation intensity of chiropractic.The greater the stimulation intensity,the more obvious the degranulation phenomenon.2)The immediate effect of chiropractic on the degranulation of the local surface mast cells of Dazhui(DU14),Zhiyang(DU9)and Mingmen(DU4)acupoints in rats can be blocked by sodium cromoglycate blockers.After three weeks of chiropractic treatment,the expression of CB2 receptor protein in the hippocampus of rats was significantly increased,and was also associated with stimulation intensity.3)Under the effect of three weeks of treatment,the thirsty stimulation of Chiropractic can significantly promote the degranulation of the surface of the large vertebrae,the Zhiyang(DU9)and the Mingmen(DU4)acupoints in the rat,and the strong chiropractic stimulation can make the rat The expression of endogenous cannabinoid receptors CB1 and CB2 was increased in the veins to the acupoints and the Mingmen(DU4).4)Under the three-week course of treatment,chiropractic treatment significantly increased the expression of CB2 receptor protein in the hippocampus of rats,and was also associated with stimulation intensity.5)After three weeks of chiropractic treatment,the expression of the source cannabinoid degrading enzyme(FAAH)in the rat cortex was increased and correlated with the stimulation intensity.6)Chiropractic therapy combined with conventional rehabilitation therapy has a certain improvement effect on social interaction and participation ability of children with mental retardation,and this improvement effect is more significant in the first month of intervention. |
