Clinical Experience Of Treatment Based On Syndrome Differentiation Of Professor Chen Xiaoning In Treating Nasal Diseases

Object:Take the treatment of nasal diseases of Professor Chen Xiaoning as the clinical research objects,by taking kinds methods,including Data mining and interview,to explore the clinical experience and academic thought of clinical treating nasal diseases based on syndrome differentiation of TCM(Traditional Chinese medicine).Then,the fuzzy and complicated clinical data can be clearly defined,to reveal the inheritance and innovation of his academic thought under the influence of the Chinese medicine masters.Professor Gan Zuwang.And it also provides clues and directions for the diagnosis and treatment of nasal diseases in TCM.Method:(1)Through literature review,the current status of the diagnosis and treatment of rhinopathy was summarized,so did the data mining research of ENT and clinical experience of Professor Chen Xiaoning.(2)Medcase v3.2 data record mining system was used to record the medical records of Professor Chen Xiaoning’s clinical diagnosis and treatment of nasal diseases from 2016 to 2019,and a special data base was established.1066 cases(including 200 cases of allergic rhinitis,200 cases of epistaxis,200 cases of chronic rhinitis,200 cases of chronic sinusitis,200 cases of adenoid hypertrophy in children,66 cases of nasal polyps)were examined by the methods of frequency method,association rules,cluster analysis,focusing on the mining of symptoms,syndrome elements,tongue manifestation,prescriptions and other contents in the medical records,which were analyzed and summarized by combined with Professor Chen Xiaoning’s clinical experience.(3)The interview was conducted in the form of famous traditional Chinese medicine interview,disease name,syndrome type(syndrome element),treatment,medication,academic inheritance and so on were recorded and discussed in details on the views of Professor Chen Xiaoning.Result:(1)The disease nature of nasal diseases is mainly based on deficiency in origin and excess in superficislity,that is to say,deficiency of viscera are often accompanied with excessiveness of pathogen,among which,there are many exterior pathogen.(2)Core syndrome elements of allergic rhinitis are deficiency of lung,cold wind,core formula:Huangqi,Baizhu,Baishao,Guizhi.Baizhi,Xixin,Xinyi,Cangerzi,Chaihu,Xuchangqing,Gancao.Core syndrome elements of chonic sinusitis are damp heat,spleen deficiency,lung deficiency,core formulas:1.Huoxiang,Peilan,Xiakucao,Baizhi,Chenpi,Yuxingcao;2.Huangqi(roasted),Baizhu,Guizhi,Baizhi.Formula 1 and 2 can be used as the core formulas of two groups of syndrome types,and can also be used as the king’s formula and the minister’s one in facing with concurrent disease,to treat the mixed syndrome of deficiency and excess.Core syndrome elements of chronic rhinitis are lung deficiency,spleen deficiency,core formula:Huangqi(roasted),Taizishen,Baizhu,Baishao,Guizhi.Baizhi,Xinyi,Chaihu,Bohe,Gancao.Core syndrome element of epistaxis is lung heat,core formula:Sangbaipi,Huangqin,Shengdihuang,Mudanpi.Xiakucao,Jiaozhizi,Niuxi,Baimaogen,Oujie.Core syndrome elements of adenoid hypertrophy in children is phlegm dampness,core formula:Chenpi,Banxia(roasted),Fuling,Beimu,Shanzha,Baizhi,Cangerzi,Chaihu,Xiakucao,Gancao.Core syndrome elements of nasal polyps:lung deficiency,cold wind,damp heat,core formula:1.Huangqi,Fangfeng,Baizhu,Xixin,Xinyi,Cangerzi,Gancao;2.Huoxiang,Peilan,Huangqin,Yuxingcao,Xiakucao,Baizhi,Xinyi.Conclusion:(1)Through literature review,it is found that traditional Chinese medicine(TCM)has advantages and characteristics in the treatment of nasal diseases,with a wide clinical group.It is also found that this study is the first attempt by using clinical large sample medical cases to do data mining research and famous TCM doctor interview research on Professor Chen Xiaoning’s clinical experience in the treatment of nasal diseases,which can more comprehensively summarize his clinical experience and academic views;(2)There are many commonalities in the process of nasal diseases diagnosis and treatment,which can be looked as a kind of disease by syndrome differentiation and treatment,with different drugs among different diseases;the relationships among the main symptoms,syndrome elements and drugs has guiding significance for clinical treatments;(3)the core formula can be obtained by frequency method,association rule method and K-means method of prescriptions.The results of K-means method is better than the others.The core components of formula are not only affected by high-frequency drug use,but also by internal and external association between them and so on.Therefore,the results of K-means method are also the validation of the other two algorithms.Unique prescription for special nasal diseases combining with syndrome differentiation are discovered;(4)in the interview study of traditional Chinese medicine,Professor Chen Xiaoning’s self summary on the diagnosis,treatment and academic inheritance of nasal diseases has effectively explained,supplemented and corrected the objective research results.
