ObjectiveTaking the patients with bell facial paralysis as the research object,the correlation between the onset of bell facial paralysis and the occurrence of Yun Qi factors was analyzed retrospectively,aiming to explore the scientific nature of Yun Qi theory,and provide the basis for the clinical application of Yun Qi theory in the prevention and treatment of bell facial paralysis.At the same time,in order to further clarify the influence of Shenzhen’s climate on the onset of bell facial paralysis,and fully reflect the academic thought of“nature and man correspond”in the theory of Yun Qi in TCM,in this study,the distributed lag nonlinear model was selected to analyze the influence of meteorological factors such as air temperature,relative humidity,wind speed,water vapor pressure,atmospheric pressure,visibility,total cloud cover,sunshine duration,daily evaporation and solar radiation on the incidence of bell facial paralysis,so as to obtain more valuable research results and provide scientific basis for reducing the adverse effects of meteorological factors on human health.MethodsThis study was a retrospective study.In the first part,according to the case screening criteria,all the hospitalized patients meeting the above criteria from January 20,2010 to January 19,2020 were collected,including a total of 841 patients.The required case information was collected and the accurate onset date of the patients was calculated.The five circuits and six qi were deduced according to the Heavenly Stems and Earthly Branches,at the same time,the total number of inpatients in each Yun Qi period was collected,and the number of non bell facial paralysis patients in the same period was calculated.The chi-square test was used to analyze the number of hospitalized patients with and without bell facial paralysis in each Yun Qi period.At the same time,the chi-square test was also used to analyze the proportion of hospitalized patients with bell facial paralysis in terms of solar terms and seasons.In the second part of the study,according to the case screening criteria,all the hospitalized patients who met the above criteria were collected from January 1,2009 to February 29,2020.A total of 863 patients were included,and the required case information was collected and the accurate onset date of the patients was calculated.The meteorological data,provided by the Shenzhen Meteorological Bureau and recorded by the Shuanglong Automatic Meteorological Observation Station of the National Climate Observatory,include the daily average temperature(℃),relative humidity(%),wind speed(0.1m/s),vapor pressure(HPA),atmospheric pressure(HPA)and other meteorological data from January 1,2009 to February 29,2020.By using R software,the distributed lag nonlinear model was selected to analyze the influence of the above meteorological factors on the incidence of bell facial paralysis under the meteorological environmental conditions in Shenzhen.Results1.An exploratory study on the incidence of bell facial paralysis based on the theory of Yun Qi1.1 The proportion of hospitalized patients with bell facial paralysis was analyzed based on the theory of“more and less”,the results showed that the proportion of bell facial paralysis patients in hospital was the highest in Xin Year(1.01%),which was the year when Shui Yun was not enough;The lowest was in the Geng Year(0.44%),which was the year when Jin Yun was too much.The difference was statistically significant(χ~2=33.864,P=0.000<0.05).1.2 The proportion of hospitalized patients with bell facial paralysis was analyzed based on the theory of the six qi of Zhu Qi,the results showed that the proportion of bell facial paralysis patients in hospital was the highest in the primary qi-Jue Yin Feng Mu(0.91%);The lowest was in the second qi-Shao Yin Jun Huo(0.63%).But the difference was not statistically significant(χ~2=10.806,P=0.055>0.05).1.3 The proportion of hospitalized patients with bell facial paralysis was analyzed based on the theory of the six qi of Si Tian,the results showed that the proportion of bell facial paralysis patients in hospital was the highest in the Si Tian of Tai Yin Shi Tu(0.93%);The lowest was in the Si Tian of Shao Yin Jun Huo(0.42%).The difference was statistically significant(χ~2=13.316,P=0.021<0.05).1.4 The proportion of hospitalized patients with bell facial paralysis was analyzed based on the theory of the six qi of Zai Quan,the results showed that the proportion of bell facial paralysis patients in hospital was the highest in the Zai Quan of Shao Yin Jun Huo(0.97%);The lowest was in the Zai Quan of Shao Yang Xiang Huo(0.49%).The difference was statistically significant(χ~2=25.319,P=0.000<0.05).2.An exploratory study on the incidence of bell facial paralysis based on solar terms and seasonal factors2.1 The proportion of hospitalized patients with bell facial paralysis was analyzed based on the 24 solar terms,the results showed that the proportion of bell facial paralysis patients in hospital was the highest in the Beginning of Spring(1.15%);The lowest was in Slight Snow(0.44%).But the difference was not statistically significant(χ~2=34.811,P=0.054>0.05).2.2 The proportion of hospitalized patients with bell facial paralysis was analyzed based on the season(divided by solar terms),the results showed that the proportion of bell facial paralysis patients in hospital was the highest in spring(0.77%);The lowest was in winter(0.69%).But the difference was not statistically significant(χ~2=2.035,P=0.565>0.05).According to the24 solar terms,the four seasons were divided into twelve seasons for further analysis.The results showed that the proportion of hospitalized patients with bell facial paralysis was the highest in Meng Chun(1.11%).The lowest value was 0.50%in Meng Dong;The difference was statistically significant(χ~2=27.319,P=0.004<0.05).3.Under the meteorological conditions in Shenzhen,the influence of meteorological factors on the incidence of bell facial paralysis was analyzed based on the distributed lag nonlinear model3.1 In the analysis of the overall effect of air temperature,RR value showed a decreasing trend with the increase of air temperature.When the average temperature of the day was less than 25℃,it would increase the risk of bell facial paralysis.The risk of bell facial paralysis was decreased when the average temperature was>25℃.When the average temperature of the day was>30℃,there was no statistical correlation with the occurrence of the disease,suggesting that the influence of air temperature on the incidence of bell facial paralysis was nonlinear,and the incidence risk of bell facial paralysis was different at different temperatures.The 3D diagram and thermal diagram of the correlation between different daily mean air temperature and the incidence of bell facial paralysis suggested that different daily mean air temperature had different effects on the incidence of bell facial paralysis in different lagged days.3.2 In the analysis of the overall effect of relative humidity,RR value showed a decreasing trend with the increase of relative humidity.When the average daily relative humidity was less than 76%,it would increase the risk of bell facial paralysis;When the average relative humidity was more than78%,the risk of bell facial paralysis would be reduced,and there was a statistical correlation with the occurrence of the disease,suggesting that the influence of relative humidity on the incidence of bell facial paralysis was nonlinear,and the incidence risk of bell facial paralysis would be different with different relative humidity.The 3D diagram and thermal diagram of the correlation between different daily mean relative humidity and the onset of bell facial paralysis suggested that different daily mean relative humidity had different effects on the onset of bell facial paralysis in different lagged days.3.3 In the analysis of the overall effect of wind speed,with the increase of daily average wind speed,the risk of bell facial paralysis also increased gradually.Since the average daily wind speed was greater than 2.6m/s,RR value increased significantly,suggesting that the influence of wind speed on the incidence of bell facial paralysis was nonlinear,and the risk of bell facial paralysis increased gradually with the increase of wind speed.The 3D diagram and thermal diagram of the correlation between different daily mean wind speed and the incidence of bell facial paralysis suggested that different daily mean wind speed had different effects on the incidence of bell facial paralysis in different lagged days.3.4 In the analysis of the overall effect of water vapor pressure,RR value showed a trend of gradual increase with the increase of water vapor pressure.When the average water vapor pressure of the day was less than 229hpa,it would reduce the risk of bell facial paralysis.When the average water vapor pressure was more than 231hpa on that day,it would increase the risk of bell facial paralysis,and there was a statistical correlation with the occurrence of the disease,suggesting that the influence of water vapor pressure on the incidence of bell facial paralysis was nonlinear,and the risk of bell facial paralysis varies with different water vapor pressure.The 3D diagram and thermal diagram of the correlation between different daily mean water vapor pressure and the incidence of bell facial paralysis suggested that different daily mean water vapor pressure had different effects on the incidence of bell facial paralysis in different lagged days.3.5 In the analysis of the overall effect of atmospheric pressure,with the increase of atmospheric pressure,RR value showed a trend of gradual increase and then gradual decrease.When the daily average atmospheric pressure was less than 1005hpa,it would increase the risk of bell facial paralysis.When the daily average atmospheric pressure was more than 1006hpa,it would reduce the risk of bell facial paralysis.when the daily average atmospheric pressure was in the range of 1010-1021hpa,there was a statistical correlation with the incidence of the disease,suggesting that the influence of atmospheric pressure on the incidence of bell facial paralysis was nonlinear,and the incidence risk of bell facial paralysis was different under different atmospheric pressure.The 3D diagram and thermal diagram of the correlation between different daily mean atmospheric pressure and the onset of bell facial paralysis suggested that different daily mean atmospheric pressure had different effects on the onset of bell facial paralysis in different lagged days.3.6 In the analysis of the overall effect of visibility,RR value showed a trend of decreasing first and then increasing with the increase of visibility.When the daily average visibility was less than 17km or more than 26km,it would increase the risk of bell facial paralysis.When the daily average visibility was between 18-25km,it would reduce the risk of bell facial paralysis,indicating that the impact of visibility on the incidence of bell facial paralysis was nonlinear,and the risk of bell facial paralysis varied with different visibility.The 3D diagram and thermal diagram of the correlation between different daily mean visibility and the incidence of bell facial paralysis suggested that different daily mean visibility had different effects on the incidence of bell facial paralysis in different lagged days.3.7 In the analysis of the overall effect of total cloud cover,with the increase of total cloud cover,RR value showed a trend of slow decrease at first and then gradual increase.When the daily average total cloud cover was less than 66%or more than 71%,it would increased the risk of bell facial paralysis.When the daily average total cloud cover was between 67-69%,the incidence risk of bell facial paralysis was reduced,suggesting that the influence of total cloud cover on the incidence of bell facial paralysis was nonlinear,and the incidence risk of bell facial paralysis was different under different total cloud cover.The 3D diagram and thermal diagram of the correlation between different daily mean total cloud cover and the incidence of bell facial paralysis suggested that different daily mean total cloud cover had different effects on the incidence of bell facial paralysis in different lagged days.3.8 In the analysis of the overall effect of sunshine duration,RR value showed a trend of gradual decrease,gradual increase and then gradual decrease with the extension of sunshine duration.When the sunshine duration was less than 5h or more than 10h,it would reduce the risk of bell facial paralysis.When the sunshine duration was between 6-9h,it would increase the risk of bell facial paralysis,suggesting that the influence of sunshine duration on the incidence of bell facial paralysis was nonlinear,and the risk of bell facial paralysis varied with different sunshine duration.The 3D diagram and thermal diagram of the correlation between different sunshine duration and the onset of bell facial paralysis suggested that different sunshine duration had different effects on the onset of bell facial paralysis in different lagged days.3.9 In the analysis of the overall effect of daily evaporation,RR value showed a trend of gradual increase,gradual decrease and then gradual increase with the increase of daily evaporation.When the daily evaporation was less than 1mm or within the range of 4-7mm,the risk of bell facial paralysis would be reduced.When the daily evaporation was in the range of 2-3mm or more than8mm,it would increase the risk of bell facial paralysis,suggesting that the influence of daily evaporation on the incidence of bell facial paralysis was nonlinear,and the risk of bell facial paralysis varied with different daily evaporation.The 3D diagram and thermal diagram of the correlation between different daily evaporation and the onset of bell facial paralysis suggested that different daily evaporation had different effects on the onset of bell facial paralysis in different lagged days.3.10 In the analysis of the overall effect of solar radiation,RR showed a slow decreasing trend with the increase of solar radiation.When the daily average solar radiation was less than 157W/m~2,it would increases the risk of bell facial paralysis.When the daily average solar radiation was more than159W/m~2,it would reduce the risk of bell facial paralysis.When the daily average solar radiation was in the range of 248-261W/m~2,it was statistically correlated with the incidence of the disease,suggesting that the influence of solar radiation on the incidence of bell facial paralysis was nonlinear,and the incidence risk of bell facial paralysis varied with different solar radiation.The 3D diagram and thermal diagram of the correlation between different daily mean solar radiation and the incidence of bell facial paralysis suggested that different daily mean solar radiation had different effects on the incidence of bell facial paralysis in different lagged days.Conclusion1.In terms of the proportion of hospitalized patients with bell facial paralysis,from the perspective of“more and less”,the highest proportion was Xin Year,which was the year when Shui Yun was not enough;From the perspective of the six qi of Zhu Qi,the primary qi-Jue Yin Feng Mu was the highest;From the perspective of the six qi of Si Tian,the Si Tian of Tai Yin Shi Tu was the highest;From the perspective of the six qi of Zai Quan,the highest proportion was the Zai Quan of Shao Yin Jun Huo;From the perspective of the season,Meng Chun(the Beginning of Spring,Rain Water)was the highest.2.Under the meteorological conditions in Shenzhen city,the influence of meteorological factors on the incidence of bell facial paralysis was nonlinear.In addition to the wind speed,the influence of meteorological factors on the risk of bell facial paralysis showed a trend of increasing first and then decreasing or decreasing first and then increasing with the gradual increase of the daily average.In the analysis of the overall effect of wind speed,the risk of bell facial paralysis increased gradually with the increase of the daily average wind speed.In addition,different meteorological factors had different effects on the onset of bell facial paralysis in different lag days.
