Discussion On Syndrome Differentiation And Prescription Of Primary Osteoporosis Based On Literature Data Mining

Objective : Based on the literature data mining,this paper systematically studies the prescriptions for the treatment of primary osteoporosis,and summarizes the syndrome differentiation and medication rules of the disease.The purpose is to inherit and carry forward the experience of traditional Chinese medicine in the diagnosis and treatment of primary osteoporosis,and to provide literature basis and theoretical reference for improving the level of clinical treatment.Methods: Literature research and data mining technology were applied to analyze and study the prescriptions for the treatment of primary osteoporosis.Using the method of philology to collect the ancient and modern literature,the selected prescriptions were summarized and standardized,and then frequency analysis,factor analysis,cluster analysis and association rule mining analysis were carried out.According to the data mining results,combined with professional knowledge,the syndrome differentiation and prescription medication rules were systematically summarized and discussed.Results: 1.Results of frequency analysis: according to the results of frequency analysis,Radix rehmanniae,Eucommia ulmoides,Poria cocos,Achyranthes bidentata and Cornus officinalis were frequently used by doctors of different generations,and most of them were tonic drugs,warming internal medicine,promoting blood circulation and removing blood stasis drugs,diuresis and dampness drugs and wind dampness drugs.The distribution of the nature and taste of the high-frequency drugs of each generation is basically the same.The most common drugs are warm and flat.The main drugs are sweet,pungent and bitter.The most common drugs are entering the kidney,liver and spleen meridians.2.The results of factor analysis,cluster analysis and association rule analysis: the prescriptions of Qin Han Dynasty to the fifth generation,Song Jin Yuan,Ming Dynasty,Qing Dynasty,modern times and the prescriptions of blood stasis syndrome type in modern times have generated 10,12,11,12,11 and 10 factors respectively by factor analysis.The results of each generation of formula cluster analysis were analyzed and 4,6,7,7,6 and5 cluster categories were generated.The combination of 22,30,17,22,17 and 12 groups of prescriptions was formed by association rule analysis.The syndrome types of osteoporosis directly and indirectly reflected by the factor analysis and cluster analysis of ancient and modern doctors can be divided into the following categories: syndrome of deficiency of qi and blood in viscera,deficiency of heat,cold and wet syndrome,blood stasis syndrome and qi stagnation syndrome.The therapeutic effect of the drugs analyzed according to the association rules can indirectly reflect the syndrome elements of osteoporosis: kidney deficiency,spleen dampness,liver deficiency,blood deficiency,Yang deficiency,yin deficiency,blood stasis,cold and wet,qi deficiency and kidney essence.Conclusion:1.The main pathological changes of primary osteoporosis are kidney deficiency,liver,spleen and kidney often interact as diseases;deficiency of kidney essence,liver and spleen failure,disharmony of qi and blood,imbalance of Yin and Yang,invasion of external pathogens,unfavorable axis,loss of daily life,sour and salty taste,lack of appetite,excessive five fatigue and five wills,and multiple deficiency and stasis are the common causes and pathogenesis characteristics of primary osteoporosis.2.From the Qin and Han Dynasties to the Five Dynasties,the syndrome differentiation system of kidney yang deficiency and cold dampness was the main body.To the Song,Jin,Yuan and Ming Dynasties,the doctors paid attention to the syndrome differentiation of liver and spleen on the basis of kidney deficiency,rather than only from the kidney.From the Ming and Qing Dynasties,the proportion of kidney yin deficiency increased,and the syndrome differentiation of yin deficiency,fire excess and blood stasis began to appear.At the same time,syndrome differentiation and treatment should be given attention to liver deficiency and spleen stasis.With the deepening of the understanding of the etiology and pathogenesis of the disease,syndrome differentiation has experienced the evolution of various times,and the development is more comprehensive and rich.The evolution of syndrome differentiation law reflects the concept of disease,prescription and body of different generations of doctors in different times.Due to the difference of academic schools and mainstream physicians’ cognition,there are some emphases on syndrome differentiation,but every era has made progress.3.In clinical treatment,we should grasp the principle of kidney deficiency and liver and spleen at the same time.The prescriptions are often combined with Tonifying the liver and kidney,tonifying qi and spleen,nourishing blood,removing dampness and dredging collaterals,soothing the liver and regulating qi,reconciling Shaoyang and invigorating the spleen and removing dampness.In a certain sense,the choice of drug properties,taste and meridian tropism reflects the theory of Yin and Yang and the theory of five elements.Pay attention to the theory of mutual root and mutual use of yin and Yang.Nourishing yin and tonifying kidney is easy to nourish greasy and generate dampness,which is often combined with the products of diuresis and spleen strengthening.Pay attention to regulating qi and blood,kidney tonifying drugs are often used in combination with blood tonifying and blood activating drugs,qi tonifying and qi activating drugs.Warming Kidney Yang is often accompanied by dispelling dampness and dispersing cold;nourishing Yin medicine is often accompanied by clearing away deficiency fire.Chronic diseases are often accompanied by blood stasis,and we should pay attention to promoting blood circulation and removing blood stasis.
