| CHAPTER ONE THE RELATIONSHIP BETWEEN PREVALENCE OF PCOS AND VISFATIN OF ZHUANG AND HAN WOMEN AT CHILDBEARING AGE IN WESTERN GUANGXIObjective: To understand the prevalence and clinical features of polycystic ovarian syndrome(PCOS)in Han and Zhuang fertile women of Western Guangxi.The prevalence,clinical manifestations and related factors of PCOS in this area were investigated to provide theoretical basis for revising national diagnostic criteria and seeking possible high-risk pathogenic factors in the area.Methods: According to the diagnostic criteria recommended by ESHRE/ASRM,epidemiological cluster sampling method was followed to research the prevalence of PCOS in 2287 cases of childbearing age,who were also given physical examination,type-B ultrasonic,detection of glucolipid metabolism,hormones,fasting blood glucose,insulin,visfatin,etc.Results:(1)228 patients with PCOS were diagnosed,among which 99 were Zhuang women and 129 were Han.The total incidence of PCOS was 7.95%,9.54% for Zhuang women,7.05% for Han women.Those fertile women less than or equal to 35 years old were at high risk.(2)The clinical manifestations of PCOS patients of childbearing age included oligomenorrnea,crinosity,obesity,hyperandrogenism,LH/FSH>2 and ovary ultrasonic change,and their proportions were respectively 77.19%,26.75%,33.33%,58.13%,49.12%and 41.67% in the total number of women with PCOS.(3)There were 2abnormal manifestations as follows: 81 patients with oligomenorrnea + PCOS,(1)accounting for 35.52%,(2)91 patients with oligomenorrnea + hyperandrogenisn,accounting for 40.79%,(3)85 patients with PCOS + hyperandrogenism,accounting for 37.28%,(4)71 patients with acne + hyperandrogenism,accountoffing for 35.52%.Changes in oligomenorrnea,hyperandrogenism,F-G assessment and BMI of Zhuang patients with PCOS were more obvious.(4)Compared with the normal control group Zhuang and Han PCOS group had obviously higher density of serum visfatin and.The difference was statistically significant(p<0.05).(5)Serum visfatin levels in the patients with PCOS were positively correlated with HOMA-IR,T,BMI,WHR,FBG,FINS(p<0.05);moreover,PCOS was closely related to menstrual disorders,family history of diabetes,BW,MBI.Conclusion:(1)The survey found for the first time: The incidence of PCOS in fertile women of western Guangxi was 7.95%,among which 9.54% for Zhuang women,7.05% for Han women.Women less than or equal to 35 years old were at high risk.(2)PCOS patients of childbearing age in western Guangxi showed high heterogeneity clinically,mainly in oligomenorrnea and hyperandrogenism,in which Zhuang patients were more obvious.(3)Fertile women with PCOS in western Guagnxi had obviously higher density of serum vasfatin and HOMA-IR.The pathogenic process of PCOS could be related to visfatin.(4)PCOS in women of childbearing age in western GX is related with menstrual disorder,family history of diabetes,BW and BMI.It is suggested that prevention and cure of PCOS should begin with change life style.CHAPTER TWO EXPRESSION OF VISFATIN IN RATS WITH PCOS AND THE INTERVENTION OF MALT EXTRACT ON PCOS RATObjective :To establish PCOS rat models to further explore the effect of malt extract on the sexual cycle,ovarian morphology,sex hormone,serum and expression of visfatin in adipose issue of PCOS rats,which aims at seeking new methods to cure PCOS.Methods:(1)Experimental rat models of PCOS were established.(2)The rats were divided into normal control group,PCOS model control group,low-dose malt extract group,middle-dose malt extract group and high-dose malt extract group.Changes in the sexual cycles,ovarian morphology,sex hormones,serums and expression of visfatin in adipose tissue of the experimental rats were detected after medical intervention.Results:(1)PCOS rat models were successfully established.(2)Compared with those of PCOS control group,the testosterone and luteinizing hormone level of PCOS rats were significantly decreased by the intervention of malt extract.(3)Compared with those of PCOS control group,the insulin resistence of PCOS rats and their serum,visfatin level in adipose tissue were significantly reduced by high-dose malt extract.Conclusion:(1)PCOS rat models induced by letrozole were the ideal animal models for POCS study,since they manifested estrus cycle change,polycystic ovary syndromes,hyperandrogenism and IR obviously and steadily;(2)The serum and ovarian visfatin levels of PCOS rat increased.(3)This study found for the first time that malt extract can lower the serum and ovarian visfatin levels of PCOS rats,improve IR as well as reduce the androgen and LH levels,which promote the clinical sign of PCOS rat,suggesting that malt extract has good effect on the treatment PCOS rat. |
