| In the current society,limited medical and healthcare resources can no longer meet people’s growing medical needs,doctor-patient conflicts are frequent,and doctorpatient relations are becoming increasingly tense.The emergence and development of Internet medical care have brought tremendous changes and value to medical staff,patients,relevant participants in the medical industry,and the whole society.Internet medical care has enabled more and more doctors and patients to use online healthcare services.Online healthcare services are continuously entering daily life and becoming a supplement and extension of traditional offline healthcare services.Internet medical care has brought about subversive changes in the interaction behaviors and methods between doctors and patients.And it has provided an effective way to alleviate and solve the problems of insufficient medical care resources,doctor-patient conflicts,and doctorpatient relations.In the context of Internet medical care,researching the problems of”doctor-patient behavior” and ”doctor-patient relationship”,exploring the characteristics of doctor-patient behavior and the doctor-patient relationship,analyzing the deep causes of current doctor-patient contradictions and tense doctor-patient relationship,and proposing a solution in China has important theoretical and practical significance.This thesis organized research content focusing on ”doctor-patient behavior” and”doctor-patient relationship” in the context of Internet medical care,combining partial analysis about doctor-patient behavior with overall research about doctor-patient interaction strategies and the doctor-patient relationship.First of all,from a partial perspective,this thesis started with research on the influence of medical information and medical resources on doctor-patient behavior.Then,from the overall perspective,it studied doctor-patient interaction strategies and the mechanism of the change of the doctor-patient relationship against the interaction process of doctors and patients.Finally,based on the systematic research on the behavior of both doctors and patients and the doctor-patient interaction process,we summarized the characteristics of doctorpatient behavior and doctor-patient relationship under the background of Internet medical care.Then,we used blockchain and Internet medical care to figure out solutions and practical guidance for solving doctor-patient conflicts and improving the doctor-patient relationship.Reviewing the domestic and foreign literature,the current research has the following shortcomings: Firstly,the existing research mainly focuses on the prediagnosis and online diagnosis,ignoring the study of patient behavior patterns in the post-diagnosis follow-up service.Secondly,there is a gap in the impact of medical resources on the price premium of doctors’ online medical services.Thirdly,in the process of research on doctor-patient interaction,the influencing factors and underlying mechanism of doctor-patient interaction strategies and doctor-patient relationship are not considered from a holistic perspective.Fourthly,the current research has not yet discussed how to propose a systematic solution to solve the problem of doctors and patients based on blockchain technology and Internet medical care.The specific research content and conclusions of this thesis are as follows:1.Taking patients as the research object,based on the ELM theory,study the influence of online healthcare information on patients’ online follow-up visits.Based on the service quality theory,this study used technical quality and interpersonal quality to measure the quality of doctors’ medical services,and used votes as a proxy variable for doctors’ e WOM.This study regarded the service quality as processed using the central route and considered e WOM of doctors as processed using the peripheral route.This study hypothesized that both technical quality and interpersonal quality and e WOM can positively impact a patient’s online follow-up intention.In addition,this study assumed whether it is a high-privacy disease and whether a doctor provides private doctor services as the moderating variables,then this study hypothesized that high-privacy disease and private doctor services can moderate the relationships between technical quality,interpersonal quality,or e WOM and a patient’s online follow-up intention.Based on 18,590 real data from an OHC(haodf.com),an empirical model was established to test the proposed hypothesis.The research results showed that:(1)The technical quality and interpersonal quality of a doctor’s medical services have a significant positive impact on a patient’s intention to use online follow-up services after offline treatment;(2)A doctor’s e WOM has a significant positive impact on a patient’s online follow-up intention;(3)The high-privacy diseases positively moderated the relationship between technical quality and a patient’s online follow-up intention,and negatively moderated the relationship between e WOM and a patient’s online follow-up intention;(4)The technical quality and e WOM of a doctor who offers online private doctor services have a stronger influence on a patient’s online follow-up intention.Online private doctor service will strengthen the influences of service quality and e WOM.2.Taking doctors as the research object,study how the market conditions of medical resources in the online medical market affect the price premium of doctors’ online medical services.This study used resource supply and resource concentration to characterize the market conditions of medical resources in the online medical service market.This study hypothesized that the supply of resources has a negative impact on price premiums,and the resource concentration has a positive impact on price premiums.Besides,this study hypothesized that high-privacy diseases will positively affect price premiums and moderate the relationship between medical resources and price premiums.By collecting the online medical service price data of 68,945 doctors on an OHC,the study established an empirical model to test the above hypothesis.The research results showed that:(1)The increase in the supply of medical resources in the online medical market will lead to a reduction in the price premium of online medical consulting services;(2)The concentration of medical resources has a significant positive impact on the price premium of online medical consulting services;(3)Compared with the ordinary diseases,the price premium of online medical services for high-privacy diseases is higher.At the same time,high-privacy diseases will weaken the negative impact of resource supply on service price premiums.When the supply of medical resources increases,the price premium of online medical consultation services for high-privacy diseases has fallen less than that of the ordinary diseases.3.Taking the doctor-patient interaction process as the research object,based on the evolutionary game theory,study the evolution process of the doctor-patient interaction strategy and the doctor-patient relationship.This study explored the deep reasons that affect the doctor-patient relationship from three perspectives: the degree of information asymmetry between doctors and patients,the degree of trust and moral hazard,to understand the mechanism of the changes in the doctor-patient relationship.The study found that:(1)In the doctor-patient relationship,the choice of interaction strategies between doctors and patients will eventually stabilize in the state of {cooperation,cooperation} or {conflict,conflict}.The doctor-patient relationship can form a zero-sum game or a win-win situation.(2)Reducing the degree of information asymmetry between doctors and patients,increasing the level of trust in each other,and reducing the moral hazard coefficient,which can promote the development of the doctor-patient relationship in the direction of cooperation.Based on the above research conclusions,this study proposed that by developing Internet medical care,online and offline hybrid medical treatment can be brought to patients diversified channels to obtain healthcare information,reduce the degree of information asymmetry,and enhance mutual trust.The actions can alleviate the conflicts between doctors and patients,and promote the harmonious development of the doctor-patient relationship.4.Research on the construction of an equal and credible medical ecological environment.This study discussed how to design a medical blockchain system based on blockchain and Internet medical technology to build an equal and credible medical ecological environment and realize online and offline hybrid medical applications.This study constructed private chains that were used in hospitals and a consortium chain that was applied to various institutions such as hospitals.Then,the study combined the consortium chain and the private chains to design a medical blockchain double chain system ——MBDS system.The systems used a distributed storage method to store encrypted medical information to realize the safe and efficient sharing of data among participating nodes.By using MBDS system,Internet medical care is incorporated into the current medical care system to build an equal and credible medical ecological environment.Patients can achieve seamless medical treatment between online medical platforms and offline hospitals,to realize the application of online and offline hybrid medical care,and promote the establishment and implementation of a hierarchical medical system.Finally,the MBDS system and the mechanism can help to achieve the goal of alleviating doctor-patient conflicts and improving the doctor-patient relationship.This thesis used Internet medical care as the research background to study doctorpatient behavior and doctor-patient relationship under the new situation and put forward suggestions for improvement.The results of this thesis are helpful to understand the characteristics and changes of doctor-patient behavior and doctor-patient relationship under the background of Internet medical care.Through the combined application of blockchain and Internet medical technology,the study designed a medical blockchain dual-chain system and realized the implementation of hybrid medical applications in an equal and credible medical ecological environment,which they can contribute to the development of smart medical care.The thesis provides a solid theoretical basis and practical guidance for the solution and improvement of doctor-patient conflicts and doctor-patient relations in the current society,and has important theoretical and practical contributions. |
