1.PurposeEjaculation is the most important behavior in the reproduction process of mammals,which is influenced by many factors.Premature ejaculation(PE)is a common ejaculation dysfunction with a more complex pathogenesis.Currently,the clinical diagnostic criteria for PE are not uniform.Except for the Lifelong PE(LPE)and Acquired PE(APE),The classification of PE were also included the Variable PE(VPE)and Subjective PE(SPE).Temperament and personality in personality traits,as related factors of mental psychology,may be involved in the pathogenesis of PE.In order to further reveal the influence of temperament and personality on the onset of PE,we explored from two perspectives: 1)the correlation of temperament and personality in PE;2)the correlation between temperament and personality and chronic prostatitis(CP)symptoms in different PE subtypes.In addition,mating behavior and molecular genetics also provide new research ideas to reveal the pathogenesis of PE.Due to the reproductive properties of mating,the mating process between humans and rats has similarities in basic links,including sexual interaction before insertion,insertion,ejaculation and post-ejaculate reactions.According to the mating test screening model,combined with ejaculation distribution theory(EDT),male rats with different ejaculation behaviors were grouped in turn,the intercourse parameters of male rats with different ejaculation behaviors were compared and analyzed,and their transcriptome gene was sequenced to further explore and analyze the possible molecular genetic mechanisms.2.The Materials and Methods1)Correlation of temperament and personality in different PE subtypes.From December 2018 to December 2019,men were collected from the outpatient clinic of Andrology in the first affiliated hospital of Anhui Medical University.Men self-reported PE were divided into PE group.According to the classification of PE,PE were divided into LPE,APE,VPE and SPE.The degree of PE was assessed mainly using the PE assessment questionnaire(Index of PE,IPE)and the temperament and personality were assessed using the temperament and personality assessment questionnaire(Temperament and Character Inventory-Revised,TCI-R).Moreover,we designed a set of questions for survey assessment regarding men’s cognitive attitudes to PE.2)Correlation of temperament and personality and CP symptoms in different PE subtypes.The study was conducted from January 2019 to January 2021.The survey subjects included male patients who went to the andrology of the first affiliated hospital of Anhui medical university for self-reported PE and men without PE symptoms.Subtyping,temperament and personality of PE were assessed as in the previous study.In addition,the self-assessment questionnaire includes the questionnaires of Patient Reported Outcomes(PRO)and the National Institutes of Health Chronic Prostatitis Symptom Index(NIH-CPSI).3)Gene mining of the CNS transcriptome of male rats with different ejaculate behaviors.3.1)Mating test: Each male rat to be screened received six mating tests(once per week for 2 hours for 6 weeks).Mount latency(ML),mount frequency(MF),intromission latency(IL),intromission frequency(IF),ejaculation latency(EL),and ejaculation frequency(EF)were analysis.3.2)Group of male rats with different ejaculatory behaviors: Based on the theory of ejaculate continuum,draw the normal distribution curve of EF values in male rats according to the EF values measured in mating tests.According to the 10%classification principle,male rats with ejaculatory behavior were classified as relatively fast ejaculation,relatively normal ejaculation and relatively delayed ejaculation.3.3)Brain tissue acquisition and sequencing of male rats with different ejaculatory behaviors: The screened male rats were further grouped according to the distribution characteristics of EF values,and the brain tissue of male rats with different ejaculatory behaviors was removed,and high-throughput transcriptome sequencing by gene sequencing technology,and further verified according to the sequencing results.Using information biology techniques to predict downstream regulated target genes.3.Results1)Correlation of temperament and personality in different PE subtypes.A total of 602 men eventually completed the survey,and 350 were included in the PE group.The distribution of PE subtypes is as follows: LPE: 18.00%,APE: 45.43%,VPE:13.71% and SPE: 22.86%.The scores of the NS dimension were significantly lower in the self-reported PE group than in the non-PE group,while the HA dimension had the significantly highest scores.Among the PE subtypes,APE had the highest NS and ST scores,while VPE had the highest HA scores.The NS of VPE,ST of LPE and HA of SPE hade the lowest scores.In terms of IPE score,the total IPE score and related dimensions(sexual satisfaction,ejaculation control,and psychological emotion of anxiety)of SPE scores were significantly higher than those of other PE subtypes.Among the reasons for PE treatment,"mainly from myself" was VPE,"mainly from my sexual partner" was LPE,and "mainly from myself and sexual partner" was SPE."Reduction in ejaculatory control " is the most concerned condition of PE patients,and "considering the importance of sexual partner participation in PE treatment" is the most recognized way in the treatment of PE patients.Moreover,the NS and ST dimension scores were significantly correlated with the PE total score and the dimension scores.2)Correlation of temperament and personality and CP symptoms in different PE subtypes.A total of 844 men eventually completed the survey,of which 479 were included in the PE group.The distribution of PE subtypes is as follows: LPE: 16.70%,APE: 48.85%,VPE: 11.27% and SPE: 23.17%.Self-reported PE patients correlated with the personal anxiety dimension score in PRO,HA and positively correlated with PE severity,and ST with satisfaction with sexual intercourse.Furthermore,self-reported PE patients were significantly negatively correlated with Qo L dimension scores in NIH-CPSI,HA was significantly positively correlated with voiding symptoms,and ST with the impact of Qo L.Further analysis of different PE subtypes revealed that NS and HA of APE subtype were significantly correlated with total NIH-CPSI score,and ST of SPE subtype was significantly correlated with total NIH-CPSI score.3)Gene mining of the CNS transcriptome of male rats with different ejaculate behaviors.A total of 70 male rats were included in the study and grouped as follows: 11 relatively too fast ejaculation,13 relatively normal ejaculation,and 10 relatively delayed ejaculation.Whole transcriptome sequencing of brain tissues from different ejaculate-acting male rats revealed statistically significant differences in the differential expression of Lnc RNA BGIG in different ejaculate-acting male rats.This deferentially expressed Lnc RNA BGIG was the first finding.To further explore the regulatory process of Lnc RNA BGIG,we used information biology technology to predict that mi RNA-506 may be the target gene involved in the downstream regulation of Lnc BGIG,and speculated that Lnc BGIG may target the regulation of mi RNA-506 through STAT3 The signaling pathway induces SERT to change the concentration of 5-Hydroxytryptamine(5-HT)in the synaptic cleft to achieve the regulation of ejaculate function.mi RNA-506,STAT3 and 5-HT transport Protein will promote the expression of SERT,and then influence the occurrence of ejaculate behavior. |
