The health of students is the foundation of the health of the whole nation, and is crucial to the prosperity of the country and the nation. In recent years, the poor body posture of primary and secondary school students, particularly related to spinal scoliosis, has become a prominent public health issue in China, attracting the attention and concern of the Ministry of Education, the General Administration of Sports, the Health and Welfare Commission and other departments in the education, sports and medical fields.According to a survey,68.7% of teenagers in China have two or more poor body postures at the same time, and the total detection rate has reached 83.1%,while the number of primary and secondary school students with spinal scoliosis has been detected as high as 5 million, and is increasing at a rate of about300,000 per year.Early detection, prevention and intervention is the expert consensus on the prevention and control of poor posture and the development of spinal scoliosis. This study focuses on how the medical field and the sports field can work together to focus on the outstanding health problems of students, and to build a new model of school-based sports and medical integration services that is relevant, comprehensive, effective and applicable to improve the poor body posture of primary and secondary school students,so as to fundamentally solve this real problem and achieve a healthy body posture and a healthy school, thus contributing to the strategic decision of a healthy China.Based on this, this study uses structural functionalism in sociology and the PRECEDE-PROCEED in health education as theoretical foundations, and takes the school sports medicine integration service model for improving the poor body posture of primary and secondary school students as the research object. The study uses research methods such as literature review, expert consultation, field investigation, questionnaire survey, measurement, and experimental methods, and employs a comprehensive research approach that combines theory and practice,as well as qualitative and quantitative methods.Through the analysis of the real problem of poor posture among primary and secondary school students,a theoretical model for improving poor posture through a school-based sports and medicine integration service model is constructed. After practical testing of the model, feasible strategies for implementation are proposed.Based on this research approach, the following result and conclusions have been drawn:(1)The school-based sports and medicine integration service model for improving the poor physical posture of primary and secondary school students is aimed at improving the poor physical posture of primary and secondary school students, with the campus as the service field, and the knowledge and methods of physical education and medicine as the service means. The service is provided by five participating entities, including education administrative departments, sports service departments, medical service departments, primary and secondary schools at all levels, and families, through the integration of resources and collaborative cooperation,to carry out a collective term for a series of service actions such as health screening, health education, health management, and health intervention.(2)The problem of poor body posture of primary and secondary school students is comprehensively examined.Based on 7,508 questionnaires from 16 administrative regions in 7provinces and semi-structured interviews with 7 categories of people in China, the study found that the main problems of primary and middle school students’ body posture including: weak awareness and poor posture behavior of students;insufficient resource allocation and poor implementation of health education of schools;biased guidance on health concepts and laborious correction methods of parents;weak permeability of policy transmission and inadequate policy guidance of governments. And the poor integration of internal and external resources, inadequate government leadership, ineffective multi-party collaboration, and inconsistent values are the fundamental reasons for the neglect of the problem of poor body posture among primary and middle school students, and the difficulty in implementing health promotion measures.(3)Construct a school-based sports and medicine integration service model from theory.Taking structural functionalism as the theoretical analysis framework, taking five participants as the resource provider, and the education bureau, college physical education colleges, hospital related departments, middle schools and primary schools, parents and students as the executive subjects. An action system composed of organic system, personality system, social system and cultural system was established.Based on the functions of each system,8 constituent elements including human resources, organizational management, and collaborative participation,as well as 19 operational links, have been identified. The multilateral interaction relationship between the participating entities and the functions and constituent elements of each system has been clarified, and seven operational processes for the practical operation of the service model have been determined.(4)The campus model of physical medicine integration services is tested in practice.A four-month school-based action was carried out at YP school, and service action plans were designed based on the results of student health screenings and diagnoses of influencing factors;after 4 weeks of environmental optimization,6 weeks of health education and 8 weeks of "exercise + KT patching" health intervention for students, teachers and parents, the material, cultural, and policy environment on campus were significantly improved;the role of teachers and parents in guiding and supervising were significant increased;students’ health knowledge reserves, behavioral habits, and attitude and beliefs were significantly improved;various observation indicators of students’ poor body posture(upper crossed syndrome)were significantly improved, with the most prominent effect of functional training.(5)Propose the guarantee strategy for school-based sports and medicine integration service model.Providing a practical basis for the orderly operation of the service model by building a guarantee mechanism for resource supply,a government-led coordination mechanism,an effect-led supervision mechanism, and a value-judgement transformation mechanism.Besides, the realization paths of every participating subjects are clarified, namely, education management agencies should deepen policy support and increase financial support;sports service departments should give play to their platform advantages and input action resources;medical service departments should provide health consultation and make good treatment services;primary and secondary schools at all levels should enhance conceptual awareness and create content systems;family participating subjects should strengthen physical guidance and focus on home-school cooperation. This study takes the prominent public health problems of primary and secondary school students as a concern, and constructs the school-based sports and medicine integration service model innovatively, aiming to improve the poor body posture of primary and secondary school students.Through practical tests, the service model has been able to comprehensively address the problem of poor body posture among primary and secondary school students, and has been proven to be relevant and effective. At the same time,by establishing a promotion mechanism and clarifying the practical path, the applicability and operability of the service model are improved. This innovation not only provides a Chinese solution to promote healthy body posture, build healthy schools and contribute to a healthy China, but also has a high value for promotion and application.
