Objective:Based on the big data analysis of real medical records in the national shared medical record database,the TCM symptom clinical evaluation scale for HBP with anxiety was formulated;the clinical manifestations and TCM syndrome characteristics of HBP-associated anxiety population in Lingnan area were analyzed,and Huang Yanshou,a famous Chinese doctor in Guangdong Province,was excavated.To teach the rules of medication and treatment for HBP accompanied by anxiety,so as to guide clinical research;through clinical trials,observe the clinical efficacy of five-element music therapy in improving HBP accompanied by anxiety due to liver depression and spleen deficiency;from the perspective of "brain-gut axis" to explore liver depression and spleen deficiency Characteristics of intestinal flora in patients with HBP associated with anxiety and the effect and mechanism of five-element music therapy on intestinal flora,metabolites,and nerve conduction pathways in patients with HBP associated with liver depression and spleen deficiency with anxiety.Methods:Chapter 1 Data Mining Research Based on the analysis results of clinical symptoms of real medical records and big data,the clinical evaluation scale of TCM symptoms associated with HBP with anxiety was formulated,the clinical manifestations and TCM syndrome characteristics of HBP with anxiety patients in Lingnan area were analyzed,and the names of Guangdong Province were mined.Chinese medicine professor Huang Yanshou’s medication rule for treating HBP with anxiety.(1)Search by keywords such as "hypertension","dizziness","wind vertigo","headache"through the nationwide ancient medical record database,modern medical record database,shared medical record database,and famous doctor medical record database The medical records related to HBP were collected,and the collected medical records were searched for the second time using "depression","depression syndrome" and "anxiety" as the search terms,and were screened according to the inclusion and exclusion criteria of medical records.The collection categories included "gender,Information such as age,clinical manifestations,tongue quality,tongue coating,pulse condition,and the four diagnostic methods were included in the database for frequency and cluster analysis.(2)To collect and sort out the medical records of patients with HBP accompanied by anxiety that have been treated in the inheritance studio of Professor Huang Yanshou,a famous Chinese medicine practitioner in Guangdong Province in the past three years,establish the "Medical Record Database of HBP Associated Anxiety Patients in Lingnan Area",and extract the clinical symptoms in the medical records for four diagnosis Comprehensive,TCM syndromes,treatment methods,TCM,prescriptions and other data,frequency statistics,cluster analysis,correlation analysis,Multi-scale data information on clinical core symptoms,common TCM syndromes,TCM treatment methods,and prescription drugs backbone complex network analysis.Chapter 2 Literature research To explore the possible mechanism of five-element music therapy in the treatment of anxiety associated with HBP with liver depression and spleen deficiency.Computer retrieved databases such as Embase,Pubmed,CENTRAL,CNKI,Wanfang,VIP,etc.,the correlation of HBP with anxiety and the research status of liver stagnation and spleen deficiency syndrome in this disease,HBP with anxiety liver stagnation and spleen deficiency syndrome and intestinal flora-The research progress on the correlation of the brain-gut axis and the related literature on the research status of the five elements music therapy to regulate emotions and improve blood pressure were analyzed and discussed.Chapter 3 observes the clinical effect of five-element music therapy in improving HBP accompanied by anxiety due to liver depression and spleen deficiency through clinical trials.After screening by the inclusion and exclusion criteria,a total of 60 qualified subjects with HBP complicated by anxiety due to liver depression and spleen deficiency were included in the outpatient department of cardiovascular medicine of the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine and the inpatient department of the Qinzheng Hospital of the Affiliated Hospital of Guangzhou Medical University.For example,60 patients were divided into the control group(conventional antihypertensive drug treatment)and the five-element music group(conventional antihypertensive drug treatment combined with five-element music therapy)using the random number table method.Age,gender,BMI,hypertension stage,Self-Rating Anxiety Scale(SAS),Pittsburgh sleep quality index(PSQI),TCM symptom,morning blood pressure,ambulatory blood pressure detection related indicators(24h SBP,24h DBP,dSBP,dDBP,nSBP,nDBP).Chapter 4 Through the analysis of 16S rRNA high-throughput sequencing,to explore the characteristics of intestinal flora in patients with HBP with liver depression and spleen deficiency accompanied by anxiety and the changes of the five elements music therapy on the intestinal flora of patients with HBP with liver depression and spleen deficiency accompanied by anxiety.On the basis of the previous clinical research,17 people in the control group and 17 people in the Wuxing Music group were collected.At the same time,17 people in the non-HBP group were recruited to collect fecal samples,identify the samples by PCR amplification,and construct a PE library and Illumina sequencing.Dilution curve and OUT cluster analysis were performed on sample data using 16SrRNA high-throughput sequencing.The gut microbiota of the HBP with liver depression and spleen deficiency type patients who did not complete the preparation of serum samples due to objective reasons such as unwillingness to draw blood were excluded.Finally,34 patients with HBP accompanied by anxiety due to liver depression and spleen deficiency completed stool samples and serum samples before and after treatment,including 17 in the control group and 17 in the five-element music group.17 people,non-HBP group.Detection of short-chain fatty acids(SCFAs),dopamine(DA),norepinephrine(NE),gamma-aminobutyric acid(GABA),and serotonin(5-HT)in serum by enzyme-linked immunosorbent analyzed.Results:Chapter 1 The TCM syndromes of people with HBP complicated with anxiety in Lingnan area ranked first with "liver stagnation and spleen deficiency syndrome",and their clinical symptoms are red tongue,stringy pulse,white coating,dizziness,and insomnia.⑤ Information on syndrome differentiation and treatment:The frequency of TCM syndromes shows that "liver stagnation and spleen deficiency syndrome" ranks first.⑥ Analysis of the Multi-scale backbone network algorithm:The weights of the syndromes associated with the clinical manifestations of "dizziness" are as follows:liver stagnation and spleen deficiency syndrome,yin deficiency and yang hyperactivity syndrome,and phlegm-dampness syndrome.Chest tightness symptoms were also closely related to liver depression and spleen deficiency syndrome.In Huang Lao’s analysis of the prescriptions for this disease,①the frequency of use of traditional Chinese medicine treatment methods is from high to low:soothing the liver and strengthening the spleen,resolving phlegm and extinguishing wind,relieving depression and soothing the mind,nourishing liver and kidney,calming liver and eliminating wind,and clearing liver and diarrhea.Fire,activating blood circulation and removing blood stasis,moistening bowel and laxative.②The first three prescriptions are Sini San,Pinellia Baizhu Tianma Decoction,and Tianma Goutengyin.③The top ten traditional Chinese medicines in terms of frequency of use are Poria,Licorice,Chenpi,Tianma,Chuanxiong,Pueraria,Atractylodes,Pinellia,Alisma,and Bupleurum.The channels of traditional Chinese medicine are ranked the top two with "liver meridian and spleen meridian".④Cluster analysis of traditional Chinese its metabolites SCFAs and brain-gut interactions.Anxiety inhibits the central nervous system,further activates the HPA axis and sympathetic nerve,and changes the structure and composition of intestinal flora,while probiotics can also improve anxiety through reverse regulation.There is a molecular biological correlation between liver stagnation and spleen deficiency syndrome and the function of gut microbiota-brain-gut axis through neuro-endocrine and other systems.Wuxing music is derived under the guidance of the basic theory of traditional Chinese medicine.It may play a role in regulating intestinal flora through the "brain-gut axis" and improving HBP accompanied by anxiety due to liver stagnation and spleen deficiency.Chapter 3 Five-element music therapy has a definite clinical effect on improving HBP accompanied by anxiety due to liver stagnation and spleen deficiency.In this study,the gender ratio of the 60 patients included was 41.67%male and 58.33%female.After treatment,the SAS scores of the two groups were decreased compared with those before treatment(P<0.05).The overall score of the five-element music group decreased from mild anxiety 54.0(52.0,56.0)to possible anxiety 50.0(49.0,51.0).The corresponding level of the overall score of the group decreased from mild anxiety 55.0(53.0,57.3)to 53.5(52.0,56.0),still belonging to the mild anxiety level.Compared between groups,the five-element music group had a more significant effect on improving anxiety.After treatment,the PSQI scores of the two groups of patients were decreased compared with those before treatment(P<0.05).The difference between groups was compared between groups,and the five-element music group had a more obvious effect on improving sleep.The total score of TCM syndromes(TCM)in the two groups after treatment was significantly decreased compared with that before treatment(P<0.05).(P<0.05).Compared with before the course of treatment,the symptoms of dizziness,headache,irritability and insomnia in the two groups were significantly improved;compared with the control group,the five-element music group had more obvious therapeutic effects on symptoms of irritability,flushing and insomnia.Compared with the five elements music group,the morning SBP,morning DBP,24h SBP,24h DBP,dSBP,dDBP,nSBP and nDBP were all lower than those before treatment and the decreasing values were 15.30,5.20,12.00,3.97,9.17,1.67,4.53 and 2.50 mmHg,respectively.The reduction was both statistically and clinically significant.The mean values of morning SBP,morning DBP,24h SBP,dSBP and dDBP of ambulatory blood pressure in the control group were lower than those before treatment,and the decreasing values were 7.30,4.07,6.37,6.57 and 2.53 mmHg,respectively,and the decreasing values were both statistically and clinically significant.Compared with the control group,the differences in the mean values of morning SBP,24h SBP,24h DBP,dSBP and nSBP in the five-element music group were statistically significant and clinically significant.It is worth noting that the 24h DBP and nDBP in the control group decreased less than those before treatment,and there was no significant difference within the group.Compared with those before treatment,the decrease values in the group were only 4.07 and 0.97mmHg.There was no statistical difference in the difference of DBP and nDBP in the morning before and after treatment between the two groups.Considering that the decrease of DBP and nDBP may be mainly affected by antihypertensive drugs.The five-element music group had better therapeutic effects on morning SBP,24h SBP,24h DBP,dSBP and nSBP.It may be considered that after the five-element music therapy,the sleep quality was improved and the nighttime autonomic nerve sensitivity was reduced,thereby affecting the nighttime and morning blood pressure.Chapter 4 Through the analysis of 16S rRNA high-throughput sequencing,to explore the characteristics of intestinal flora in patients with HBP with liver depression and spleen deficiency accompanied by anxiety and the changes of the five elements music therapy on the intestinal flora of patients with HBP with liver depression and spleen deficiency accompanied by anxiety.(1)Characteristics of intestinal flora in HBP patients with liver depression and spleen deficiency accompanied by anxiety:Compared patients with depression and spleen deficiency type accompanied by anxiety were decreased.Compared with the non-HBP population,the species abundance of HBP patients with liver depression and spleen deficiency with anxiety was lower.The composition of the core microbial community at the phylum level of HBP patients with liver depression and spleen deficiency accompanied by anxiety is basically the same as that of the non-HBP population,mainly Firmicutes,Proteobacteria,Actinobacteria,Bacteroidetes and Verrucobacterium However,there were significant differences in the relative abundance of bacterial phyla between groups.HBP patients with liver depression and spleen deficiency accompanied by anxiety and non-HBP populations had the phylum Firmicutes,Proteobacteria,Actinobacteria,Bacteroidetes,Verrucobacterium Doors differ significantly.The sum of Firmicutes and Bacteroidetes in HBP patients with liver depression and spleen deficiency with anxiety was much lower than the non-HBP group.The Firmicutes/Bacteroidetes ratios(F/B values)of the two groups were 15.29 and 7.52,respectively,suggesting that the F/B values tended to increase in HBP patients with liver depression and spleen deficiency accompanied by anxiety.In terms of relative abundance at the genus level,the difference between the two groups was more obvious.There were significant differences in 137 species of bacteria including Escherichia coli-Shigella,Brautia,and Bifidobacterium between HBP patients with liver depression and spleen deficiency with anxiety and non-HBP people.At the same time,the relative abundance of beneficial flora and symbiotic bacteria in HBP patients with liver depression and spleen deficiency accompanied by anxiety was significantly lower than that in non-HBP population,such as Bacteroides,Faecalibacterium and so on.The predominant phyla of intestinal flora in HBP patients with liver depression and spleen deficiency with anxiety were the same as those in the non-HBP population,but there were significant differences in the relative abundance of the dominant phyla.The predominant genus of intestinal flora in HBP patients with liver depression and spleen deficiency with anxiety and non-HBP population were inconsistent,and there were 137 species of bacteria with significant differences in relative abundance.Using LEfSe analysis,24 potential biomarkers were discovered in HBP patients with anxiety due to liver depression and spleen deficiency.(2)Antihypertensive treatment can promote the recovery of intestinal flora diversity in patients with HBP complicated by liver depression and spleen deficiency with anxiety The results of this study showed that there were significant changes in the diversity of gut microbiota in patients with HBP.The results of α-diversity analysis reflecting the diversity of a single sample showed that the ace and chao indices of intestinal flora in HBP patients were changes in the non-HBP group.The β-diversity analysis,which reflects the changes in diversity between groups,showed that the distribution of the PCoA analysis plan of each sample of HBP patients was significantly different from that of the non-HBP group,with clear boundaries and a long distance from each other.This suggests that the gut microbiota of HBP patients is significantly different from that of the non-HBP group.Further research showed that both the five-element music group and the control group could significantly increase the alpha diversity of intestinal flora and up-regulate the species abundance of intestinal flora,indicating that in this study,the treatment of HBP patients with anxiety due to liver depression and spleen deficiency was not effective.The therapeutic effect may be achieved by improving the diversity of gut microbiota,that is,modulating the species diversity and abundance of each species in the gut microbiota.Judging from the recovery of the disease,the improvement of related scales in the five-element music group was better than that in the control group,but there was no significant difference in the a diversity and species abundance of intestinal flora between the five-element music group after treatment and the control group.Therefore,we believe that the five elements music therapy may play its role in improving symptoms by regulating the specific intestinal flora of HBP patients with anxiety due to liver stagnation and spleen deficiency.Further comparison of the F/B value difference between the five-element music group before and after treatment and the control group before and after treatment showed that there was no significant difference between the two,but the F/B value of the five-element music group before and after treatment had a more obvious downward trend,indicating that blood pressure was mainly caused by blood pressure.Drugs play a role in regulating F/B value,and five-element music therapy may play a role in lowering blood pressure and improving intestinal dysbiosis.(3)Five-element music therapy can significantly down-regulate the levels of Blautia and KlebsiellaThis study shows that the level of bifidobacteria in the five-element music group after treatment was higher than that before treatment,and the difference of the increase was not significantly different from that in the control group before and after treatment,but the upward trend in the five-element music group was more Clearly,suggesting that the five elements of music therapy may play a role in the regulation of Bifidobacterium.In addition,we also observed the regulatory effect of five-element music therapy on Blautia and Klebsiella at the genus level.In this study,the numbers of Blautiah,Escherichia-Shigella and Klebsiella in the five-element music group and the control group were all lower than those before treatment,and the decreasing trend of Blautiah and Klebsiella in the five-element music group was more significant.Among them,the decreasing trend of Escherichia-Shigella in the control group was more significant.Chapter 5 uses the main metabolite SCFAs of intestinal flora as the medium,based on the brain-gut axis,to explore the potential mechanism of five-element music therapy in the treatment of anxiety associated with liver depression and spleen deficiency type HBP.(1)Five-element music therapy can inhibit the expression of SCFAsCompared with the non-HBP population(n=17),the HBP patients with liver depression and spleen deficiency complicated with anxiety(n=34)had higher levels of serum SCFAs(P<0.05).Compared with the difference before and after treatment in the control group,the difference of SCFAs before and after treatment in the five elements music group was statistically significant(P<0.05).(2)Five-element music therapy can reduce the release of DA and NE,and up-regulate the expressions of GABA and 5-HT.Compared with non-HBP population(n=17),serum DA and NE levels were up-regulated(P<0.05),while 5-HT and GABA levels were down-regulated(P<0.05)in HBP patients with liver depression and spleen deficiency complicated with anxiety(n=34).);the levels of DA and NE in the control group and the Wuxing Music group after treatment were down-regulated compared with those before treatment(P<0.05),while the levels of 5-HT and GABA were increased compared with those before treatment(P<0.05);Compared with the values of DA,NE,5-HT and GABA before and after treatment in the Wuxing Music group,the difference was statistically significant(P<0.05).(3)Five-element music therapy can inhibit HPA axis pathway,improve anxiety and lower blood pressure.Compared with the non-HBP population(n=17),the levels of serum CRF,ACTH,CAs and CORT were all up-regulated in HBP patients with liver depression and spleen deficiency with anxiety(n=34)(P<0.05).Compared with the control group,the differences in CRF,ACTH,CAs and CORT before and after treatment in the Wuxing music group were significantly different(P<0.05).Conclusion:1.From the data mining and analysis results of the national medical records of HBP and anxiety combined with ancient and modern shared famous medical records,it can be seen that the common clinical symptoms of HBP and anxiety co-occurring include emotional symptoms(such as upset,irritability)and somatic symptoms(covering nervous system symptoms——"dizziness,dizziness,insomnia,headache",digestive system symptoms"anorexia,dry stool",respiratory and circulatory system-"chest tightness" and other system symptoms),manifested as physical and mental symptoms.2.The data mining analysis results of the "Medical Records Database of HBP Associated Anxiety Patients in the Studio of Professor Huang Yanshou,a Famous Traditional Chinese Medicine Doctor in Guangdong Province" showed that the liver and spleen were the main disease locations of HBP associated anxiety patients in Lingnan,and the clinical manifestations were dizziness,limb numbness,and palpitations.,chest tightness and other symptoms of Qi depression combined with constipation,anorexia,abdominal distension,belching and other symptoms of spleen deficiency.The most common TCM syndromes are"liver stagnation and spleen deficiency syndrome".In addition to the pathogenesis of liver qi,the clinical pathogenesis is often accompanied by spleen deficiency and phlegm disturbance.Professor Huang Yanshou,a famous traditional Chinese medicine practitioner in Guangdong Province,used soothing liver and spleen as the core therapy in the clinical treatment of HBP patients with anxiety.3.There is a molecular biological correlation between liver stagnation and spleen deficiency syndrome and the function of gut microbiota-brain-gut axis through neuro-endocrine and other systems.Gut microbiota can affect blood pressure and anxiety by regulating the nervous system through the brain-gut axis.Wuxing music can also improve blood pressure and anxiety by regulating the nervous system.Based on the two-way regulation mechanism of the brain-gut axis,Wuxing music may play a role in regulating intestinal flora through the "brain-gut axis" and improving HBP accompanied by anxiety due to liver depression and spleen deficiency..4.For patients with HBP associated with anxiety due to liver depression and spleen deficiency,who have stably taken individualized western medicines for antihypertensive drugs,and whose blood pressure is still at the upper and lower normal high values,the five-element music therapy can further reduce the levels of
