Objective:1.Based on literature data retrieval,to analyze the research hotspots of traditional Chinese medicine(TCM)related to primary osteoporosis in China, explore the treatment principles and methods of TCM for primary osteoporosis, analyze the evolution law,and predict the research direction.2.Based on PEST-SWOT,establish a Chinese TCM research model related to primary osteoporosis, analyze the advantages and disadvantages of traditional Chinese medicine research, and predict the research needs from macro, meso and micro three dimensions.Methods:1.According to the keywords of "research hotspot","evolution law","osteoporosis" and"TCM",the literature on TCM research hotspot and evolution law related to primary osteoporosis was retrieved from CNKI,Wanfang and VIP databases, and the research literature was included according to the inclusion and exclusion criteria. Then, the research status of TCM related to primary osteoporosis in China is reviewed, summarized and summarized.2.In Wanfang,CNKI and VIP databases,"TCM" and "osteoporosis" were used as key words, and the title or key words with those were searched for TCM research related to primary osteoporosis.GCLUTO1.0 was used for bidirectional cluster analysis.UCINET6.0 was used to calculate the centrality and density of each cluster.3.The search term "TCM+health services" was used to search laws, regulations and normative documents on the website of Chinese Legal Resource Library,Peking University Law Database, the China national government portal, the official website of the National Health Commission and the official website of the State Administration of TCM.Then,to establish a SWOT-PEST analysis model of TCM related to primary osteoporosis in China, analyze the external environment from the perspective of politics, economy, society and technology, and conduct qualitative assessment of macro and micro environment in general.Results:1.According to the retrieval strategy,21 articles,221 articles and 6324 articles were respectively hit,and 7 articles,115 articles and 2679 articles were left after literature cleaning. The results showed that the focus of overseas research was on the development of molecular targeted drugs and the development of bone quality diagnosis technology, while the focus of domestic research in the future may be to strengthen the integration of traditional Chinese and Western medicine, especially the combination of epidemiology and basic medicine.2.A total of 6324 relevant Chinese literatures were retrieved from Wanfang,CNKI and VIP databases, and 2679 valid literatures were left after cleaning. There were 3457 keywords in the remaining literature, which appeared 12388 times, with an average word frequency of 3.58 times and an average of 4.62 keywords in each paper. There were 504 TCM keywords in the literature, which were classified into 6 categories:Tonifying kidney, promoting blood circulation and removing blood stasis, strengthening spleen, softening liver, nourishing blood and invigorating qi.Overall, tonifying kidney was the highest with 144.27,followed by promoting blood circulation and removing blood stasis and invigorating spleen with 14.79 and 12.28 heat values, respectively. The top 20 high-frequency keywords were clustered into 5 clusters, and the clustering effect was the best. The results showed that the treatment of syndrome differentiation, etiology and pathogenesis, kidney tonification therapy, osteoporosis, elderly, pain, treatment methods,TCM therapy, vertebral compression fracture and review,a total of 10 keywords, located in the fourth group.3.In economy,China’s total health expenditure reached 7217.5 billion yuan in 2020,an increase of 12.47 times compared with 2002;And the per capita health expenditure reached 5,1 12.34 yuan in 2020,an increase of 1 1.34 times compared with 2002.Socially,the number of people aged 65 and above increased from 93.77 million in 2002 to 20.56 million in 2021.It is estimated that during the 14th Five-Year Plan period, the total number of people aged 60 and above will exceed 300 million, accounting for more than 20 percent. In terms of technology, the number of TCM hospitals in 2020 was 4426,an increase of 1.78 times compared with 2002;The number of medical visits in TCM hospitals was 518 million in 2020,an increase of 1.88 times compared with 2008.The actual total number of bed days and the average number of beds in TCM hospitals increased from 150,213,113 days and 411,542.78 days in 2010 to 34,279,219 days and 936,591 beds in 2020,both increasing by 2.28 times.Conclusion:1.At present, there are few researches on the research hotspot of osteoporosis and its evolution law in China, especially the lack of quantitative and data-based research.2.TCM in the treatment of osteoporosis could play the advantages of more targets and fewer side effects;Aging, tonifying kidney, activating blood circulation and removing blood stasis are the research hotspots and development trends of TCM research related to primary osteoporosis in the future. Most of the previous studies took bone mineral density as the research index and rats as the animal model. The connection between basic research and clinical research is not closely.3.On the macro level,it is recommended to carry out research on policy analysis, disease burden, public health and social perception of prevention and treatment of primary osteoporosis with TCM.It is suggested to carry out empirical data mining research on prevention and treatment of primary osteoporosis with TCM.Microscopically,it is suggested to follow up the latest progress of basic medicine, expand animal models, and strengthen the interaction between basic medical research and clinical application research.
