The Study Of TCM Theory Building And Application Of "Heat Stagnation" State Based On TCM State Science

ObjectiveThis research takes the "heat stagnation" in traditional Chinese medicine(TCM)as the study object,first create the new theoretical concept of "heat stagnation" state by examining the basic concepts of "heat" and "stagnation" as well as the relationship between each other,and in the angle of TCM original innovating thinking from systematic science and state medicine,aims to treat and separate the different diseases which with the "knot" characters by the concept of "state",so as to provide the treatment idea to the diseases of inflammation,cancer and epidemic diseases.Then provide theoretical guidance for the rational application of traditional Chinese patent medicines using,exerting the "heat stagnation"state theory in TCM theory,method,practical using and other aspects.MethodsThis research belongs to exploratory research on TCM theoretical study which with four methods,that is,philological research,literature research,speculative research and data mining knowledge discovery,then exploring and constructing the theory in a scientific interpretation and analytical explanatory manner.Part One:By the philological research method of seeking meaning of the character,this part examines the shape and meaning of "re"(heat)and "jie"(stagnation)in the word books and traditional literature,then explore the etiological and pathogenesis relationship between each other,then build the diseases connection of heat and stagnation by the reasoning and analysis of the conceptual categories.Part Two:By the literature research method,in the keyword of "heat stagnation",this part aims to ancient electronic literature materials of Chinese Medical Code and CNKI,by extracting and systematically organizing the connotations and concepts of the keyword "heat stagnation",as well as retrieving the content of modern literature,systematically organize relevant diseases and theories.Part Three:By the speculative research method,based on the discussing of principle and meaning of systemic science and state medicine as the foundation,in the TCM original thinking,this part create the "heat stagnation" state theory.In the contribution process,multidisciplinary was taken to elucidate the theory,speculative research method was used to contribute and analysis to the theory.Meanwhile,by integrating the previous research results of "TCM state studies"and "Five kinds of Differentiation methods of TCM" in our team,based on the principle of"heat stagnation",occurrence mechanism,pathogenesis characteristics,disease category and key point of differentiation and treatment,of"heat stagnation" was under the systematic analysis,then build the logical relationships between different elements.It hopes to form a conceptual category system and sorting out scientific laws within the theoretical category,to guide the practical application of the theory.Part Four:By the data mining and knowledge discovery methods,bibliometrics was used to analysis the current published literature of Pien tze huang,from the aspect of hot spots,disease characteristics,relevant institutions,authors and other information,to discuss the relationship of Pien tze huang in clinical efficacy,application,scientific research with "heat stagnation".In according to the efficacy of clearing away heat,cooling blood and removing the stasis,removing the swelling and reliving the pain of Pien tze huang,it can be reasoned that the treatment scope of Pien tze huang belongs to the disease of "heat stagnation" state.Further more," heat stagnation" theory could be used to explain the mechanism of Pien tze huang in disease prevention and treatment and guide the Pien tze huang follow the rules of treat same disease with different methods and treat different diseases with same method,and based on this theory,it can also provide theoretical foundation to the Pien tze huang’s modern research and second development.ResultsLiterature research section:Firstly,by exploring the original meaning of"heat" "and "stagnation",it is believed that there is an internal connection of etiology and pathogenesis between each other,that is,stagnation induced by heat,heat induced by stagnation and inter induced between each other.From the perspective of location,"heat stagnation" diseases existed in multiple parts of the human body,and have specific tongue and pulse characteristics record.Meanwhile,there are specific Chinese material medica existed about the "heat stagnation" with a wide range of drug categories.The main effects among them are clearing heat,purging fire,cooling blood,and relieving exterior syndrome.Under the multiple aspects influence,the organization of the "heat stagnation" diseases were separate of "heat" "stasis" "blocking" "phlegm" "dampness" and "toxicity".What’s more,in modern literature research,the category of heat stagnation are wide and many scholars had explored some new theories which related to this key word.Theory construction part:From the interdisciplinary perspective,systematic theory and holism theory in TCM complement each other,on the systematic and holism pursuing level,TCM state medicine was generated to exert the maximal function of different medicine.In modern science,"heat stagnation" state could be explained by the change of heat entropy which is characterized by periodical,composited,multifactorial and systematical changes,and it takes the qi,blood and body fluid as carrier to developed into different kinds of"heat stagnation" state.For the scope of the "heat stagnation" state,it includes the inflammation,cancer and epidemic diseases which belongs to heat syndrome in TCM.In the identification process,TCM "Five Differentiation"method could be used to take the comprehensive understanding to the disease by the systematic recognition of the human beings itself and the whole development process of the disease pathogenesis.The general treatment principles of the "heat stagnation" state are clearing away heat,resolving blood stasis,penetrating pathogenic factors and tonifying qi.It can be specifically divided into follows:clearing the heat in blood vessels so as to intervene from the bottom;dispelling blood stasis to detoxifying and resolving stasis;state-target adjustment by the multiple methods used and tonifying the healthy qi to improve the weakness condition,etc.So as to remove the "heat stagnation" state by adjusting internal micro environment.Regarding the treatment to the diseases in the "heat stagnation" state,it emphasizes the importance of using the "heat stagnation" state as a guideline to guide the drugs application in the principle of treating different diseases with same method,prevent the misuse of the medicine and misdiagnosis in clinical as well as the disease prevention by the body state understanding.Clinical application part:Based on the research of published literature on Pien tze huang,it indicts that Pien tze huang is not a "panacea"with all purpose function.The bad hype in the current market may lead to chaos in the use of traditional Chinese patent medicines and simple preparations.As a representative of the national treasure and famous medicine,Pien tze huang is the name card of the international image of traditional Chinese medicine.Research has found that it can be used to treat diseases with heat and stagnation syndrome or in the period with heat symptom,therefore,it should be confirmed that the efficacy range of Pien tze huang is in the "heat stagnation" among different diseases,to the rational using,it should identify the cold or heat in clinical practice instead of the disease name.Meanwhile,it has found that the treatment scope of the diseases in modern research are more widely in the drug package insert,so it could have a secondary development.For the efficacy of the Pien tze huang,it has found that it not the Pien tze huang’s only effects in the treatment,in most cases,the combination of other medicine or method are used together to promote the whole function,therefore,it can be confirmed that the mechanism action of the Pien tze huang is to adjust the whole state of the body,which have a same function in“heat stagnation”theory.In addition,Pien Tze Huang has a special effects on liver diseases.In TCM,it believes that the liver is the thief of all diseases.Under the background of a specific body constitution group,the prevention and adjustment of the liver diseases is conducive to giving play to the advantages of disease prevention.This can be seen as the principle of Pien Tze Huang in treating diseases before they become ill.At the same time,there are some loopholes and shortcomings in modern research of Pien Tze Huang,such as paying too much attention to microscopic parameters and ignoring the importance of TCM theory in modern research of Chinese medicine.Therefore,The establishment of the "heat stagnation" state theory is conducive to providing theoretical support for modern scientific research of drugs in TCM,and also has a certain guiding significance for regulating the use of other similar TCM patent medicines.ConclusionsThis study combines traditional Chinese medicine state theory with TCM "heat stagnation" by clarifying the relevant concepts of "heat stagnation",and constructs a TCM "heat stagnation" state theory based on TCM state theory from the perspective of original thinking in TCM.Regarding the definition of the theoretical system of the"heat stagnation" state,it is believed that this state can represent the entire state of the human body or a reflection of a certain stage in the disease,and this state is often in the process of the serious and dangerous stage in the disease occurrence and development,which the character is belonging to positive and solid,and the pathological elements of the "heat stagnation" state are multiple and complex.Therefore,observation and treatment should be conducted from a systematic scientific perspective in a multiple aspects.The "heat stagnation" state are commonly seen in the internal medicine which difficult to treat.Currently,the treatment plans in modern medicine are not perfect,and it is urgent to select specific effective prescriptions in clinical practice.From the perspective of TCM,by exploring the laws of the formation in the "heat stagnation"state,it is conducive to eliminating the growth of pathogenic factors and controlling the development of the disease as a whole by avoiding it early,treating both the obvious symptoms and the original syndrome,then restoring the body internal environment to the cold and heat balance.The proposal of the "heat stagnation" state theory in TCM is an innovation to TCM theory,with the Pien Tze Huang’s efficacy as the theoretical application object.Under the guidance of the "heat stagnation"state theory,it is emphasized that the clinical application of the Pien Tze Huang should be distinguished strictly in the cold,heat,deficiency and excess attributes of disease symptoms which cannot adhere to the Western medicine diagnosis and treatment thinking.Taking the concept of "heat stagnation" as the outline,from the concept original tracing to the theoretical analysis,until the“heat stagnation”state theoretical construction and innovation,it can form a main theoretical guidance based on traditional Chinese medicine theory,then derive various "fruits",which is greatly beneficial for the enriching of the development of traditional Chinese medicine theory.Furthermore,it can be extended as a theoretical basis for scientific research and design,then providing guidance for future research on TCM health management and disease prevention before it onset，It can also establish a theoretical benchmark for the rational use and modern research for the drugs with clearing heat,detoxifying and dispersing nodules function.
