| Objective:Choose cases with Parkinson’s disease (PD), collect their digital and quantitative four diagnostic features by the TCM Four Diagnosis Auxiliary Apparatus, to develop a digital syndrome differentiation scheme of PD combined with the clinical doctor’s subjective diagnosis. Search relevant acupuncture classics and modern clinical literature both at home and abroad, and establish a database for acupuncture intervention of PD and a quantitative identification system of PD and tremor syndrome in TCM Four Diagnosis Auxiliary Apparatus. To develop acupuncture intervention strategy for PD based on its digital four diagnostic features and the database. It may provide methodological support for standardizing clinical studies and management of diseases systematically. Developing such a digital system provides research mentality for establishing digital doctor workstation and mobile medical service of acupuncture.Methods:36 patients with PD were selected for this study. The digital and quantitative features of their tongue, pulse, sound and inquiring were collected by the BD-SZ TCM Four Diagnosis Auxiliary Apparatus. A search was conducted using CNKI, VIP, CBM, PubMed, Springer, SCI and Encyclopaedia of Traditional Chinese Medicine with main key words of acupuncture and Parkinson’s disease" or " tremor" et al(1949-2014). A databased about acupuncture intervention for PD was established via Microsoft Access2007, which provides basis for analysis the intervention methods of PD and extends the databses of BD-SZ TCM Four Diagnosis Auxiliary Apparatus as well. The relevance rule method in Weka was mainly adopted to analyze the acupoints selection characteristics and points combination rules in the treatment of PD with acupuncture.Results:1 In 36 patients with PD, There were 17 cases with slight red tongue,21 cases with thin coating,27 cases with white coating,13 cases with dry coating,11 cases with watery coating. Deep pulse was identified in 22 cases and wiry pulse in 21 cases.11 cases were identified as wood type constitution,9 as earth type. The primary symptoms were slow movement (n=32), tremor of limbs (n=30). The main syndromes of TCM were up-stirring of liver wind (n=15), qi and blood deficiency (n=7) and liver and kidney deficiency (n=7).2 On the basis of the collection of 106 modern literatures (94 articles in Chinese and 12 in English) and 182 articles of the old literatures about the treatment of PD with acupuncture, the database of acupuncture intervention for Parkinson’s disease was established.3 Totally 106 modern clinical literatures showed that 146 acupoints were used for PD for 751 times. The most frequently used acupoints were LR3, DU20, LI4, GB20, chorea-trembling control area, GB34.48.9% of the selected acupoints were on the four limbs and 27.4% were on the head and neck.15.4% of the selected acupoints were distributed on the DU meridian. The total frequency of usage of the points on yang meridians accounted for 41.4% and Five-shu acupoints of the spccific acupoints for 30.2%. The most commonly used point combination was GB20, LI4 and LR3 based on data mining.4 Totally 182 acupuncture classics showed that 89 acupoints were used for PD for 344 times. The most frequently used acupoints were LI11, LU5, GB34, TE3, LR3 and LI4. The acupoints of the Bladder meridians were the first option. 90.6% of the selected acupoints were on the four limbs and 27.4% were on the head and neck.15.4% of the selected acupoints were distributed in the BL meridian. The total frequency of usage of the points on yang meridians accounted for 68.4% and Five-shu acupoints of the spccific acupoints for 52.3%. The most commonly used point combination was SJ5, TE3 and LI11 based on data mining. Conclusion:The digital and quantitative four diagnostic features of PD and the database about acupuncture intervention for PD can be integrated in TCM Four Diagnosis Auxiliary Apparatus. Thus, the clinical features of PD patients in various stages, acupuncture intervention and its therapeutic effects can be recorded dynamically through the digital and quantitative diagnostic techniques in TCM, which may provide methodological support for standardizing clinical studies and management of PD systematically with acupuncture, so as to guide the clinical practice of acupuncture. Developing such a digital system provides research mentality for establishing digital doctor workstation and mobile medical service of acupuncture. |
