| Objective:To observe the clinical effect of Achyranthes bidentata alcohol extract ion, verify thephysical penetration in the treatment of knee osteoarthritis, and explore mechanisms ofAchyranthes bidentata alcohol extract, provide theoretical and experimental basis forclinical use of Achyranthes bidentata alcohol extract.Method:A randomized controlled trial,80 patients with knee osteoarthritis were randomly divided into two groups for clinical trials, the treatment group and the control group of each 40 people, were treated with ethanol extract of Achyranthes bidentata Huoxue Zhitongtincture, intermediate frequency penet-ration therapy. Treatment once daily, for 1 weeksfor a course of treatment, continuous treatment of 2 courses, with high frequency color Doppler ultra-sound to observe and measure the record in the two groups before treatment, after 2 weeks of treatment and the end of treatment after January knee effu-sion, synovial membrane thickness and the thickness of articular cartila-gethickness, parallel VAS, WOMAC score, statistical analysis of clinical curative effect in order to evaluate the Achyranthes bidentata alcohol ext-ract on osteoarthritis of knee joint.Results:Achyranthes bidentata alcohol extract, Huoxuezhitong tincture could reduce the patients with knee synovial membrane thickness, reduce joint effusion volume, decrease VAS and WOMAC integral, after 2 weeks of treatment of Achyranthes bidentata alcohol extract in reducing joint effusion, synovial thickness joint is better than Huoxuezhitong tincture, statistically signi-ficant differences (t=3.469, P=0.001 and t=2.529, P=0.014; in articular cartilage thickness), VAS score, WOMAC scores, no significant difference (t=0.630, P=0.531; t=1.638, P=0.106; t=1.015, P=0.314), the two groups bef-ore and after treatment were compared, in addition to the articular carti-lage thickness (treatment group:t=1.284, P=0.207; control group:t=0.661, P=0.513), joint effusion, synovial membrane thickness, VAS score, WOMAC integral comparison difference has statistics significance (treatment group: t=11.397, P=0.000; t=5.585, P=0.000; t=16.281, P=0.000; t=6.158, P=0.000; control group:t=9.808, P=0.000; t=3.279, P=0.002;t=17.913, P=0.000; t= 3.343, P=0.002); Follow up in January after the end of treatment, the Achyranthes bidentata alcohol extract in articular cartilage thickness increased, reduced to a WOMAC integral are better than those of Huoxuezhitong tincture, the difference was statistically significant (t=2.952, P=0.004; t=4.824, P=0.000), there was no statistical significance comparingjoint effusion, synovial membrane thickness, VAS score between the two side surf aces (t=0.832, P=0.409;t=1.156, P=0.251; t=1.126, P=0.265), the two groups before and after treatment were compared to control group, in addition to the articular cartilagethickness (t=1.205, P=0.236), joint effusion, synovial membrane thickness, VAS score, WOMAC integral comparison difference has statistics significance (treatment group:t=48.536, P=0.000; t=14.366, P=0.000; t=33.615, P=0.000; t=18.540, P=0.000;control group:t=55.824, P=0.000; t=9.201, P=0.000; t=28.403, P=0.000; t=10.446, P=0.000);The 2 groups after treatment in the control rate comparison, Achyranthes bidentata-alcohol extracts (84.2%,97.4%) than the control group (62.2%,83.8%), with significant difference (2=4.660, P=0.031; X 2=4.088, P=0.043), the clini-cal comprehensive therapeutic effect of 2 methods in the treatment of knee osteoarthritis have moredifference, and the difference was statistically significant (Z=0.931, P=0.032, Z=1.371, P=0.018).Conclusion:Achyranthes bidentata alcohol extract, Huoxuezhitong tincture can significantly reduce the effusion of knee joint cavity, reducing synovial thickness, reduced joint VAS score and WOMAC integral, in the observation of the curative effect of short term of Achyranthes bidentata alcohol extract in alleviating joint effusion, synovial membrane thickness, amount of joints are better than those of Huoxuezhitong tincture, in the longperiod of follow-up in the ethanol extract of Achyranthes bidentata could obviously increa-se the thickness of articular cartilage, in summary, Achyranthes bidentata alcohol extract is a type of medicine preparation for external use is worthy of clinical applicationis safe and effective for treating liver and kidney deficiency type of knee osteoarthritis. |
