Study On The Correlation And Evaluation Of Allergic Rhinitis With Physical Fitness And Its Clinical Research

Objective:To observe the main point, main studies, literature quality, of the research literature related constitution on allergic rhinitis, based on the past 12 years (2003-2014) traditional Chinese medicine literature of allergic rhinitis. In the light of research of diseases related constitution, and promote the development of constitutional theory. To analyse and evaluate the published clinical research literature in allergic rhinitis to find out the number, quality, safety and clinical curative effect, to provide evidence of evidence-based medicine.Methods:To retrieve the allergic rhinitis of Traditional Chinese or Chinese and Western literature originated from CNKI(China knowledge resourcse database(Grid2008edition)), Wanfang Data, VIP(2003.1.1 2014.12.31) by computer retrieval and manual retrieval. Literature retrieval method is title combined with full-text, Reading the title for early screening, eliminate repetitive literature, animal experiment, the therapy of TCM literature, medicine preparation literature. Reading the full text to classify and summarize the literature according to the constitution related AR, research methods, treatment methods, making EXCEL to record classification for fine items such as different treatment, prescriptions, acupuncture point, finally summarizes the results of the analysis use quantitative analysis, the CONSORT statement and the Meta systematic evaluation.Results:1. The constitution related allergic rhinitis of literature analysis The research literature of AR of constitutional theory is not much, the starting point in theoretical research is different, but all realize that the root cause of AR is inner constitution factors, namely, the Tebing constitution or allergies. Epidemiological studies results are more objective, adopt the professor Wangqi’s the constitution of TCM scale as the basis for determining constitutions and the conclusion is consistent. Most patients with AR are mixed constitution, such as constitution of qi asthenia, constitution of yang asthenia, constitution of yin asthenia.The proportion of constitution of qi stagnation is low, but it increase proportionally with the extension of the course. Now the research in lab haven’t approve the difference of immune level in different constitutions,it ask for further verified.2. Quantitative AnalysisThe published literature of traditional Chinese medicine or Chinese and western medicine of AR showed a trend of increase year by year. Especially in recent six years (2009-2014) is a rapid development period, clinical research institute accounts for a larger proportion. And RCT holds an absolute advantage in the form. The traditional Chinese medicine as the main treatment means is still used in Chinese medicine treatment. And the application of traditional Chinese medicine drugs in frequency of the former eight prescriptions were Yupinfengsan, Cangerzisan, XinQin particles, Buzhongyiqi decoct ion, Xiaoqinglong decoct ion, Wenfeizhiliudan, Shenqiwan, Guizhi decoction. The main external treatment of traditional Chinese medicine is sticking therapy, and medication have consensus, Tianjiufang from ZhangshiYitong as the main component. Medicine limited in nose are scattered,the main are unblocks nostril, such as Cangerzi, Xinyi, Baizhi.The incidence of AR in children is well matched in adult,but most research didn’t distinguish in age.3. Evaluate the quality of RCT literature of AR based on the CONSORT statement There are 150 literature RCT used traditional Chinese medicine as intervention bringed into the study. In test design,only 2 pieces estimate the sample size,5 pieces described test design,8 pieces mentioned the methods of bring random allocation sequence,10 pieces point the allocation concealment, but only 2 pieces elaborate the methods of allocation concealment,and 3 pieces report blinded manner. In selection and intervenetion of subjects,most literature didn’t describe the specific time recruitment of patients into the group, only 26 pieces provide the approximate date,18 pieces explicate the inclusion and exclusion criteria,majority literature didn’t describe interventions detailed.In outcome,34 pieces described the baseline and clinical features of the subjects, and make statistics comparisons, most author think highly of definition of outcome, but only 12 pieces record the number and reason of the dropped cases,39 pieces provide the statistical methods,37 pieces observed any adverse reactions of the subjects in research,only 13 pieces point out the limitation and the possible bias in the test.4. Systematic evaluation RCT of treatment of children AR using Yupinfengsan There are 13 RCT literature and 1177 AR children bringed into the study. The methodological quality are generally low, and there are highly risk of bias. The two study showed YupingfengJiawei is better than the matched group used western medicine in healing AR, themeta analyse for other 2 study showed YupingfengJiawei is better than the matched group used western medicine in improving significant symptom of nose and signs score of AR children, the meta analyse for other 2 study showed YupingfengJiawei combined with western medicine is better than the matched group used western medicine in improving significant symptom of nose and signs score of AR children, the meta analyse for other 3 study showed YupingfengJiawei combined with western medicine is better than the matched group used western medicine in controlling recurred after 1 year of stopping treatment to AR children,2 study showed YupingfengJiawei is better than the matched group used western medicine in controlling recurred after 2 months of stopping treatment to AR children,another study showed YupingfengJiawei is better than the matched group used western medicine in controlling recurred after 3 months of stopping treatment to AR children.Most study have no reportion for adverse reactions,so we can’t yet reach a affirmative conclusion in security of YupingfengJia wei.Conclusion:The factor of constitutions play a key role in paroxysm,evolution, prognosis of AR.With the development of the constitutions theory and enrich of the theory of constitutions combined with diease, the research combined constitution and AR is obvious lag.The research of clinical treatment should to be further strengthen to improve and promote the constitution theory. The clinical treatment regard Professor Wangqi’s differentiation of constitutions, disease, symptoms and signs theory as guideliness should to be promoted.The analysis to literature showed that the scholars pay more attention to clinical studies of fusing traditional Chinese medicine treatment, and evaluated the use of RCT in intervention of clinical studies.It suggests the improvement of clinical scientific research ability of clinical workers. The Chinese prescriptions used in the treatment are concentratly, that illustrate these prescriptions in the treatment of allergic rhinitis in the clinical efficacy of got the attention of many scholars. Chinese medicine therapy, combine traditional Chinese and western medicine therapy is still the AR clinical research emphasis in the future. The study in therapy of Tianjiu are mature, the curative effect is affirmably. The clinical study of Chinese medicine limited in nose to treat AR are immature,but the mode of administration is convenient, express, painless. And the patients will accept it with pleasure,so the study in this way should be developed.Most researchers lack of knowledge in scientificalness of RCT of AR. If to report specificationaly in accordance with the internationally recognized CONSORT, will improve the quality of the clinical study in AR, and provide reliable basis to promote the clinical application of Chinese medicine treatment for AR. The incidence of AR in children is well matched in adult. YupingfengJiawei is advantage compared with the normal western medicine in treat children AR. But it should be improved by further strict clinical trial to large sample.
