| Background:Overactive bladder(OAB) is a clinically troublesome disease and of high incidence, which greatly affects the quality of life of people.Since the name is put forward, proposed diagnostic criteria have been revised several times.Western Medicine is still in the exploratory stage of its pathogenesis. Now treatment includes behavioral therapy, drug, neuromodulation therapy, surgery, etal.Behavioral therapy and anticholinergic drugs are currently the most commonly used treatment. All can promote to ease OAB patients’condition.Their efficacy is mostly affected by the indications, patient’s compliance, sensitivity and other factors.Lacking of evidence of indications, safety and long-term efficacy, the clinically extensive use of Botulinum toxin injections, capsaicin perfusion, neuromodulation therapy, surgical therapy need more time in the future.According to OAB’smanifestation, theTraditional Chinese Medicinecan attributeit to difficulty in micturition or urinary incontinence, enuresis, stranguria and so on.Taking use of herbal,or acupuncture,or the combination of acupuncture with medicine in treating OAB is effective clinically. But these studies still exist some problems, such as ununified diagnostic criteria and complex manipulation.Researches indicating the functionmechanism of acupuncture in adjusting bladder function have achieved many progress in these years. The acupuncture treatment of OAB is safe according to meta-analysis.Objective:To further detect the clinical efficacy and sefety of electro-acupuncture on OAB,and to make sure whether the effect of electro-acupuncture deep needling at Zhongliao(BL33) and Huiyang(BL33)therapy for OAB is specific with placebo needle as control group.Method:A total of 22 cases were selected in five research centers including Beijing Hospital.13 cases were treated with electroacupuncture deep needling at Zhongliao(BL33) and Huiyang(BL33) in the treatment group, while 9 patients in the control group received placebo-acupuncture.Treatment was conducted with three sessions a week and every other day for each session, for four consecutive weeks. Patients assisted in filing in the Voiding Diary. If the collected data fit in a normal distribution, the t-test would be used to compare variables. If not,the Wilcox test would be used insteaded. All analyses, consisting of average urgency frequency, daily urinary frequency, night urinary frequencyand incontinence frequency before and after treatment, would be conducted in the SPSS programmer.20.0).Results:After 4 weeks treatment, the average urgency frequency, daily urinary frequency, night urinary frequency in the treatment group before and after treatment had statistically significant difference(P<0.05).The statistically significant difference in daily urinary freauency and nieht urinary frequency showed in the 2nd week.In terms of incontinence frequency, the improvement was not obvious in the in the treatment group. While the outcome measures in the control groupwere not statistically significant(P>0.05). The result showed thatelectro-acupuncture deep needling treatment for OAB had a satisfactory therapeutic effect generally, which was prompt and sustained,as well as excluded the placebo effect. No adverse reactions showed in this trail. Conclusions:Eletroacupuncture deep needling at BL32 and BL35 can effectively treat OAB,well adjust bladder function, greatly improve patients’symptoms(such as urinary urgency, urinary frequency and urge urinary incontinence)without definite adverse reaction. It is a method with minor selection of acupoint, simple to implement and worthy of further clinical promotion.Since this trial involved a small number of cases, without long-term efficacy observation, without contrast with masculine medicine, without analyzing the statistics of Overactive Bladder Questionnaire(OAB-q),it is necessary to wait the related multi-center,large randomized controlled clinical trials to confirm and consummate electro-acupuncture treatmentof OAB. |
