| Objective:Observing the clinical traditional syndromes, laboratory indexes and general curative effect of Chinese medicine (TCM) combined with western medicine treatment by Qingrelishiyiqiyangyinhuoxue of diabetes mellitus (DM) with urinary tract infection (UTI) to explore the advantage of TCM and lay the foundation for the further study and clinical experiences of Chinese medicine treatment of diabetes mellitus with urinary tract infection.Methods:Though randomized and prospective clinical study design, in-patient or out-patient patients from wangjing hospital between Dec 1st 2013 to Jan 1st 2015 were randomly divided into two groups, named experimental group and control group,34 patients for the experimental group to accept the treatment of traditional Chinese medicine combined with western medicine,the other 35 patients as the control group,treated with western medicine. Western medicine treatment includes intensive glucose treatment and antibiotic resistance to infection treatment. The treatment course is 2 weeks and follow-up examination is taken at 6 weeks after treatment. Comparing the differences between two groups in clinical indicators, bacterial clearance rate, recurrence rate and laboratory indexes before and after the treatment to observe the curative effect of integrated traditional Chinese medicine and western medicine treatment intervention in diabetes mellitus with urinary tract infection.Main results:1. Analysis of the age,sex,duration of diabetes mellitus,BMI, glycosylated hemoglobin,fasting blood glucose and postprandial blood glucose results showed;high-risk groups concentrate on 50-75 age, middle-aged and old women after menopause, DM course in more than five years, BMI greater than 23kg/m2,glycated hemoglobin level higher than 7%, FPG greater than 7.3mmol/L,2hPG greater than 8.7 mmol/L.2. Analysis of the distribution of pathogenic bacteria showed:the main pathogenic bacteria were gram negative bacteria, a total of 48 strains (80%), gram positive bacteria, a total of 10 strains (16.7%), fungus 2 strains, accounting for 3.3%. E. coli was the main pathogenic bacteria,a total of 41 strains was isolated,account for 68.3%,followed by staphylococcus aureus.a total of 6 strains isolated,accounted for 10% of total 4 in enterococcus strains,6.7% of the overall, klebsiella pneumoniae.a total of 4 strains isolated,6.7% of the overall.3. The comparison results between experimental group and control group showed:the total effective rate of experimental group was 93.3%,the total effective rate of control group was 86.7%,so the overall curative effect of the experimental group was significantly better than that of control group(P<0.05).Total effectiveness of experimental group syndrome was 96.7%,total effective rate of control group syndrome was 86.7%,so the experimental group of TCM syndrome curative effect was significantly better than that of control group(P<0.05). The improvements of four primary symptoms concluding frequency of urine nasty and short,urethra astringent pain, lassitude and asthenia,dry throat and thirst and two secondary symptoms concluding shortness of breath and lazyness of talking,feverishiness in palms and soles of the experimental group were significantly better than that of control group(P<0.01),the improvements of two secondary symptoms concluding urethral burning and lower abdomen painness of the experimental group were better than that of control group(P<0.05),the improvement of backache of two groups did not show statistical difference (P>0.05).4. Bacterial clearance rate of experimental group was 90%, bacteria clearance rate of control group was 76.7%,the experimental group was greater than the control group(P>0.05).The ecurrence rate of follow-up 6 weeks after treatment of experimental group was 11.1%, while the control group was 34.8%,two groups have statistically significant difference (P〈0.05).5.The results of urine WBC average decline before and after treatment between experimental group and control group showed,WBC average of the the experimental group was less than that of control group, two groups have statistically significant difference(P<0. 05). FPG average and 2hPG average droped before and after treatment in two groups,two groups have no statistical difference(P>0.05). The pertinence between urine WBC differentials and 2hPG differentials before and after treatment of experimental group and control group showed, the greater 2hPG differentials were,the greater urine WBC differentials were,so controlling 2hPG positively was significant for the control of infection.Conclusion:Qingrelishiyiqiyangyinhuoxue combined with antibiotic treatment of diabetes mellitus with urinary tract infection can effectively relieve the clinical symptoms,improve the bacterial clearance rate, reduce recurrence rate and the effect is better than antibiotic treatment. Integrated traditional Chinese and western medicine treatment can be the effective prevention and control for patients with diabetes mel1 itus with urinary tract infection,as well as improve the recovery rate of patients with urinary tract infection. |
