Comparative stody between CTA (Computed TomographyArterigraphy) and DSA (Digital Substration angiography)concerning the blood supply of brochogenic carcinoma.Purpose: To further investigate the blood supply ofbronchogenic carcinoma by the way of combining CTA and DSA, andtherefore, to provide theoretical evidence for interventional theraphy ofthe disease.Materials and metbods: 20 cases of bronchogenic carcinoma (14central tumors, 5 peripheral bumors) underwent CTA and DSA of bothbronchial atery and pulmonary artery of which 3 cases suspected ofaddihoned supply blood from intemal mammary artery underwent DSAand CTA of intemal mammary atery.ResultS: DSA showed that the tumor derived its blood supplyfrom bronchial artery (or both bronchial artery and internal mammaryartery), but not from pulmonary atery in all the cases. Pulmonary arteryin 15 cases were normal and the other pulmonary atery in 5 cases werenarrowed with different degree or occluded.CTA showed the same blood supply as on DSA. On CTBA tumorvasculature were seen within tumor in all cases. In 17 cases tumorvasculAn were located the peripheral part of tumor,while in 3 cases,inthe celltral pat of tUInOL CTA of intemal mammary artery showed tUmorvascultae were in the peripheral part of tumor. On the other hand,Ctw showed that plumory artery were closely attached by the mass in 6cases, the branch of artery were enclosed by the mass in 2 cases, andpulmonary atery were not seen aroud mass in the remaining cases.Tumor were not shown within tllmor The findings on CTA and DSAwere well correlate. The enhancement was 67.78f32.50Hu on CTBAand 1l .4l l3. l3Hu on CTPAtort0.05 ).Conclu8ion: (l) The bronchial artery is the main arterial suPPlingblood for lung cancer and other artery of systemic circulation may alsopAnciPate in the blood suPPly of the disease, the pulmonary rtry doesnot pAnciPate in the blood suPply of lung carcinoma. (2) Combinationof CTA and DSA helPs boer study the blood suPply of lung cancerCTA of bronchial atery and illtemal mammary artery show the theorvessels were mainly distributed in the periPheral part of tUmoL CTAindicate tha pulInonary atery don't participate in the blood suPply oflung carcinoma and some branches of pulmonary artery can be enclosedor attached by the mass. Cwi suggest that some branches of pulInonaryatery within the area of tUmor on DSA are actually normal brancheswithin lung tissue which overlaP with the mass and these blood vesselsaren't tUInor vessels. (3) Intra-arterial chemotheraPy or embolhation forlung cancer should be performed through the bronchial artery There'5no need of trans-pulmonary atery aPproach.
