| ABSTRACTBackgroundEssential Hypertension (EH) is a complex disorders,and is thought to result fi~m the interaction of environmental and genetic factors.The renninangiotensin system (RAAS) is an important component of blood pressure regulation, playing roles in salt-water homeostasis and vascular tone, and has been suspected to be involved in hypertension. Therefore, genes coding for components of this system are attractive candidates for the investigation of the genetic basic of EH.Left ventricular hypertrophy (LVH) , a major independent risk factor for morbidity and mortality from cardiovascular disease, and.is widely thought to be a consequence of left ventricular pressure overload, however, recent epidemiologic observation show that many subjects with LVI-I have normal BP level, suggesting that factors other than hemodynamic overload may concribute to the hypertrophy. These findings suggest the potential importance of genetic factors, including gene polymorphism of RAAS in determining the LVH.So It is important to exploring the possible association the gene polymorphism of RAAS with LVH.ObjectiveTo investigate the relationship of gene polymorphism of angiotensionconverting enzyme (ACE) , aldosterone synthase (CYP ii B2) with EH andLVH in hypertensive patients.Methods1.Blood pressure, body mass index, fasting blood glucose,serum cholesteral and triglyceride were measured in 136 patients with EH and 110 controls2.. Left ventricular mass and geometry were assessed with ECG and duplex sonography in hypertensive patients.3.DNA was extracted from peripheral blood leukocytes.-5-4. The insertion/deletion polymorphism of ACE gene was monitored bypolymerase chain reaction (PCR),and the --344C/T polymorphism ofCThllB2 gene was monitored by PCR and HaeIII restrictionendonuclease digestion methods.Resultsl. The systo!}c blood pressure, diastolic blood pressure and serumglycerinate level of hypertensive grouP were significatly higher thanthose of control while other cIinic features such as fasting blood glucose,and serum cholesterol level showed no difference betwCen EH grouPs andcontrols.2. Clinic features such as fasting blood glucose, index and serum lipid levelshowed no difference between LVH+ grouPs and LVH-grouPs.3. The analysis of individual genotypes of ACE gene showed no significamdifference in genotype or allele frequencies distriblltion on EH comParedwith contIols.4. CYP1lB2 gene showed a significan difference in genotyPe or allelefrequencies distribution on LVH+grouPs compared with LVH- grouPs(P<0.05), and the C allele of CYP11B2 gene was significanly morefrequent than the T allele in the LVH+ grouP comPared with the LVH-grouP.7. A significan difference in the CT+II combined genotyPe frequenciesdistribution on LVH+grouPs compared wth LVH- grouPs was showed(P<0.05).Conclusion- 6 -1.No Association between the polymorphism of ACE gene and essential hypknsion could be found.2.No Association between the polymorphism of CYP11B2 gene and essential hyptension could be found.3.A significant Association between the polymorphism of ACE gene and left ventricular hypertrophy in EH could be found, and I allele of ACE gene may be an independently risk factors for left ventricular hypertrophy in hypertensive patients.4.A significant Association between the polymorphism of CYPI 1B2 gene and left ventricular hypertrophy in EH could be found, and C allele of CYP 11 B2 gene may be an independently risk factors for left ventricular hypertrophy in hypertensive... |
