The Study of the Relationship Betweentnterleukin 1~ and Periapical PeriodontitisABSTRACT[Objective] OWe examined the relationship between clinical or radiographic findings and interleukin 1 l~ (IL-i ~3) by detecting its concentration in periapical exudate of Periapical Periodontitis in vivo. We also studied the therapeutic reaction of IL-i 1~ to Formocresol and Tinidazole in the course of root canal treatment. OTo investigate the therapeutic mechanism of Tinidazole and the changes of inflammatory and non-inflammatory cell number in rat periapical periodontitis, we examined the cell components of rat periapical periodontitis and their reactions to anti-anaerobe drug Tinidazole.PART I: Animal ExperimentThe Cell Components in Rat Periapical Periodontitisand Their Responses to Therapeutic Drugs[Methods and Materials] We selected Sprague-Dawley rats (8 weeks old) as experimental animals. 14 rats were divided randomly into T and C group. We used the first mandibular and maxillary molar of each rat as experimental teeth. After the pulps of experimental teeth were exposed from the occlusal surface, we injected 4% Tinidazole solution into the rats of T group and physiological saline solution into those of C group everyday until sacrifice by infra-peritoneal administration. At different periods (Od, 3d, 7d, 14d) after injection, the rats were sacrificed and the bones containing experimental teeth were taken out. Serial histological slices were prepared and stained with HE. Microscopical analysis including the cell number in periapical tissues was performed and tabulated according to histological criteria.[Results] Between 3 and 14 days of induced rat periapical periodontitis in C group, the fibroblast density in periapical tissues decreased significantly while the density of polymorphonuclear leukocytes (PM7Ns) increased quickly. PMN was the most prevalent inflammatory cell in periapical tissues in C group at all periods. The score of periapical inflammation index in C group was 2 at 3d and 3 at 7d while both of them in T group were 0.PART II: Clinica1 Study1. The Detection oflnterleukin-l 0 in Periapical Exudates ofPhoenix Abscess and Correlations with the CIinical Findings[Mcthods and Materials] We selected 40 permanent teeth randomly whichwere diagnosed as Phoenix Abscess or Chronic Periapical Periodontitis. TheperiaPical signs and symptoms were recorded. Radiographs were taken and theradiolucent areas were calculated. After the pulp cavity of each tooth wasexposed, the root canal was enlarged to 30# and a 30# paPer point was insertedinto it. Then the wetted length of the paPer point was measured immediate1y.The wetted part of the paper point was cut away and p1aced into an Eppendorftube containing 200ul samp1e diluter After centrifugation, the supernatant wascollected and stored at -70C until the measurement. The collected periaPicalexudate volume was calcuIated from the standard curve that had described therelationship between the wetted length and absorbed fluid volume onto 30#paPer poillt. IL-l0 concentration in periapical exudate was measured byenzyme-linked immunosorbent assay system. All the statistical analyses werefinished with Sigmastat software.[ ResuIts l The Phoenix Abscess group contained significantly higherconcentrations of IL- 16, higher volumes of exudates and larger radiolucent areasthan the Chronic Periapical Periodontitis group. The concentrations of IL-16 hada significan correlation with periaPical swelling and foul odor. The relationshipbetween samp1e volumes and clinical percussion was nluch close. There was asignificant relationship between IL-l0 concentrations and sample volumes inPhoenix Abscess group.2.
