BACKGROUND:Intestinal ischemia and reperfusion injury (I/R) is one of common tissue lesions in surgery practice .It not only can elicit topical lesion in alimentary tract,but also can lead to translocation of bacteria and endotoxin from intestine into systemic circulation ,which can elicit series of reactions in reticuloendothelial system(RES) ,and then release a great amount of associated intermediates and cytokines, sometimes even mutiple system organ failure can happen .The traditional Chinese composite medicine -Danbiqing granule is made from rhubarb,Dan-shen root( Radix Salviae Miltiorrhizae),dahurian patrinia herb,Chinese pulsatilla root (Radix Pulsatillae) and Glycyrrhiza.lt can activate blood by removing blood stasis and eliminate heat or toxicity.Clinical practice and animal experiments have demonstrated that it is used to treat acute cholangitis of severe type ACST,abodominal infection and some other acute abdomen. But the aspect of its protective effect on I/R injury has not been investigated.METHODS: In order to evaluate protective effect and the mechanism of Danbiqing granule in different dosages on I/R injury.Rabbits were randomly divided into five groups: the shamgroup, model group, and prophylactic group of Danbiqing granule(0.3g/kg;1.2g/kg;3.6g/kg).. the superior mesenteric artery clamped for 60 minutes and reperfused for 6 hours as ischemia / reperfusion model was established .Danbiqing granule was given by oral injection to the rabbits of prophylactic group for 3 days before operation. In the sham group, superior mesenteric artery was only isolated without clamping.Blood samples were collected from common carotid artery of each rabbit, before occlusion,at the end of occlusion, and 0.5,2,6hour after reperfusion. Melondialdehyde (MDA), superoxide dismutase (SOD)activity , nitrogen monoxide(NO), endotoxin in plasma are measured.And after reperfuse 6 hours, rabbits were sacrificed, the tissues of lung,kidney and intestine were removed, then the assay of their myeloperoxidase (MPO) and MDA was done.RESULTS: (l)MDA content: Compared with sham group,MDA content of model group increased after ischemiajt reached the peak at 0.5 hour after reperfusion ,then it decreased gradually(P<0.01). Compared with model group,Danbiqing granule could decrease MDA content, the large dosage of Danbiqing granule could decrease MDA content significantly, the middle dosage could decrease it comparatively weak,the small dosage had the least effect.(2)SOD activity: SOD activity of model group increased after ischemia and reperfusion, it reached the peak at 0.5 hour or so after reperfusion, and then itidecreased gradually.compared with sham group, it changed insignificantly.Compared with model group ,Danbiqing granule could increase SOD activity.the large dosage could increase it remarkably.the middle dosage could increase it weakly,and the small dosage had the least effect of increment. (3)MPO content: Compared with sham group ,pulmonary MPO and intestinal MPO content increased significantly in model group (p<0.01),and pulmonary MPO content was higher than intestinal MPO content.Compared with model group,pulmonary MPO and intestinal MPO decreased significantly in prophylaxis group .(4) Endotoxin level: Compared with sham group ,plasma level of endotoxin increased significantly in model group,compared with model group, the plasma level of endotoxin in prophylaxis group decreased significantly .(5) NO content: Compared with sham group, nitrogen monoxide content during ischemia remained unchanged practically , But after reperfusion it increased significantly( P<0.01).Compared with model group,Danbiqing granule could decrease nitrogen monoxide content significantly. The largest dosage of Danbiqing granule had the largest effect. (6)morphology: After ischmia and reperfusion ,lung,intestine,liver tissue in model group are injuried significantly, compared with model group, prophylaxis group can attenuate the injury.CONCLUSIONS: During intestinal ischemia andreperfusion,contents...
