Objective to study the methods of a microsurgical craniotomy via keyhole approach and evaluate its safety and efficacy. Methods supraorbital, subtemporal, posterior cranial fossa, frontal, keyhole approaches were performed for pituitary adenomas, craniopharyngiomas, meningiomas, aneurysms in anterior cerebral circulation, gliomas and acoustic neurinoma, optic nerve sheath meningioma et al respectively. Results All 15 neoplasms were totally or subtotalty removed. 5 aneurysms were cliped. A case of trigeminal neuralgia and a case of facial tics were cured by neurovascnlar decompression. A case of acute extradural hematoma and a case of AVM were removed. Most of patients recovered very well and fast after operations. None of the severe complications related to surgical approach could be seen. Conclusion keyhole approach is safe and effical. It deserves application.
