| Objective: To elucidate multidrug-resistance mechanism and prognosis of colorectal carcinoma by investigating expressions and relationship of multidrug resistant gene P-gp ,Topo II ,bcl-2 and c-erbB-2. The aim was to estimate the prognosis of colorectal carcinoma and to offer a valuable opinion on deciding chemotherapy plan.Methods:The expressions of P-gp ,Topo II ,bcl-2 and c-erbB-2 in 64 cases of colorectal carcinomas were investigated using immunohistochemical SP method. The relationship was also analysed between the results and clinicopathologicai parameters,prognosis.Result: (1) Of the 64 cases colorectal carcinomas ,28(43.75%) were positive for P-gp, 18(28.13%) for Topo II ,41(64.06%) for bcl-2, and 26(40.63%) for c-erbB-2. The co-expression of P-gp and c-erbB-2 was 15(23.44%).(2)A significant relationship was found between P-gp expression and c-erbB-2 expression (r=0.2631 p < 0.05) (3)There were no marked significances relating to sex ,age ,clinical stage, histologic type, depth of invasion and metastasis of lymph nodes of P-gp expression, Topo II expression ,bcl-2 expression and c-erbB-2 expression(p > 0.05) (4)The analysis of single factor revealed a significant relationship between c-erbB-2 expression and the prognosis of patients that positive c-erbB-2 expression had a shorter average survival time .The correlation with prognosis was analyzed by Cox proprotion hazard model .It showed that c-erbB-2 expression was independently associated with patient survial length (P=0.0013), while the depth of tumor invasion impacted patient survival length, the more deep, the more dangerous .Conclusions:The experiment shows that the combinative effect of P-gp, Topo II ,bcl-2 and c-erbB-2 may be the chief mechanism of multidrug resistance of the patients with colorectal carcinoma .The assay of P-gp, Topo II ,bcl-2 and c-erbB-2 in patients with colorectal carcinoma is important and has instructive significance to estimate prognosis and work out a chemotherapy plan.
