| Objective: Acute necrotizing pancreatitis (ANP) is a common surgical emergency with many complications. The pathogenesis of pancreatitis is not yet completely ascertainment. Abnormal ectopia enzyme activation is the key step for pancreas autodigestion. Until now there is no pathway for terminate or stop such autodigestion caused by avtivated enzyme. Recently, many drugs are applied to clinical, and provide many choices for treatment of pancreatitis. Unfortunately, there are too expensive but unreliable. So it is desire for both patient and doctor to find a effective and cheap drug. The aim of this study was to evaluate the feasiblity of chlorpromazine on treatment of acute necrotizing pancreatitis rats.Material and Methods: 120 healthy female SD rats were randomly divided into three groups: Group I normal control (NC group, n=30); Group II acute necrotizing pancreatitis (ANP group, n=45); Group III chlorpromazine treatment (ANP+CPZ group, n=45). ANP modle in group III and II was induced by the injection of 5% sodium taurocholate into pancreatic duct with the dosage of 1 ml/kg and rate of 0.2ml/min, group I was injected the equivalence saline with the same method as a control of sham operation. 0.4% Chlorpromazine was administered to group III Intraperitoneal with the dosage of 0.25ml/100g immediately after the model was made. The drug was supplied at 24h\ 48h repeatedly. Normal saline was given to Group IIin the same way and the same time. The animals were sacrificed at 24. 48. 72hour under sterile circumstance. The blood samples were collected for analysis of serum amylase(AMY). phospholipase A2 (PLA2). interleukin-6 (â… L-6). Pancreas, lung, liver and kidney were collected for pathologic examination. Spormann score was undertook for evaluation the damage of pancreas.Results: 1. The 72 hours survival rate was 100%. 71%. 57.8% in group â… . â…¡. â…¢ respectively, that of group â…¢ was higher than that in group â…¡. 2. Serum amylase in group â…¡ and group â…¢ increased highly and decreased gradually. The serum amylase level was lower in group â…¢ than that in groupâ…¡ at 24. 48. 72h (P<0.05),but still above the normal control level (P<0.01). 3.The changes of PLA2 was similar to AMY. The PLA2 levels of group â…¢ were lower than that of group â…¡ (P<0.01). 4. â… L-6 increased highly in both groups. â… L-6 level decreased obviously in group â…¢ when it was still higher in group â…¡. There was a significant difference between the two groups (P<0.05). 5. Pathologic changes: The structure of pancreas was almost normal in group â… . A large area necrosis and hemorrhage in pancreas was observed in the group â…¡, with a large amount of ascites and calcification spots. The normal pancreas was distributed sporadic. The surface of liver and lung was congestion, sometime necrosis could be observed. Compare in group â…¡ , the pathologic changes of group â…¢ were lighter. The Spormann score of group â…¢ was lower than that in group â…¡ (P<0.05).Conclusion: Chlorpromazine has shown therapeutic effects on treatment of ANP and it may provide a new feasible treatment approach for ANP. |
