A Study On The Prethrombotic State In Children With Acute Viral Encephalitis

Objective:The authors tested not only the extent of vascular endothelical cell lesion and the platelet activation but also the change of fibrinolysis system and platelet parameters in order to investigate whether there is prethrombotic state in acute viral encephalitis so that the clinical physicians can explain the pathophysiological change more wholly.The results of research also can instruct the clinical doctors how to deal with patients correspordingly so that they can improve their prognosis.Methords:Platelet parameters-platelet count (Plt)and mean platelet volume(MPV),von Willebrand factors (VWF)which reflects the extent of the vascular endothelical cell lesion,the granule membrane protein- 140(GMP-140)-the activation-specific antigens on the platelet surface which reflects the extent of platelet activation and the D-dimer which reflects the change of fibrinolysis system were measured in 60 children of whom 30 had mild acute viral encephalitis and 30 had severe acute viral encephalitis as well as 30 sex- and age- matched controls without familial bleedy and coagulative disease after clinic and test screeing and not using the drugs which can influence the fibrindytic andcoagulation system.The plasma concentration of VWF D-dimer were detected by monoclonal antibody CLISA.The platelet parameter and the expression of GMP-140 on platelet surface were determined by automatic blood cell analysis apparatus and flow cytometry analysis apparatus separately during the acute and recovery stage.Results:!.The platelet count had no marked difference in children with mild acute viral encephalitis during acute and recovery stage compared with controls(P>0.05).The platelet count hadn't different from controls in children with severe acute viral encephalitis during acute stage ,but they were gradually increased during recovery stage and have marked difference from controls(p<0.01). 2.The MPV had no marked difference in children with mild acute viral encephalitis during acute and recovery stage compared with controls.The MPV increased sharply in children with severe acute viral encephalitis during acute stage than controls(P<0.01)and they gradually decreased during recovery and have no difference from controls(P>0.05).3.The expressive rate of GMP-140 on platelet surface and the plasma VWFwere significantly higher in children with acute viral encephalitis than controls.The difference between the mile and severe acute viral encephalitis was also significant(P<0.01).During the recovery stage,the expressive rate of GMP-140 on platelet surface and the plasma VWFhaddecreased sharply and had no difference from controls in children with mild acute viral encephalitis,but they were also higher than controls in children with severe acute viral encephalitis(P<0.05).4.The plasma D-dimer had no marked difference in children with mild acute viral encephalitis during acute and recovery stage compared with controls(P>0.05). The plasma D-dimer increased sharply in children with severe acute viral encephalitis during acute stage than controls(P<0.01)and they gradually decreased during recovery but they still have marked difference from controls(P<0.05). 5.There were sigificant positive correlations during the expressive rate of GMP-140 on platelet surface and the plasma VWF in children with acute viral encephalitis. 6.There weren't correlations during the expressive rate of GMP-140 on platelet surface and the plasma D-dimer in children with mild acute viral encephalitis,but There were sigificant positive correlations during the expressive rate of GMP-140 on platelet surface and the plasma D-dimer in children with severealiti acute viral enceph s.Conclusions:1.The prethrombotic state may occurs in patients with acute vrial encephalitis .Testing the change of platelet counts the expressive rate of GMP-140 on platelet surface the plasma VWF and D-dimer can find the prethrombotic state immediately so that clinical physicians can treat patients more effective and improve the prognosis.2.The expressive rate of GMP-140 on...
