Background and objectiveHepatic ischemia-reperfusion injury ( HIRI ) is the main problem during the operation of hepatolobectomy or liver transplantation. It is the prime reason of liver function failure after liver operation. Now the mechanisms of HIRI are considering : microcirculatory disturbance ; oxygen free radical ; calcium paradox ; cellular factor and inflammatory factor , et al . The oxygen free radical may be the most important factor above all.The Prostaglandin E1 ( PGE1 ) is proved good effects on HIRI. And Liposomal Prostaglandin E1(Lipo-PGE1) is a new medicine which capsules the normal Prostaglandin E1 with liposome. It has better stability , targeting , slow-releasing and safety. We want to study the Lipo-Prostaglandin E1 protecting against hepatic ischemia injury on liver function and oxygen free radical.Material and MethodsSixteen rabbits was random divided into two groups. Each was made a model of hepatic ischemia-reperfusion injury : operation of blocking the hepatic portal 45 minutes for hepatic ischemia , then recovering the blood supply 1 hour for reperfusion. One group received a dose of Lipo-PGEi 3ug/kg intravenous injection into portal vein system before the hepatic portal blocking. The another group received the same dose of normal saline. Blood ALT , AST , LDH were tested to observe the liver function changing. Tissue MDA, SOD were tested to observe the oxygen free radical deleting.The microscope and electron microscope for comparative pathologic changing were made too.ResultWhen the hepatic portal was blocking 45 minutes , the results of the Lipo-PGE1 group : ALT 376.0 ± 46.7U/L , AST 1344.9± 53.0U/L , LDH 2183.4 ±122.7U/L , MDA 0.800 ±0.069nmol/mgprot , SOD 157.72 ±4.52NU/mgprot ; the results of NS group : ALT 499.5±25.3U/L , AST 2044.8±13.6U/L , LDH 3661.9 ± 203.0U/L , MDA 1.122±0.107nmol/mgprot, SOD 127.14±3.85NU/mgprot. (P<0.01)When the hepatic portal was recovering 1 hour , the results of the Lipo-PGEi group : ALT 1435.6±29.0U/L , AST 1501.8±63.5U/L , LDH 3252.0± 179.6U/L , MDA 1.561±0.086nmol/mgprot, SOD 126.81 ±5.01NU/mgprot ; the results of NS group : ALT 2513.3 ± 63.0U/L, AST 2356.3 ± 102.7U/L, LDH 3966.4 ± 222.1U/L, MDA 2.244±0.133nmol/mgprot, SOD 83.54 ±6.22NU/mgprot. (P<0.01)The pathologic changing was found that the hepatic lobules of the Lipo-PGEi group was better than the NS group , and the degree of the cellular edema of the Lipo-PGEi group was less than the NS group at the time of hepatic portal blocking 45 minutes and recovering 1 hour.The electron microscopy showed that the appearance of mitochondrion and endoplasmic reticulum of Lipo-PGEi group was better than the NS group at the time of hepatic portal recovering 1 hour.ConclusionThe Lipo-PGEi can protect against hepatic ischemia injury, including decreasing the oxygen free radical, protecting the cell structure and improving the liver function.
