| Objective: Gastric carcinoma is one of common malignant tumor in our country, the patients died from Gastric carcinoma are more than those died from other Malignancies. Chemotherapy is an important therapy method for gastric carcinoma. Fliorouracil liposome is the principal part of co-Fliorouracil liposome's parenteral solution(139-3), in which ginseng polysaccharide has been put into as immunopotentiator. It has the characteristics of slowing releasion and targetive. This drug injected by vein has done well in therapy of carcinoma, while there has no reports about using it in the way of abdominal cavity perfusion.In this research, the progressing gastric cancer patients were given co-Fliorouracil liposome parenteral solution by two ways vein injection and abdominal cavity perfusion), and contrasted with those patients who were given Fliorouuacil parenteral solution (5-Fu) and control groups before operation. We observed the effect on leukocyte,red cell,blood platelet and the function of liver and kidney.After detect cell cycle, proliferation index and the expression of Bcl-2, Bax gene of gastric carcinoma by using the method of FCM. Westudied the poison side-effect and the effect on therapy of cancer. We also have discussed the difference of curative effect on gastric cancer between using 139-3 and 5-Fu.It suggests that clinic therapy can be based on.Methods: 60 cases of progressing gastric cancer patients who are from 50 to 70 years old were Selected and randomly divided into 5 groups, Each group has 12 cases,ie:139-3 abdominal cavity perfusion group and vein injection group ,5-Fu abdominal cavity perfusion group and vein group,control group.The medication groups are given 5-Fu with 8.33mg/kg and 139-3 with 1.33mg/kg.Drugs are all dissolved into 500ml 0.9% Nacl liquid.Each group began to use drug before 2 days of operation, once a day for three times;Abdominal cavity medication group's drug liquid was infused to Abdominal cavity by puncturing at spot of appendix and vein medication group's drug liquid was injected into vein. Before given drugs ,all patients were examined of blood and liver and renal function. At same time, local-irritation and ill effect in gastrointestinal were observed. We collected two fresh constitution specimens from gastric carcinoma. one of them were detected APOI, cell cycle, PI and the expression of Bcl-2,Bax with flow cytometry and fluorescence stained with acridine orange for observing their apoptosis form. Another were observed the change of pathologic under microscop.3 days after operation, we taked blood from the patients to detect their RT of blood ,liver andrenal function, calculate the deference to those of preoperation.Results: 1 The effect of local-stimulation and ill effect in different medication methods. Only two casein in abdominal cavity perfusion group had local-stimulation, such as abdominal pain, abdominal distention, abdominal wall spasm etc. All groups had no peritonitis symptom and ill effect in digestive tract. There were no significant deference in WBC, RBC ,PLT, ALT, AST ,BUN and Cr among all of the groups(P>0.05).2 The effect on gastric caicinoma cell apoptosis and the expression of Bcl-2 gene and Bax gene by different medication methods.2.1 Medication group's APOI and the expression of Bax were higher than that of control group(P<0.01), and its expression of Bcl-2 were lower than that of control group(P<0.01).2.2 As we had given the drugs in different ways, APOI of 5-Fuip was significantly higher than that of 5-Fuiv(P<0.01), APOI of 139-3ip group is strikingly higher than that of 139-3iv group's too(P<0.01). 5-Fuip group's expressions of Bcl-2 were decreasing tendency to that of 5-Fuiv groups, the expression of Bax was increased, but not significantly (P>0.05). Contrasted with 139-3iv group, expressions of Bcl-2 was tendency to decrease, 139-3ip group's expression of Bax was tendency to increase, but not significantly too(P>0.05).2.3 With different drugs in the same way, APOI of 139-3ip's is strikingly higher... |
