Objective:To evaluate the neuroprotective effect of topiramate (TPM) to neuronal injury caused by excitotoxic glutamate or epileptic seizure. Methods:Postnatal wistar rats (within 24 hours) culture neuronal cell was performed. Different concentrations of TPM (50,100,250,500,1000μM) and glutamate (50 and 100μM) were added into cultured neurons respectively according to experiment requirement for 24 hours, survival rate of cultured neuronal cell was detected with Mattson method. In 43 patients groups, before and after received TPM, 5ml blood was drawn in 24-48h after seizure. After centrifugation, the serum samples were stored at -80℃ until analysis. Before seizure, the levels of the neuron-specificenolase(NSE) after received TPM was compared with the baseline levels before received TPM.Results:Results showed under the condition of 50μM sodium glutamate (GLU), 50-1000μM TPM could increase the survival rate of neuron from 30.9% to 40.4-71.9%; under the condition of 100μM GLU, 50-1000μM TPM could increase the survival rate of neuron from 22.2% to 34.7-68.1%. The neuroprotective effect which caused by this administration model that was neurons pre-exposed in TPM, then GLU treated on neurons was better than the other administration model that was GLU treated on neurons primarily, TPM treated on neurons later (P﹤0.05, respectively). The neuroprotective effect of TPM was increasing accompanied with the growing dose of TPM. The mean peak NSE concentrations before and after received TPM were 15.1±1.4μg/L and 8.0±1.3μg/L (p<0.05), respectively.Conclusion:There is protective effect of TPM to neuronal injury induced glutamate cytotoxicity and epileptic seizure.
