Backgrounds and objectives: Ovarian response and the endometrial receptivity are crucial steps to the success of IVF-ET. During the process of IVF-ET, the response of ovary cannot be predicted exactly, which causes the probable unsuitable treatment protocol and too little ova retrieved. Both of that mightl lead to too little number of embryos and the cancellation of the treatment cycle, whereas, it might cause the ovarian hyperstimulation syndrome (OHSS) , which will influence the implantation, even make end of the pregnancy or endanger the life. Improper transfer occasion can lead to the failure of implantation, if too many embryos are transferred, the rate of multi-pregnancy will increase, on the contrary, it will lead to the failure of pregnancy. These two factors mentioned above bring to the unsatisfying outcome of ART. In the past, age and basal FSH level are used to predict the responsiveness of ovary, but the true age is not always consistent to the age of ovary and the basal FSH level cannot reflect the ovary response exactly. So there is not yet any reliable and accepted standard to selecting appropriate treatment protocol and initial dosage of gonadotropins(Gn). The endometrial thicknessand type were used to assess its receptivity. Howerver, more and more facts showed that they are not considered and it is the blood flow of uterine artery that determines the fate of implantation in IVF-ET. Transvaginal Color Doppler Sonography(TV-CDS) can be used to investigate the physiological and pathological minor vascular changes in the ovary cycle and is valuable to the clinical applications and researches. But the research on TV-CDS is only limited to the ovarian and uterine hemodynamic in spontaneous cycles and the diagnosis, judgment of treatment of PCOS domestically. In this research, ovarian stromal and uterine artery blood flow were monitored with TV-CDS firstly in our country, which offered useful information to predicting ovarian response, selecting treatment protocol, avoiding the occurrence of OHSS and enhancingg the pregnancy rate. Additionally, it is significant to assessing the endometrial receptivity, and then deciding the optimal transfer occasion, the number of embryo transferred and promoting the pregnancy rate, lessening the frequency of multi-pregnancy.Patients: One hundred and sixteen infertile patients undergoing IVF-ET treatment were recruited from the Reproductive Center of The First Affiliated Hospital of Zhengzhou University from November 2001 to April 2002. Grouping according to age, infertile duration, basal FSH level, endometrial thickness, days of Gn administrated, number of ova retrieved, PCOS, OHSS, PI, RI and pregnancy referred the result. The main reasons of infertility are Fallopian tube occlusion and male azoospermia. Systemic diseases are excluded in all patients. There are no ovarian cysts or the diameters of cysts are less than 10mm in the basal state. COH protocols used in this research are short protocol and standard corpus lutein stage protocol.Methods: 1 The time-resolved fluoroimmunoassay was used to measure endocrine level of basal state and on the day of hCG administration; 2 TV-CDS was used to monitor the blood flow of uterus and ovary; 3 The methods of IVF-ET referred the routine ways of our center.Results:1 Monitoring the hemodynamics of ovarian stroma in COH 1. IThe change of ovarian stromal hemodynamicsThe blood stream resistance shows the decreasing trend after treatment with Gn, but there is an increase after the day of hCG administration (Dhco). Then there is another decrease on the day of ET (DET).1.2 The relationship between the ovarian responsiveness and ovarian stromal blood flowOvarian stromal blood flow PI, RI show no correlation with the number of follicles on D2, DHCG and the total ova retrieved (P<0. 05) ;PSV does not correlate with the number of follicles on D2, but do with the follicles on DHCG and the total ova retrieved.
