Osteosarcoma is the most common primary malignant tumors of the bone. Recently, because of the rapid development of chemotherapy, the cure rate of osteosarcoma increased gradually, up to more than 50%. But there are still 20-40% patients died of metastasis. 80% patients had undeveloped metastatic foci meanwhile they were diagnosed. So the chemotherapy is important to the treatment of osteosarcoma besides the surgical operation. The key of chemotherapy is the sensitivity of the rumor cells to the chemotherapy Pharmaceuticals. Multidrug-resistance (MDR) is the major cause of many tumor chemotherapy defeat. So it is important to investigat the mechanism of osteosarcoma MDR deeply.We assayed the expression of P-gp, GST- π and Topo Ⅱ of 94 cases of human osteosarcoma by ABC immunohistochemistry, for clinical formulation individual chemotherapy scheme and lengthening of the disease-free survival rate.The histological morphology of osteosarcoma is verity. In order to observe the ultrastructure of osteosarcoma deeply, we observed 9 cases of primary osteosarcoma by electron microscope and discussed the significance of the ultrastructure of various cells of osteasarcoma. The results are as follows:1. P-gp positive granules were brown or yellow-brown, located on osteosarcoma cell membrane and some cytoplasma, The positive rate is 45.7%(43/94 cases), There was no significante between the expression and age,sex, tumor size, Price grade and Dahli's typing (P>0.05). But the P-gp expression was significantly related to WHO classification. The P-gp expression of intraosteal well differential type was lower than that of other three types (P<0.05).2. GST- n positive granules located on cytoplasma and some nucleus. The positive rate is 71.3%(67/94 cases). There was no significance between the GST- n expression and age, sex, tumor size, Price's grading, Dahlin's typing and WHO classification (P>0.05).3. Topo II positive granules located on nucleus and some cytoplasma. The positive rat is 34.9%(32/94 cases). There was no significance betweenthe Topo II expression and age, sex, tumor size, price's grading and Dahlin's typing. But the expression was significantly related to WHO classification. The Topo II expression of intraosteal well differential type was higher than that of other three types (P<0.05).4. The expression of P-gp and GST- n was significantly related to prognosis (P<0.05), but the expression of Topo II was not significantly related to prognosis (P>0.05).5. Ultrastructurally, we observed four types of osteosarcoma cells. They are osteoblast-like cells, chondrablast-like cells, fibroblast-like cells and multinuclear giant cells. They have their characters of their own. Close connection can be seen between fibroblast-like cells.6. The round cell osteosarcoma which few reported Ultrastructurally revealed that the cell is round or ovid, has 1-2 nucleus, margination of the heterochromatin, prominent nucleolus, few organells, irregular and small mitochondria. The charactristic features of round cell osteosarcoma is that some cytoplasma has intracytoplastic inclusions which have banded collagenmicrofiber and calcium.7. Hydroxyl apatite crystallizes can be seen in the osteosarcoma interstitial, and feather form change can be seen in the crystalling concentrated areas.The conclusions are as follows:1. The P-gp over-expression of mdr-1 gene is a direct evidence of osteosarcoma MDR. It is one of the basic causes of osteosarcoma chemotherapy defeat. P-gp played an important role in the osteosarcoma MDR. It will have important role in the osteosarcoma MDR. It will have implication value to invert the "outside pump" action of P-gp in the inverting of osteosarcoma MDR.2. The over-expression of GST- n often occurred in some difficult-treated tumors. It reflected the intrinsic MDR of some tumors. Our research revealed that there were 67/94 cases osteosarcoma having diffirent GST- n expression. It inferred that osteosarcoma is a kind...
