Endometriosis (EM) is a common disease of gynecology, with the incidence in the general population being raised. EM can cause sterility, and the rate of recurrence is high. EM is common benign condition, though its pattern of growth is similar to malignant disease. It can affect seriously the health of body and mind in women due to the specific property of easy recurrence and young patients. There are four theories proposed for the origin and pathogery of EM : Sampson theory, hereditary theory,dioxin's theory and immunological change of internal environment.But no single theory can account for the location and growth of the ectopic emdometrium in all cases of EM.With the development of molecular biology and immunology,the basic investigation of EM was increasingly deeper.It revealed that the relation between episode andimmunological function,pathological changes of EM, and suggested EM as a auto-immune disease. Relative experiment indicated patients' internal environment adjustment was abnormal.This test provide the evidence for the pathogenesis of EM by testing the levels of interferon (IFN- ),transforming growth factor 1 (TGF- 1) and vascular endothelial growth factor (VEGF) in sera and Peritoneal Fluid (PF) ,and exploreing the correlation between these cytokines (cks) and EM. In this study, in order to observe it treating effect of treating patients with EM in stage I / II .They had been treated by microwave under laparoscope and followed.Part I Measurements of IFN- , TGF- 1 and VEGF in Sera and Peritoneal Fluid of Patients with EMIn this study, IFN- Y 1 TGF- 1 and VEGF contents of 56 cases with EM (30 cases in stage I/II,26 cases in stage III/IV),and 25 cases without EM. Results: (1) The median concentration of TGF-13 , in sera and PF of patients withEM were significantly higher than those in controls (P<0.05); The level of IFN- y in sera is not different between those in stage I / II and in stage III/IV .There were no different between those in stage I / II and in stage III/IV. The level of TGF- 1 in PF was significantly higher than that in sera in each group (P<0.001). (2) The median concentration of IFN- y in sera and PF of patients with EM were significantly lower than those in controls (P<0.05), The level of IFN- y in PF in stage III/IV of EM group was significantly lower than that of in stage I /II(P<0.05). There were no different in other groups. The level of IFN- y in PF was significantly higher than that in sera in each group(P<0.001). (3) The level of VEGF in sera and PF of patients with EM in each stape was significantly higher than that in controls (P<0.05). The level of that in stage I / II was significantly higher than that in stage III/IV(P<0.05).The median concentration of VEGF in PF was significant higher than that in sera in each group. Condusion: The level of IFN- y was reduced,but the levels of TGF- 1 and VEGF were rised. IMMunologic response was superiority in Th2, and reducing pathological Th2 can make Thl/Th2 equilibrate, might be a pathway of the treatment of EM.Part II To treat and abserve the women with infertility of endometriosis by microwave under leparoscopeIn this study, we selected 24 cases of stage I / II, excluding the others infertile diseases, their spouses were normal.These patients were treated by microwave under laparoscope.Rusults: Fifty percent of 24 cases were pregnant during 10 months after operation, the median concentration was 6.9 months. There was not any conplication of operation. Conclusions:Microwave could treat stage I / II of EM, ameliorate internal environment, and improve the rate of cyesis.
